Successful use of long-acting octreotide for protracted gastrointestinal bleeding related to protein-losing enteropathy after the Fontan procedure: a case report

Abstract Gastrointestinal bleeding complicated with protein-losing enteropathy after the Fontan procedure has been often reported in recent years, but there is no established therapy for it. We report the case of an 18-year-old boy who suffered from abdominal pain, melena, and anaemia due to intractable haemorrhagic protein-losing enteropathy after the Fontan procedure. He was successfully treated with octreotide therapy.

Gastrointestinal stromal tumors (GIST) are rare mesenchimal neoplasmas of the gastrointestinal tract. The diagnosis of this tumors are often very difficult. Patients with this tumor are usually admitted to the hospital cause of the gastrointestinal bleeding, abdominal pain, abdominal distension, dysphagia, obstructive jaundice and bowel obstruction. In this case report, we present a 86 year old patient with massive GIST of the stomach which was not preoperatively diagnosed. .

Introduction. Aortoenteric fistula is a communication between the aorta and segments of the gastrointestinal tract. Primary aortoduodenal fistula is an extremely rare cause of gastrointestinal bleeding associated with a high mortality rate. Case Report. We report a case of a 63-year-old man admitted due to abdominal pain lasting for a week. Abdominal ultrasound and computed tomography angiography revealed an aortoduodenal fistula without an aortic aneurysm. Conclusion. This case is an example of a rare cause of gastrointestinal bleeding, as well as presentation of aortoduodenal fistula that was not caused by an abdominal aortic aneurysm.

Einleitung: Bei der Differentialdiagnose von intraabdominalen Tumoren ist an die mesenterialen Zysten zu denken. Wir möchten mittels Fallbeispiel an diesen seltenen Befund erinnern. Fallbeispiel: Es handelt sich um eine 35-jährige Patientin mit zweiwöchiger Anamnese von progredienten Abdominalschmerzen. Die Abklärung mittels Sonographie und Computertomographie ergab einen 14 x 12 x 3cm grossen zystischen abdominalen Tumor ohne Beziehung zu Uterus, Adnexen oder Oberbauchorganen, worauf die Patientin laparotomiert und die Zyste reseziert wurde. Diskussion: Mesenteriale Zysten sind selten. Die Pathogenese ist unterschiedlich und die klinische wie auch die radiologische Diagnostik schwierig. Die Symptomatik reicht vom akuten Abdomen über unspezifische Abdominalbeschwerden bis hin zum asymptomatischen Zufallsbefund. Mesenteriale Zysten können entlang des gesamten Gastrointestinaltraktes vom Duodenum bis zum Rektum auftreten. Therapie der Wahl ist die Zystenresektion. Schlussfolgerungen: Mesenteriale Zysten sind seltene intraabdominale Befunde. Die definitive Diagnosesicherung und die Therapie besteht in der Resektion.

Il moltiplicarsi dell'offerta di opportunità di gioco d'azzardo anche in luoghi a bassa soglia di accesso e non dedicati in maniera esclusiva (bar, centri commerciali, tabaccherie), ha rinnovato l'interesse degli esperti riguardo ai costi sociali, economici e psicologici associati a tale pratica.Il comportamento del gioco d'azzardo può condurre ad una dipendenza, riconosciuta e classificata dal DSM5 e può comportare la compromissione del paziente dal punto di vista psichico, familiare e sociale. Questa patologia colpisce una percentuale variabile dall'1% al 3% della popolazione generale. Pertanto, in Lombardia, il numero di persone affette da questo problema potrebbe variare fra 100.000 e 300.000.Nel presente case report viene discusso il caso di un giovane con scompenso psicotico di tipo schizoaffettivo e GAP, che si rivolge al SerD.

Introduction: The umbilical vein can become recanalised due to portal hypertension in patients with liver cirrhosis but the condition is rarely clinically significant. Although bleeding from this enlarged vein is a known complication, the finding of thrombophlebitis has not been previously described. Case report: We report the case of a 62-year-old male with a history of liver cirrhosis due to alcoholic liver disease presenting to hospital with epigastric pain. A CT scan of the patient’s abdomen revealed a thrombus with surrounding inflammatory changes in a recanalised umbilical vein. The patient was managed conservatively and was discharged home the following day. Conclusion: Thrombophlebitis of a recanalised umbilical vein is a rare cause of abdominal pain in patients with liver cirrhosis.

Gallstone ileus is an uncommon and often life-threatening complication of cholelithiasis. In this case report, we discuss a difficult diagnostic case of gallstone ileus presenting as small gut obstruction with ischemia. A 56-year-old female presented with abdominal pain and vomiting. A CT scan was performed and showed an evolving bowel obstruction with features of gut ischemia with pneumobilia although no frank hyper density suggestive of a gallstone was noted. The patient underwent emergency surgery and a 60 mm obstructing calculus was removed from the patient's jejunum, with a formal tube cholecystostomy. JMS 2018: 21 (2):117-119
