Experimental Model of Fungal Sinusitis: A Pilot Study in Rabbits

We have established an experimental model of fungal sinusitis in rabbits to analyze the chronology and the pathogenesis of the development of noninvasive fungal sinusitis due to Aspergillus fumigatus. Thirty-four Pasteurella-free New Zealand white rabbits divided into three groups were included in this study. In the first group (10 rabbits), A fumigatus was inoculated into the maxillary sinus. In the second group (10 rabbits), A fumigatus was inoculated into the maxillary sinus in the presence of a wound in the mucosa. In the third group (14 rabbits), A fumigatus was inoculated into the maxillary sinus in the presence of a blocked ostium. On days 15 and 30, endoscopic, histopathologic, bacterial, and mycological examinations of both maxillary cavities and mucous membrane were performed. The rabbits were painlessly sacrificed 30 days after inoculation; mucosal and bone biopsies of the maxillary sinus cavities were performed for histopathologic studies. We found that noninvasive fungal sinusitis had been induced in 2 rabbits of the second group and 8 rabbits of the third group. We conclude that introduction of fungi into a sinus with a blocked ostium induces fungal sinusitis. The present model of experimental fungal sinusitis seems to be reproducible and suitable for further studies of the development of fungal sinusitis.

Two experiments investigated the acceptability of multiple questions. As expected, sentences violating the Superiority Condition were accepted less often than sentences obeying it.The status of the Superiority violations was not improved by the addition of a third wh, regardless of whether the third wh was an adjunct or an argument, though it was improved by the addition of a second question (e.g., and when).Further, in a small pilot study directly comparing a sentence with adjacent final wh-phrases that may induce a stress clash (I'd like to know who hid it where when) with a sentence violating Superiority but avoiding the final adjacent wh-phrases (I'd like to know where who hid it when), half the participants indicated that the Superiority violation sentence sounded better.This suggests that the status of some additional-whsentences may appear to improve simply because the comparison sentence with adjacent final wh-phrases is degraded.Overall, the results of the studies suggest that there is no need to complicate syntactic theory to account for the additional-wh effect, because there is no general additional-wh effect.

In May 2010, the release of the iPad in Australia brought a whole new dimension to learning. This chapter presents the preliminary findings of a pilot study conducted at a large distance education university designed to explore the use of the iPad as a tool for learning from three perspectives. The first is the use of the iPad from a lecturer’s point of view, outlining how it can be used to enhance the task of teaching in distance education. The second is from a student’s point of view, exploring how the iPad can assist in distance education study. The third examines the iPad from an insider perspective, reviewing the variety of apps available including those for social networking. The overall impression is that the iPad has great potential as a tool for learning but it will not necessarily reduce the need for desktop or laptop computers.

With the phenomenal growth of mobile applications or apps used for teaching and learning, we are all challenged with determining which ones are effective and efficient in meeting our specific instructional needs. The use of mobile apps directly impacts students, teachers, administrators, trainers, and employees worldwide. Apps are used across all discipline areas in a variety of settings including applied interdisciplinary approaches. With this in mind, it is critical to have a workable set of app analysis questions based on current best educational practices to assist in making informed decisions on app selections to provide quality teaching and learning experiences. This chapter provides a mixed method research study combining class observations with results from three pilots in an effort to create a set of quality questions for quickly evaluating mobile apps for instructional implementation. After creating a set of questions for evaluating the quality of the apps based on current best instructional practices, the following three pilot studies were conducted. The first pilot allowed students to select an app of their own choice followed by a survey to evaluate the app using both quantitative and qualitative open-ended responses. The second pilot had all students examine the same app followed by the same survey to analyze potential differences in results and to gain additional insights. The third pilot study used the same questions, but this time rather than using it to evaluate the app, the students evaluated the quality of the questions used. During the third pilot study, students were looking strictly at the quality of the questions for instructional use. All study participants were graduate-level students in Instructional Design and Technology and were aware of best instructional practices. It is anticipated, post study, instructors and trainers can begin using the evaluation instrument, selecting those questions meeting their unique instructional needs.

Rabbits provide a unique challenge for routine endotracheal intubation in clinical practice because of various distinctive anatomic and physiologic features. Many previously proposed methods for endotracheal intubation in rabbits are limited by several factors, including the needs for expensive equipment and high levels of technical expertise. We evaluated capnography for its effectiveness in assisting endotracheal intubation in rabbits. New Zealand white rabbits were divided into 3 groups of 5 animals. In the first 2 groups, mainstream (nondiverting) or sidestream (diverting) capnography (MC and SC groups, re- spectively) was used; in the third group (LS group), a laryngoscope with a size 00 Miller blade was used to guide endotracheal tube placement. Anesthesia was induced through intramuscular administration of ketamine (10 mg/kg), medetomidine (0.1 mg/kg), and midazolam (1 mg/kg) mixed in the same syringe prior to administration. Intubation time was defined from the point of opening the jaws to the completion of the first capnogram after intubation. Intubation was accomplished successfully in all animals in both capnography groups, but 2 rabbits in the laryngoscopy group could not be intubated. Intubation time was compared among groups was compared by using one-way ANOVA, and posthoc Bonferroni testing was applied to isolate significant differences between groups. The intubation time (mean ± 1 SD) was 46.4 ± 12.6 s in the MC group, 147.2 ± 44.2 s in the SC group, and 385.0 ± 114.1 in the LS group, with intubation time significantly differing among all groups. In conclusion, both mainstream and sidestream capnography-guided endotracheal intubation techniques were more effective and efficient than conventional laryngoscope-guided endotracheal intubation in rabbits. Furthermore, mainstream capnography was preferred over sidestream capnography because mainstream capnography resulted in significantly shorter intubation times.

Abnormalities of the underlying bone of the paranasal sinuses have sometimes been shown in Wegener’s granulomatosis (WG). We describe an interesting case of WG with extensive bone abnormalities in the sinuses mimicking fungal sinusitis. A 30-year-old woman presented with intermittent unilateral epistaxis. Biopsy was performed for the granulation tissue in the right nasal cavity, and she was diagnosed as having WG. Computed tomography (CT) revealed a ring-like calcification, mimicking a fungus ball, in the right maxillary sinus. Endoscopic sinus surgery was performed to confirm the diagnosis. A spherical bony structure, surrounded by granulation tissue, was identified in the maxillary sinus. The wall of the “bony ball” was fragile, like an egg shell. No fungus was found in the sinus. Thus, the extensive bone abnormalities were due to WG.
