Abstract Objectives: Several recent publications have described the lack of education in transgender health care among providers across all levels of medical training. Here we describe a QI project that developed and implemented a transgender health care curriculum for the University of Massachusetts Medical School-Baystate Medical Center (UMass-Baystate) pediatric and combined internal medicine-pediatrics residency programs. Methods: We designed a curriculum for the UMass-Baystate pediatric (9 residents/yr) and med-peds (8 residents/yr) residency programs. The curriculum included grand rounds presentations on transgender health care, didactic sessions integrated into the residents’ protected educational time throughout the academic year, and a panel discussion with non-binary and transgender individuals from the community. The didactic sessions included a mixture of lectures, role- playing, and case-based discussion. The curriculum development was guided by a curriculum design specialist and adapted each year based on feedback. Residents’ self- reported comfort and competency level were assessed through a survey at baseline and at the end of each year. Results: Ninety-eight percent (42/43) completed the baseline survey. Forty percent (17/42) had received no formal training in medical school, and 21% (9/42) had never taken care of a transgender patient. At baseline, 62% felt a little less comfortable and 50% felt somewhat competent, 2.4% very competent caring for transgender individuals compared to cisgender individuals. After three years, 25% felt a little less comfortable and 44% felt somewhat competent, 19% felt very competent caring for transgender individuals compared ot cisgender individuals. The community panel was very well received. Free text comments regarding the curriculum included “very helpful,” “loved the panel,” “clinically relevant.” Conclusions: This QI initiative served as the groundwork for the development of a formal curriculum to enhance medical education among residents in caring for transgender individuals. After three years, residents felt more comfortable and competent in caring for transgender individuals as compared to the baseline survey. We will continue to adapt the curriculum as it continues. The curriculum has expanded to include the pediatric nurses and the UMass medical students.

The curriculum of the Bachelor of Dental Surgery in Faculty of Dentistry, USIM is based on the integration of naqli (revealed) and aqli (acquired) knowledge. It aspires its graduates to exhibit the values of balanced professionals in providing community oral health care by being technically competent, socially responsible, spiritually enlighted and professionally accountable. Their clinical supervisors are in an enviable position to assess them from these perspectives. The aim of this paper was to determine the supervisors’ perception on the performance of the graduates from the Faculty of Dentistry, USIM who were under their charge. It was a qualitative study on the performance of 100 graduates from the first three batches. A total of 21 supervisors spread across the country responded to an open-ended questionnaire on the qualities of USIM graduates under their supervision.  They may supervise more than one graduate. The perceptions of the supervisors were categorised into three major themes. They felt that the graduates were socially responsible in performing their functions with competence in the community oral health services They were seen to have good personal and leadership qualities. Their supervisors also perceived them to require further exposure, guidance and training as they were new to the service.  The results indicated that the Bachelor of Dental Surgery programme at USIM was able to instill the values of a balanced professional in dentistry among its graduates but these values need to be further enhanced in providing care in the community. These findings can provide the principles in curriculum development to enhanced the qualities of the graduates in the context of balanced professional.

ABSTRACT There are 985,026 physicians with Doctor of Medicine (MD) and Doctor of Osteopathic Medicine (DO) degrees licensed to practice medicine in the United States and the District of Columbia, according to physician census data compiled by the Federation of State Medical Boards (FSMB). These qualified physicians graduated from 2,089 medical schools in 167 countries and are available to serve a U.S. national population of 327,167,434. While the percentage of physicians who are international medical graduates have remained relatively stable over the last eight years, the percentage of physicians who are women, possess a DO degree, have three or more licenses, or are graduates of a medical school in the Caribbean have increased by varying degrees during that same period. This report marks the fifth biennial physician census that the FSMB has published, highlighting key characteristics of the nation's available physician workforce, including numbers of licensees by geographic region and state, type of medical degree, location of medical school, age, gender, specialty certification and number of active licenses per physician. The number of licensed physicians in the United States has been growing steadily, due in part to an expansion in the number of medical schools and students during the past two decades, even as concerns of a physician shortage to meet health care demands persist. The average age of licensed physicians continues to increase, and more licensed physicians appear to be specialty certified, though the latter finding may reflect more comprehensive reporting. This census was compiled using the FSMB's Physician Data Center (PDC), which collects, collates and analyzes physician data directly from the nation's state medical and osteopathic boards and is uniquely positioned to provide a comprehensive snapshot of information about licensed physicians. A periodic national census of this type offers useful demographic and licensure information about the available physician workforce that may be useful to policy makers, researchers and related health care organizations to better understand and address the nation's health care needs.

BACKGROUND The global incidence in the treatment of transident people is increasing. In the COVID-19 pandemic, many consultations had to be cancelled, postponed or converted to a virtual format. Telemedicine in Transgender Health Care management could support physicians. OBJECTIVE This study analyses the Acceptance, Use and Barriers of Telemedicine in times of SARS-CoV-2 in Transgender Health Care in Germany. METHODS This prospective cross-sectional study was based on a survey of gynecological endocrinologists and transident patients in Germany during the COVID-19 pandemic. Descriptive statistics were calculated and regression analyses were performed to show correlations. RESULTS We analyzed the responses of 269 transident patients and 202 gynaecological endocrinologists treating transident patients. Most believed that telemedicine was useful. Physicians as well as patients rated their knowledge of telemedicine as unsatisfactory. The majority of respondents said they do not currently use telemedicine, although would like to do so. Patients and physicians reported that their attitude had changed positively towards telemedicine and that their usage had increased due to COVID-19. The majority in both groups agreed on implementing virtual visits in stable disease conditions. In the treatment phases, 74.4% (150/202) of the respondents would use telemedicine during follow-up. Half of the respondents would choose telecounseling as a specific approach to improving care (128/202, 63.2%). Obstacles to the introduction of telemedicine include the purchase of technical equipment (132/202, 65.3%), administration (124/202, 61.2%) and poor reimbursement (106/202, 52.4%). CONCLUSIONS Telemedicine in Transgender Health Care finds limited use but high acceptance among doctors and patients alike. The absence of a structured framework is an obstacle to effective implementation. Training courses should be introduced to improve the limited knowledge in the use of telemedicine. More research in teleendogynaecology is needed. These include large-scale randomized controlled trials, economic analyses and the exploration of user preferences.
