A case is described of gross subcutaneous emphysema progressing to acute respiratory obstruction. The case is assessed with reference to a short review of the literature and a plea is made for awareness of serious complications of an apparently benign condition.

Neurological complications of infective endocarditis are frequent, especially ischaemic strokes. As intravenous thrombolysis in infective endocarditis-related ischaemic stroke has a controversial benefit/risk ratio, alternative treatment regimens have to be considered. We present the case of a young patient with septic embolism of the middle cerebral artery who was successfully treated with mechanical thrombectomy, and give a short review of the existing literature.

Splenogonadal fusion is a rare congenital abnormality. In this case report, the diagnosis of splenogonadal fusion was made after the removal of an abnormal mass at ‘routine’ left herniotomy. A cautious approach at surgery resulted in no apparent damage to the testicle. A short review of the literature is included.

The case presented is a rare example of subcutaneous emphysema isolated to an extremity. This is a benign condition and should not be confused with necrotizing gas-producing infections. In most cases, the condition rapidly resolves without treatment, usually in less than 48 hr. In rare cases where the mechanical process causing the subcutaneous emphysema persists, interruption of this process is required.
