Predictors of Treatment Outcomes in Anxious Children Receiving Group Cognitive-Behavioural Therapy: Pretreatment Attention Bias to Threat and Emotional Variability During Exposure Tasks

Background and objectives: Pretreatment attention bias towards threat stimuli has been shown to predict treatment outcomes following exposure-based treatments. The extent of emotional variability experienced during exposure therapy has also been found to predict better treatment outcomes in anxious adults. The present study examined whether pretreatment attention bias towards threat stimuli and greater emotional variability during exposure activities were associated with stronger treatment outcomes in anxious children receiving group-based cognitive-behavioural therapy (CBT). Methods: Twenty-six anxious children completed a visual probe task with emotional faces, followed by a 10-week CBT program in a group format. Children completed weekly within-session exposure activities during the last 5 weeks of group CBT. Results: Pretreatment attention bias towards threat stimuli, greater emotional variability and within-session habituation during exposure activities were significantly associated with reductions in clinician- and/or parent-rated anxiety symptoms after the 10-week CBT program. Treatment responders had significantly higher peak emotional distress ratings during exposure activities. However, threat attention bias and within-session exposure measures were not significantly related. Conclusions: Pre-existing individual differences in attention bias to threat cues and the degree of emotional reactivity experienced during exposure activities are both important independent predictors of treatment outcomes for anxious children receiving group-based CBT.

Abstract Anxiety disorders are common among adolescents and lead to poor long-term outcomes. Cognitive Behavioural Therapy (CBT) is an evidenced-based intervention for adolescent anxiety disorders, but little is known about whether and how parents should be involved. This systematic review evaluated how parents have been involved and associated treatment outcomes in studies of CBT for adolescent anxiety disorders. Electronic systematic searches were conducted in PsycINFO, Embase, CINAHL, Medline, AMED databases, to identify studies investigating CBT for adolescent anxiety disorder(s) that included parents in treatment. Twenty-three papers were identified. Parents were involved in treatment in a number of different ways: by attending separate parent sessions, joint parent–adolescent sessions, or both, or through provision of a workbook while attending some adolescent sessions. Content varied but was most typically aimed at the parent developing an understanding of core CBT components and skills to help them manage their adolescent’s anxiety and avoidance. Treatment outcomes indicate that CBT with parental involvement is an effective intervention for adolescent anxiety disorders; however, it is not possible to draw conclusions regarding whether parental involvement (generally or in any particular form) enhances treatment outcomes. Poor reporting and methodological issues also limit the conclusions. Further research is required to identify whether there are particular types of parental involvement in CBT that bring clinical benefits to adolescents with anxiety disorders generally, as well as in particular circumstances.

Two of the most popular psychotherapeutic approaches to treat depression are cognitive-behavioural therapy and psychodynamic psychotherapy, yet little consensus has been reached concerning which therapy is most beneficial for the treatment of depression. A review of the literature revealed that, while cognitive-behavioural therapy and psychodynamic psychotherapy are the most effective psychotherapeutic modalities for the treatment of depression, evidence suggests that neither of these modalities is superior to the other. Furthermore, multiple issues plague the studies investigating these treatments. Efficacy and effectiveness are often confounded, while rates of remission and response are often far less than might be expected from such highly regarded and widely used treatments. Severity of depression appears to moderate treatment outcomes, yet many studies overlook this, while the impact that the aetiology of a patient’s depression has on treatment outcomes is largely ignored in the literature. Additionally, a majority of studies have focused on therapies of short duration, which often have poor follow-up results. Finally, mechanisms of change in the treatment of depression have been ignored to a large extent, but there is some evidence that non-specific therapeutic factors may be more important than specific therapeutic techniques in producing positive treatment outcomes. These issues need to be closely examined and resolved if researchers and clinicians are serious about optimising treatments, improving outcomes, and adequately addressing the serious problem of depression.
