A cardiac murmur, which is often heard in the newborn baby in the first few hours of life, is described. This murmur probably arises from a left-to-right flow through a persistent ductus arteriosus. This paper presents findings on the relationship between the presence of this murmur, asphyxia, and the maturity and clinical condition of the infant. Asphyxia and dyspnea were found to be associated with a higher incidence of the murmur, especially in premature infants. A fall in rectal temperature was associated with a amdioration of these manifestations, and in three babies warming brought about dyspnea as well as return of the murmur. The implications of this are discussed.
