In recent years, the skeletal anchorage through miniscrews has expanded the treatment options in orthodontics (Yamaguchi et al., 2012). We hereby present a modified method for tooth extrusion for cases where crown-lengthening surgery is contraindicated for aesthetic reasons. This modified method uses three orthodontic appliances: a mini-implant, an orthodontic wire, and a bracket. The aim of this case report was to increase the length of the clinical crown of a fractured tooth (tooth 23) by means of an orthodontic extrusion with the modified method of Roth and Diedrich.

This article describes the management of a case of oblique crown root fracture of a central incisor where the fractured fragment was used as a temporary replacement crown in order to maintain esthetics during the period of orthodontic extrusion of the remaining root. Placement of the final restoration after orthodontic extrusion resulted in good esthetics and periodontal health postoperatively. The merits and demerits of this treatment modality are discussed.

Trauma to the aesthetic region of the mouth is very common. However the treatment of the traumatic injuries of the tooth depends upon various factors. Orthodontic extrusion of the fractured tooth is regularly practiced treatment modality. Orthodontic extrusion of such tooth is physiological, less invasive and long lasting. The extrusion of the tooth brings enough dental tissue to maintain the biological width and ferrule later into restorative/prosthetic phase. In this article we will report a case which was extruded 4mm after endodontic treatment and restored by crown.

Post endodontic restoration in cases of grossly destructed teeth are difficult as the available tooth structure is less, margins are subgingival and of compromised prognosis because the restoration might encroach upon the periodontium affecting its status to deteriorate. Interdisciplinary approach using forced eruption or Orthodontic extrusion along with minimal surgical crown lengthening can give adequate supragingival tooth surface for proper restoration and protect its root support from getting violated. This is the case report of an unsalvageable tooth that is restored to satisfactory functional and esthetic restoration utilizing interdisciplinary approach.

Orthodontic extrusion can be time consuming and has a long retention period, making cooperation a critical factor. On the other hand, it has also been shown that surgical techniques may be useful to extrude and save the root. Surgical methods need at least 3 week for root stabilization in new position. This is a case report of a surgical method to treat intruded teeth.
