An Audit of the Effectiveness of Acupuncture on Musculoskeletal Pain in Primary Health Care

Little is known about the use of acupuncture in general practice. We performed a retrospective review of the use of acupuncture in relieving musculoskeletal pain, a condition that is commonly encountered in general practice. A sample of 116 patient records was reviewed, from which 92 patients (mean age 52 years; 64% female) met the inclusion criterion of musculoskeletal pain. Information obtained included age, sex, diagnosis, duration of the problem, length of treatment (weeks), number of treatments, duration of each treatment (minutes), number of needles used, level of benefit obtained from the treatment, and recurrence of pain. There were many different conditions encountered. We found an association between the general practitioner using fewer needles and patients experiencing greater pain relief. This could be a reflection of treating myofascial pain syndromes, which often appear to respond well to a single needle in the key trigger point. Overall, we found that sixty-nine percent of patients had a good or excellent response to acupuncture treatment. We recommend acupuncture as a treatment option for patients who do not respond to the usual therapies (non-steroidal anti-inflammatory drugs) for musculoskeletal conditions.

Pain syndrome accompanies absolute majority of diseases, therefore problems of adequate anesthesia remain acute not only for urgent conditions but also for everyday medical practice. Modern approaches of anesthetic therapy should consider not only pathogenic mechanisms of evolving pain syndrome by a certain patient but also necessity to administer drug combinations. It allows potentiating an analgesic effect, to decrease effective dosages of individual drugs and minimize adverse effects. The administration of non-steroidal anti-inflammatory drugs together with B vitamins and tolperisone is justified in a marked pain syndrome with a spastic component and/or spasms of skeletal muscles, muscle contractures, myofascial pain syndromes, as well in rehabilitation therapy after orthopedic and traumatology surgeries. The article considers pathogenic details of such combined therapy, discloses mechanism of synergic action of the proposed drug combination.

Recent investigations of dysfunctional pain processing in the central nervous system have contributed much knowledge about the development of chronic musculoskeletal pain. Many common chronic musculoskeletal pain syndromes - including regional myofascial pain syndromes, whiplash pain syndromes, refractory work-related neck-shoulder pain, certain types of chronic low back pain, fibromyalgia and others - may essentially be explained by abnormalities in central pain modulation. The growing awareness of dysfunctional central pain modulation may be a conceptual breakthrough leading to a better understanding of common chronic pain disorders. A new paradigm will have multiple clinical implications, including re-evaluation of clinical practice routines and rehabilitation methods, and will focus on controversial issues of medicolegal concern. The concept of dysfunctional central pain processing will also necessitate a mechanism-based classification of pain for the selection of individual treatment and rehabilitation programs for subgroups of patients with chronic musculoskeletal pain due to different pathophysiological mechanisms.

The Sternocleidomastoid (SCM) muscle may develop myofascial trigger points and the physical manifestation of pain and signs and symptoms due to these trigger points is commonly referred to as SCM syndrome. The diagnosis of SCM syndrome can be onerous as non-specific signs and symptoms associated with SCM syndrome may vary in presentation. The subject in this case report had post-acceleration-deceleration induced injury to SCM with hypertrophy of sternocleidomastoid muscle on the left side. The patient reported pain over the left SCM muscle, forehead, around the ipsilateral eye, over the cheek, the tip of the chin, sternoclavicular joint and deep in the throat upon. The patient was ascribed various diagnosis before a definitive diagnosis was made. The patient underwent three trigger point injections under ultrasound guidance, last one with BOTOX, and reported long-lasting pain relief. Overall with intensive physical therapy and trigger point injections the acute symptoms resolved. In pronounced pain syndrome the most important functional shifts (the generators of pathologically increased excitement) by recording somatosensory generated potentials arc revealed at higher levels than in slight tenderness. At the same time in slight painfulness tenderness a great number of the generators of mild activity arc found, showing insufficient activity of antinociceptive system. After treatment a number of the generators of pathologically increased excitement decrease, at the same time the migration to the caudal parts of nervous system occurs. The neurophysiologic basis for possible worsening myofascial pain syndromes is created.
