Risk assessment of waterborne protozoa: current status and future trends

Throughout the past decade much research has been directed towards identifying the occurrence, epidemiology, and risks associated with waterborne protozoa. While outbreaks are continually documented, sporadic cases of disease associated with exposure to low levels of waterborne protozoa are of increasing concern. Current methodologies may not be sensitive enough to define these low levels of disease. However, risk assessment methods may be utilised to address these low level contamination events. The purpose of this article is to provide an introduction to microbial risk assessment for waterborne protozoa. Risk assessment is a useful tool for evaluating relative risks and can be used for development of policies to decrease risks. Numerous studies have been published on risk assessment methods for pathogenic protozoa including Cryptosporidium and Giardia. One common notion prevails: microbial risk assessment presents interesting complications to the traditional chemical risk assessment paradigm. Single microbial exposures (non-threshold) are capable of causing symptomatic illness unlike traditional chemical exposures, which require a threshold to be reached. Due to the lack of efficient recovery and detection methods for protozoa, we may be underestimating the occurrence, concentration and distribution of these pathogenic micro-organisms. To better utilize the tool of microbial risk assessment for risk management practices, future research should focus in the area of exposure assessment.

The 1st International Conference on Microbiological Risk Assessment: Foodborne Hazards was held in July 2002. One of the goals of that conference was to evaluate the current status and future needs and directions of the science of microbial risk assessment. This article is based in part on a talk presented at that meeting. Here, we review the types of food consumption data available for use in microbial risk assessments and address their strengths and limitations. Consumption data available range from total population summary data derived from food production statistics to detailed information, derived from national food consumption surveys, about the types and amounts of food consumed at the individual level. Although population summary data are available for most countries, detailed data are available for a limited number of countries and may only be available in summary format. Despite the relatively large amount of detailed information collected by these national surveys, information crucial to microbial risk assessments, such as the specific types of foods, the eating patterns of susceptible populations, or an individual's propensity for consuming high-risk foods (e.g., eating undercooked hamburgers, raw shellfish, or temperature-abused foods), are not collected during these surveys.

During the last decade, the source to tap risk-based approach to pathogens in drinking water has been largely promoted. This paper addresses the issue of source water pathogen contamination, which is the first step of quantitative microbial risk assessment. It is focused on a selection of pathogens considered to be a major risk to human health. Source water quality is highly variable and understanding the reasons for this variability is important as it will influence the requirements for treatment, treatment efficiency and the resulting health risk associated with the finished water. A framework for source water microbial quality assessment based on catchment surveys and monitoring programmes was set and tested on ten water sources. The monitoring programmes included faecal indicators and pathogens, during both baseline and hazardous event conditions. Concentrations varied greatly within and between systems. Faecal indicators were shown to be poor surrogates for pathogen presence and concentrations. There was no recurring evidence that the pathogens correlated together and links between microbial parameters appeared to be very site specific. Such variability between systems shows the importance of running local monitoring programs for use in risk assessment. Finally, pathogen detection methods are not yet optimal due to their sensitivity and to the lack of knowledge on viability and infectivity of pathogens. A great effort needs to be made in the future to ensure better quality data as this may have large implications in the statistical risk assessment calculations.
