Background: Endodontic treatment of various forms of pulpitis with variations of root canal system anatomy should be performed with high quality. The use of various antibacterial agents is aimed at maintaining the success of endodontic treatment. The aim of this study was to evaluate the penetration and fixation of the nano-silver solution on the dentinal surface during endodontic treatment. Materials and methods: the study was carried out on 70 extracted single-rooted teeth, randomly divided into two groups. In the teeth of the first group, the smear layer was removed after canal preparation with 17% EDTA solution; in the second group, the smear layer was not removed. In both groups, for the final treatment of the canal, a colloidal 1% solution of нанo серебра nanosilver was used. Samples were cut and prepared for analysis using micro-CT, scanning electron microscopy (SEM), X-ray microanalysis and energy dispersive spectrometry (elemental mapping). Results: in 100% of cases in groups of teeth with a preserved smear layer, the ability of a 1% colloidal solution of nanosilver with particles of 1–2 nm to be fixed on dentin with a removed and preserved smear layer and to leave a film on the dentinal surface was established. In the samples with removed smear layer, silver was found in 73.5% of cases. Conclusion: The nano-silver solution with a particle size of 1–2 nm proved its ability to penetrate the dentinal surfaces and create a final film covering the dentinal surface of the root canal before applying the sealer.

Endodontic treatment approaches aim to achieve proper treatment and prevention of apical periodontitis to enhance the oral health status and enhance the prognosis of affected teeth. However, many complications can develop secondary to endodontic treatment. The management of post-treatment apical periodontitis might be challenging to clinicians, and the prognosis is usually lower than that of primary apical periodontitis. Therefore, identifying the potential etiology and intervening against them might be ideal for these cases. The present literature review discusses the commonest causes reported in the literature to predispose to the development of post-treatment apical periodontitis. Most of the various investigations in the literature indicate that post-treatment apical periodontitis is usually caused by either extraradicular or intraradicular infections, like primary apical periodontitis. However, it should be noted that some studies also reported that technical or procedural errors might predispose to the pathogenesis of the condition. However, it has been reported that the presence of associated bacterial infection conditions this.

A combined endodontic-periodontal lesion is characterized by the association of endodontic and periodontal infection in the same tooth, attacking and destroying its attachment system. This complicates the therapeutic management because the simultaneous control of the two infections would be inconvenient in clinical practice. It would therefore be essential to sequence the stages of both periodontal and endodontic treatment. The general objective of this article would be to shed light on the contribution of the endodontic approach in the treatment of combined endodontic-periodontal lesions and to specifically underline its influence on the prognosis of this condition.

Palatogingival grooves (PGG) are developmental malformations infamous for precipitating endodontic-periodontal lesions. Pertaining to their discreet occurrence, variation in groove depth and funnel like shape, adherence of plaque and calculus is apparent thereby jeopardizing the periodontium and pulp. A case of palatogingival groove in maxillary left lateral incisor was diagnosed and managed with endodontic treatment followed by open flap debridement to seal the groove with biodentine and fill the defect with bone graft. On follow up, the periapical lesion and the periodontal pocket were successfully resolved.

Por meio deste estudo é possível analisar que a instrumentação dos canais radiculares após o uso do Glide Path traz inúmeros benefícios, incluindo o melhor deslizamento dos instrumentos, redução do número de fraturas e menor intercorrências intrarradiculares. Além disso, essa preparação inicial do canal radicular com o uso do Glide Path realiza uma desobstrução, criando um “caminho” livre para o melhor deslize dos instrumentos no canal radicular. Os instrumentos possuem características e particularidades que levam ao mesmo objetivo de preparar o conduto para uma segura instrumentação, destaca-se aqui o WaveOne Gold, R-Pilot e o PathFile. É uma técnica que vem sendo difundida e utilizada na endodontia contemporânea com o intuito de realizar um procedimento eficaz.---Abstract: Analyze the use of instruments to prepare the Glide Path and understand how this endodontic instrument brought advantages in instrumentation techniques. This is a literature review with selected articles from the last 10 years, in order to achieve the objectives proposed by the theme. Articles in English and Portuguese were used, with all information pertinent to the topic extracted. Through this study, it is possible to analyze that the instrumentation of root canals after the use of the Glide Path brings numerous benefits, including better sliding of the instruments, reduction in the number of fractures and less intra-radicular complications. In addition, this initial preparation of the root canal using the Glide Path performs clearance, creating a clear “path” for the best glide of the instruments in the root canal. The instruments have characteristics and particularities that lead to the same objective of preparing the conduit for safe instrumentation, the Wave One Gold, R-Pilot and PathFile stand out here. It is a technique that has been disseminated and used in contemporary endodontics with the aim of performing an effective procedure. Through this bibliographical study, it is possible to consider that the use of the Glide Path favors the preparation of the root canal and proves to be effective and safe when used before instrumentation, thus, with the creation of a more favorable environment for instrumentation, the chances of success in endodontic treatment.
