Objective: To compare the host and biomechanical response to a fully absorbable poly-4-hydroxybutyrate (P4HB) scaffold to the response to PP mesh in an animal model of vaginal POP surgery. Design: A study employing a sheep model Setting: KU Leuven Center for Surgical Technologies Population: 14 parous female Mule sheep Methods: P4HB scaffolds were surgically implanted in the posterior vaginal wall of sheep. The comparative PP mesh data were obtained from an identical protocol. Main outcome measures: Gross necropsy, histological and biomechanical evaluation of explants, and the in vivo P4HB scaffold degradation were evaluated at 60- and 180-days post-implantation. Results: Gross necropsy revealed no implant related adverse events using P4HB scaffolds. The tensile stiffness of the P4HB explants increased at 180-days (12.498 ± 2.66 N/mm (P=0.019)) as compared to 60-days (4.585 ± 1.57 N/mm) post-implantation, while P4HB degraded gradually. P4HB scaffolds exhibited excellent tissue integration with dense connective tissue and a moderate initial host response. P4HB scaffolds induced a significantly higher M2/M1 ratio (1.70 ± 0.67 SD, score 0-4), as compared to PP mesh (0.99 ± 0.78 SEM, score 0-4) at 180-days. Conclusions: P4HB scaffold facilitated a gradual load transfer to vaginal tissue over time. The fully absorbable P4HB scaffold, in comparison to PP mesh, has a favorable host response with comparable load bearing capacity. If these results are also observed at longer follow-up, a clinical study for vaginal POP surgery may be warranted to demonstrate efficacy. Septic shock is defined by a dysregulated host response to infection, resulting in life-threatening circulatory, cellular, and metabolic abnormalities. The short-term mortality is approximately 45-50%, and around half of survivors of sepsis may have subsequent long-term cognitive decline. Apart from early hemodynamic, respiratory resuscitation and appropriate antimicrobial treatments, there is no approved adjunct therapy for sepsis.  JMS 2018: 21 (2):134-135

AbstractThe aim of this paper is to estimate government consumption multiplier and to examine the effect of various characteristics of countries on the size of fiscal multiplier. We apply a panel VAR model following Ilzetzki et al. (2013) for a sample of 28 EU countries covering the period from 1995 to 2017. Key findings are, first, the estimated average fiscal multiplier is larger than unity. Second, the size of fiscal multiplier is larger in the cases of lower public indebtedness, for more developed European countries and for more financially open economies, which is also in line with relevant empirical literature. Regarding the role of trade openness, the results are inconclusive. In addition to this, membership of countries in the European integrations positively affects the size of fiscal multiplier. Therefore, fiscal policymakers should use fiscal stimuli as the instrument of boosting short-term economic growth selectively and consider country-specific characteristics. This paper contributes to the ongoing discussion in two ways, it examines the effect of additional characteristics of countries on the size of fiscal multiplier and updates existing empirical literature.

Context. The current IAU2000 nutation model performed different approximations, one of them being that the Oppolzer terms associated to the planetary perturbations of the nutations were assumed to be smaller than 5 μas and thus were neglected. At present, the uncertainties of the amplitudes of individual components of the observed nutations are better, and the conventional nutation model does not fit the accuracy requirements pursued by the International Astronomical Union (IAU) and the International Association of Geodesy (IAG). Aims. The objective of this work is to estimate the magnitude of the lacking Oppolzer terms of the planetary nutations and find out whether they are still negligible or not. Methods. The Oppolzer terms resulting from the direct and indirect planetary perturbations of the Earth’s rotation have been computed for a two-layer Earth model following the Hamiltonian theory of the non-rigid-Earth. Results. The planetary Oppolzer terms for the non-rigid Earth are not really negligible as believed, and some of them have amplitudes larger than 10 μas, therefore significantly above the current level of uncertainty of individual harmonic constituents. Conclusions. In the short term, the IAU2000 nutation model must be supplemented with suitable corrections accounting for those missing components; its planetary component must be thoroughly revised in the medium term.
