Abstract. Uncertainty, fear to harm the patient, discomfort handling the discussion and lack of time are the most cited barriers to prognostic disclosure. Physicians can be reassured that patients desire the truth about prognosis and can manage the discussion without harm, including the uncertainty of the information, if approached in a sensitive manner. Conversational guides could provide support in preparing such difficult conversations. Communicating ’with realism and hope’ is possible, and anxiety is normal for both patients and clinicians during prognostic disclosure. As a clinician pointed out: ’I had asked a mentor once if it ever got easier. – No. But you get better at it.’

Summary Aim: The study was designed to evaluate the feasibility, results and safety of a very short period of compression after ambulatory phlebectomy. Patients, methods: From September to December 2006 we prospectively studied 49 subsequent patients who underwent ambulatory phlebectomy for branch varicose veins. Post-operatively the treated part of the leg was bandaged for forty-eight hours. Results: No major adverse events occurred. Although 46.9% of treated patients had visible haematomas two days post-operatively, after six weeks this was only visible in one patient. The results of this study show that the clinical outcome of ambulatory phlebectomy in combination with compression for forty-eight hours is at least equal to a conventional (longer) period of compression. Conclusion: Post-operative treatment can be safely reduced without the need of adjuvant compression hosiery. This will reduce patient discomfort after ambulatory phlebectomy without jeopardizing the clinical outcome.

Neste artigo a Simulação de Monte Carlo (SMC) é aplicada de forma a exemplificar seu uso, avaliando o comportamento dos diferentes tipos de aviários em atender demandas simuladas, com base nas variáveis de preço e custo aplicados à indústria e produtor. O modelo foi implementado em Excel com o apoio do suplemento Crystal Ball. Ao final do artigo pode-se observar o comportamento dos subprodutos do frango de corte sob regime dos diferentes níveis de tecnologia de aviários. Concluiu-se que os aviários climatizados positivo e negativo apresentam semelhanças nos resultados alcançados na simulação, o que demonstra uma baixa relação custo/benefício no investimento em aviários climatizados negativos, pois necessitam de investimento inicial elevado. Este artigo considera uma análise superficial, contemplando apenas uma parcela do cenário real existente ao produtor.
