One of the most important and promising directions of introducing information and telecommunication technologies into the healthcare system in Russia is the development of a methodology for prolonged monitoring of the patient's condition and remote consultation by the attending physician. This is due to the fact that many diseases that hold leading positions in the structure of morbidity, disability and mortality of patients require active monitoring of the patient, including during the period between planned visits to the clinic. The creation of such systems can not only prevent medical errors, facilitate rapid response, but also helps to solve the most global problem in any chronic disease - poor adherence to treatment. Therefore, the development and implementation of a remote consultation system based on telemedicine technology will reduce the burden on the outpatient unit by reducing the need to visit a doctor and significantly improve the quality of medical care. However, the use of telemedicine technologies requires the existing business process reengineering of medicine center and process of information exchange between participants in this process - regional and federal medical centers. The article suggests the approach to forming information exchange model between federal and regional medical centers using telemedicine technologies for remote monitoring.

This chapter shows that Iran has similar advantages to countries with a more developed brand of health tourism, including low costs, quality health services, competent doctors, and abundant natural and cultural attractions. The Health and Medical Ministry has developed 6 mandatory guidelines for medical centers receiving health tourists: these include the general condition of the facility, the workforce, medical facilities, geographical location, operational conditions, and the content of medical center websites. The Ministry offers facilities to hospitals and organizations applying to join the medical tourism program. In particular, the progressive provision of medical procedures, the distribution and marketing of health services, medical training, medical products, and equipment, is within a standardized framework of guidelines and development priorities. The chapter concludes that these interventions are designed to establish an effective presence in world and regional medical tourism markets.

Introduction. The prevalence of obesity and associated comorbidities is comparable to the scale of the epidemic. Along with the change in lifestyle, pharmacotherapy is necessary to lead obesity patients to a healthier state.Aim. The evaluation of efficacy and safety of Reduxin® (sibutramine + microcrystalline cellulose) application among patients with alimentary obesity and comorbidities in routine clinical practice. Analysis of results of Primavera program.Materials and methods. The observation program Primavera was conducted since November 2012 to July 2015 under the auspices ofEndocrinology Research Centre and the Russian Association of Endocrinologists. The program "Primavera" was attended by 3095 doctors of various specialties and 98.774 patients being treated in 1272 hospitals in 142 Russian cities. The average age of the patients was 39.4 ± 10.4 years, the ratio of male / female – 17.7%/82,3%, the average body mass – 99.1±14.28 kg, the average BMI – 35.7±4.41 kg/m2, the average waist circumstance – 105.7±13.7 cm. The Duration of Reduxine® treatment was determined by the attending physician and ranged from 3 months to 1 year.Results. The duration of therapy was 3 months for 3% of patients, 6 months for 59.3% of patients, 12 months for 37.7% of patients. The BMI reducing dynamics during 3, 6, 12 months was 3.4±1.53 kg/m2 (average 9.5±4.28 kg), 5.4±2.22 kg/m2 (15.0±6.22 kg), 7.2±3.07 kg/m2 (20.0±8.62 kg) respectively. The average waist circumference reduction for 3, 6 and 12 months was 6.3±4.31 sm, 10.6±6.30 sm, 16.0±8.94 sm, respectively. It was shown that the weight loss during prolonged (more than six months) Reduxine therapy under the supervision of a physician was associated with decreased levels of systolic and diastolic blood pressure and had no affect on heart rate. Adverse events were reported for 4.1% of patients, according to doctors, the cause-effect relationship with the use of Reduxine can be estimated as probable or definite for 1.9% of patients, possible – for 1.7%.Conclusions. Observation programs allow to implement the principles of active monitoring of the efficacy and safety of the drug in the current clinical practice and to develop a skill of reasonable prescribing. In Primavera program it was shown that the use of Reduxine® (sibutramine+ microcrystalline cellulose) leads to loss of body weight and doesn’t lead to serious adverse effects.

The National Network of Libraries of Medicine (NNLM), established as the Regional Medical Library Program in 1965, has a rich and remarkable history. The network’s first twenty years were documented in a detailed 1987 history by Alison Bunting, AHIP, FMLA. This article traces the major trends in the network’s development since then: reconceiving the Regional Medical Library staff as a “field force” for developing, marketing, and distributing a growing number of National Library of Medicine (NLM) products and services; subsequent expansion of outreach to health professionals who are unaffiliated with academic medical centers, particularly those in public health; the advent of the Internet during the 1990s, which brought the migration of NLM and NNLM resources and services to the World Wide Web, and a mandate to encourage and facilitate Internet connectivity in the network; and the further expansion of the NLM and NNLM mission to include providing consumer health resources to satisfy growing public demand. The concluding section discusses the many challenges that NNLM staff faced as they transformed the network from a system that served mainly academic medical researchers to a larger, denser organization that offers health information resources to everyone.

AbstractIn this paper I review three models of language that have dominated language learning and teaching in the last 40 years: the textual model, the information exchange model, and the multilingual model. I analyze each one and consider how it stacks up to instances of language use in a globalized world. I then propose moving beyond the metaphors of citizens and consumers, and consider language teaching as educating denizens of a global ecology that requires sensitivity to context, political awareness, ethical answerability and a good dose of situational cunning.

In this paper, by analyzing the characteristics of data exchange process of SaaS-based collaboration supporting platform for industrial chain, a dynamic and secure business data exchange model for the platform is established. On this basis, methods for business data automatic obtain and format conversion, client authentication based on encryption lock and SOAP extension, business data encryption based on public key of the platform, and instant key created by client is discussed. The implementation of these methods is studied based on a .NET environment. Furthermore, the dynamic and secure business data exchange model is used in the SaaS platform of the automotive industry chain and SaaS platform of the injection industry chain, as it meets the multi-source and heterogeneous information exchange requirements.
