Methodologic Quality of Studies on Prognostic Factors for Primary Open-angle Glaucoma Progression Measured by Visual Field Deterioration

Background: Primary open-angle glaucoma (POAG) is an age-related chronic optic neuropathy causing progressive constriction of visual field, which compromised quality of life (QoL) of older adults. Objective : The study aims to determine the QoL according to the severity of visual field using Bahasa Malaysia version of the Glaucoma Quality of Life- 36 (Glau-QoL 36) in older adults with POAG in Malaysia. Methods: A cross-sectional study was conducted in two tertiary hospitals in Malaysia: Hospital Universiti Sains Malaysia, Kelantan, and Hospital Selayang, Selangor. POAG patients who were ≥ 60 years old at the time of recruitment had minimal cataract, underwent cataract or trabeculectomy surgery at least 3 months prior and were on medical and surgical treatment. The severity of POAG was based on the modified Advanced Glaucoma Intervention Study (AGIS) score on two reliable reproducible Humphrey visual field SITA program 24-2 analysis. Face to face, one-on-one interview was conducted using validated Bahasa Malaysia version of GlauQol 36. Results : A total of 360 older adults with POAG were recruited. Majority were between ages 60-67 (38.3%) with 64 (17.8%) mild, 93 (25.8%) moderate, 115 (31.9%) severe POAG and 88 (24.4%) end-stage severity of POAG. The majority of the recruited patients were not working (88.9%) and live with their families (68.1%). There was a significant association between GlauQoL 36 score of all domains: daily living, driving, physiological well-being, self-image, anxiety, burden of treatment and confidence in healthcare with the severity of POAG (p<0.001). Increased severity of POAG was associated with decreased QoL in all GlauQoL 36 domains except confidence in healthcare. There was also a significant increase in dependency, with a majority of the end-stage were living with their families (p<0.001). Conclusions: QoL and independency of older adults with POAG decrease with worsening of visual field defect. Addressing the problem of visual-related activities in older adults with POAG may reduce their dependency and QoL. Happy living is important to lead to healthy living among older adults with POAG.

This review of modern literature presents the issue of early diagnosis of primary open-angle glaucoma (POAG) which is one of the most important problems in ophthalmology. According to the World Health Organization (WHO), the number of glaucoma patients all over the world ranges from 60.5 to 105 million people, and in the next 10 years it will increase by another 10 million. The subtle onset of the disease usually leads to late diagnosis, since visual field defects are often first detectable by visual field testing when 25–35% of retinal ganglion cells have already been lost. In modern studies, it has been established that the cell death in POAG is accompanied by oxidative stress. It is considered a key factor indicating the pathological process of a fairly large number of chronic diseases of the whole body, including complicated eye pathologies and inflammatory processes. The resulting free radicals oxidize lipids and phospholipids which leads to their peroxidation and triggers damage to cell membranes. The lipid peroxidation is one of obligatory processes occurring in neurodegenerative conditions including POAG. Modern technologies analyzing lipid peroxidation may accelerate the diagnosis, to identify glaucoma biomarkers at an earlier stage, and as a result to maintain quality of vision more efficiently. These studies were carried out in different countries and using different methods such as serological proteomic analysis, chromatography and mass spectrometry, and new technological approaches such as MultiOmics. The study of the protein profile, lipase activity, nitric oxide concentration, activity of carbonic anhydrase enzyme, malondialdehyde (MDA) and other compounds are also very informative. Various fluids were taken as a basis for the study: tears, aqueous humor of the anterior chamber, blood and its components. Together these technologies enable to evaluate the oxidative process in different ways. Earlier diagnosis of glaucoma is very important as it will enable to maintain the quality of vision for a longer time and reduce the rate of the disease progression. Glaucoma is a diverse group of disorders affecting the eye with a common characteristic potentially progressive optic neuropathy that is determined by both structural changes and functional deficit in which IOP is a key modifiable factor. In Primary Open Angle Glaucoma (POAG), IOP independent mechanisms of glaucomatous nerve damage and visual field loss with unobstructed angle of anterior chamber is observed. The patient has reported gradual diminution of peripheral aspect of visual field (Rt. eye - 6/12 and Lt. eye - 6/12 on Snellen’s distant vision chart) in both eyes for 2 years. There was marked peripheral field defect on both confrontation test and perimetry visual field analysis test. The disease shows clinical similarity with Kaphaja Adhimantha, a disease affecting the whole eye mentioned in Susruta Samhita and Ashtanga Hrudaya. It is a chronic disease comes under life style related disorder. Dukhena Roopam Pasyathi (distorted image or constricted visual field), Sirodukha (headache), Srava (watering), Kandu (itching), Pamsupoornatha (foreign body sensation), Aviladarsana (diminished vision) and Gourava (heaviness of eye and head) are the clinical findings explained in the context of Kaphaja Adhimantha. These clinical finding mentioned in classical literature shows resemblance with POAG. The meticulous deployment of kaphaja Abhisyandha- Adhimantha treatment protocol can be used to prevent the progression of ganglionic damage and preservation of eye sight. The logical interpretation on the basis of both subjective and objective clinical findings concluded the diagnosis as Kaphaja Adhimantha and treatment principle adopted was Apatharpana, Kaphahara, Abhisyandhahara and Srothovisodhana. After treatment his vision has improved as 6/6 (Rt. Eye) and 6/6 (Lt. Eye) and remarkable change in field analysis. The study discusses about the effectiveness of Ayurvedic management in POAG.
