Aortocardiac fistulae (ACF) are exceptionally due to infective endocarditis; they are usually congenital, posttraumatic, or complicate aortic dissection. In infective endocarditis setting, their presence should prompt urgent surgery as patients can deteriorate rapidly. We report the case of a 78-year-old female patient with the first ever reported quadricuspid aortic valve infective endocarditis complicated by multiple aortocardiac fistulae. Additionally, we provide a brief review of ACF, in infective endocarditis and quadricuspid aortic valve.

Introduction: Quadricuspid aortic valve is a rare anatomical abnormality, its incidence ranging from 0.008% to 0.033%. Case Report: Wedescribe the case of a female patient with a quadricuspid aortic valve. The direct visualization showed a type E quadricuspid aortic valve. Aftercomplete excision of the valve, it was replaced by a 23mm biological prosthesis. Postoperative recovery was uneventful. Discussion: Since thefirst case, described in 1862, fewer than 200 cases have been diagnosed so far in world literature. Given its frequent association with othercardiac malformations, it is important that physicians seek their preoperative diagnosis.

Ralstonia pickettiiis a rare pathogen and even more rare in healthy individuals. Here we report a case ofR. pickettiibacteremia leading to aortic valve abscess and complete heart block. To our knowledge this is the first case report ofRalstoniaspecies causing infective endocarditis with perivalvular abscess.

AbstractThis case report describes a 55-year-old male who presented with acute Type A aortic dissection. He underwent emergent surgical repair, and his intraoperative transesophageal echocardiography revealed a quadricuspid aortic valve. His aortic root measured 45 mm. Quadricuspid aortic valves have previously been associated with aortic root dilation. This case illustrates the possible association of quadricuspid aortic valves with aortic dissection, similar to what is described with bicuspid valves.
