A muscle hernia is defined as a protrusion of the muscle belly through an acquired or congenital fascial defect. Muscle herniation through fascia is a relatively rare entity. Though predominantly asymptomatic, rarely they can be cause of vague pain in the leg, aggravated by exercises. Various conservative measures have been described for asymptomatic hernias, but treatment of symptomatic cases remains controversial. Here we present a case of symptomatic post traumatic tibialis anterior muscle herniation which was treated successfully with autologous fascia lata graft in Yenepoya Medical College, Mangalore in the month of January 2013.Muscle hernias should be kept in mind as a rare differential diagnosis whenever patients present with persisting vague leg pain with or without swelling. If conservative treatment fails, we recommend closure with autologous graft or fasciotomy to relieve the symptoms.Kathmandu Univ Med J 2013; 11(4): 332-334

Objective: Assess patient performance and quality of the stump after amputation at the Chopart (midtarsal) joint, with double-bundle transfer of the tibialis anterior muscle tendon to the talar neck. Methods: This study evaluated the medical records of 5 patients who underwent Chopart amputation with double-bundle transfer of the tibialis anterior tendon to the talar neck, assessing pre and postoperative performance and gait. Results: The patients were operated on between January 2008 and December 2018, and the data obtained from the survey allow us to conclude that, after the proposed procedure, all patients reported an improvement in walking, besides noting a significant reduction in the degree of stump equinus. Conclusion: The surgical technique described in this article produced a significant improvement in patient performance as assessed by the AOFAS hindfoot score, and prevented the formation of ulcers in the anterior region of the stump. Level of Evidence IV; Therapeutic Study; Case Series.

Muscle hernia is an uncommon cause of leg swelling. It can be detected in the early stages only if there is a high index of suspicion. It is common in lower extremity compared to the upper extremity. Tibialis anterior muscle in the leg is commonly involved. Dynamic sonography and magnetic resonance imaging (MRI) are the mainstay in their diagnosis, which demonstrate a facial defect with herniation of muscle fibers. We report a case of 23-year-old male patient who presented with a painless swelling in the anterolateral aspect of the left upper leg. Dynamic sonography done with high-resolution probe demonstrated a defect in fascia overlying tibialis anterior with herniation of outer muscle fibers which increased during dorsiflexion and reduced in the supine position at rest. MRI of the left leg confirmed the findings.
