Value Acquisition in an Occupational Therapy Curriculum

As the health care industry has become increasingly competitive, health care practitioners have become aware of the need to demonstrate that the occupational groups to which they belong perform unique and vital services to clients and patients. This awareness has fostered the development of a body of literature on professional identity and development that includes discussion of the role of values in defining and differentiating professional groups The purpose of this study was to begin exploration and discussion of the process by which students in one occupational therapy curriculum acquire and integrate professional values. The results of the study indicate that the acquisition of professional values may be a developmental process that is not complete at graduation from academic training. Additionally, the study indicates that value acquisition is a fertile area for research, one of tremendous importance for the education of future practitioners.

Despite its prominent place in the Occupational Therapy Guidelines for client-centred practice, the exact role of spirituality in clinical practice has been difficult to delineate. In this paper current concepts of spirituality are outlined and ways in which these concepts have been applied in health care are described. To illustrate these spiritual concepts, the experiences of individuals whom the authors have met in their clinical practice are outlined in a series of vignettes. Finally, a revision of the model of occupational performance which enhances consideration of spirituality in occupational therapy practice is proposed, and some guidelines for occupational therapists wishing to incorporate spiritual concerns in therapy are suggested.

The profession of occupational therapy claims to be patient centred; however, research shows that the views of patients and health care professionals often differ. Toileting is a fundamental daily activity, yet to date there have been few studies investigating the perceived importance of independence in toileting. A qualitative study was conducted to determine the views of stroke survivors and their occupational therapists regarding the importance of independence in toileting. A symbolic interactionistic framework was used, allowing the views of these two disparate groups to be compared. Thirteen stroke survivors and seven occupational therapists were interviewed. Five categories were identified in the data collected: the occupational form normally used in toileting; incontinence and continence; independence and dependence in toileting; issues relating to toileting and hospital discharge; and the role of occupational therapy. Both participant groups agreed that independence in toileting was important in avoiding the need for assistance and in avoiding feelings of decreased self-esteem. However, the patient participants' views extended further in that they stated that the method of toileting was important, not merely that it was conducted independently. In conclusion, it is essential that occupational therapists consider how they can facilitate a return to the method usually undertaken for toileting by patients. If a return to the previous methodology for toileting will not be possible, then the occupational therapists must give consideration as to how to facilitate adjustment by the patients.

Unintentional injuries, the leading cause of morbidity and mortality among youth in the United States, are burdensome and costly to society. Continued prevention efforts to reduce rates of unintentional injury remain imperative. We emphasized the role of practitioner influence across a linear concept of injury prevention comprising delivery, practice, and application/generalization and within the context of child developmental factors. Specific strategies for injury prevention tailored to the cognitive development stage of the patient are provided. This information may be useful to health care practitioners, who have significant interaction with youth and their families.

O estudo propõe um debate sobre o cuidado em saúde mental na atenção básica, partindo do entendimento de que a produção do cuidado se dá em redes, não apenas institucionais, mas redes vivas e de potência criativa. O percurso metodológico se deu por meio da cartografia, de modo que foi possível rastrear o território para além dos serviços de saúde. Como meio de produção de dados utilizaram-se os diários de campo produzidos na vigência de doze meses do projeto de extensão: Saúde Mental na atenção básica- desafio e necessidade, desenvolvido em uma unidade integrada de saúde da família de um município do estado da Paraíba. A vivência e atuação no território se deram inicialmente pelo mapeamento das redes de cuidado territoriais, como também a problematização das ações de cuidado na atenção básica, com o intuito de ampliar o debate sobre o papel das redes vivas no cuidado em saúde. Tal discussão possibilitou o desenvolvimento de ações em saúde pautadas na integralidade do cuidado, coesão social, produção de vida. Estes resultados apontam para a necessidade em se pensar ações compartilhadas e inseridas no território, tendo em vista que a saúde envolve também os territórios existenciais pelos quais os sujeitos circulam e produzem relações. A terapia ocupacional tem contribuído significativamente na consolidação deste cuidado. The study proposes a debate on mental health care in primary care, based on the understanding that the production of care takes place in networks, not only institutional, but also living networks and creative power. The methodological approach was through the mapping, so it was possible to trace the territory beyond the health services. As a means of production, data field diaries were  produced in the presence of twelve months of the extension project: Mental Health in the primary care - challenge and need, developed in an integrated health unit family in a municipality of state of Paraíba. The experience and operation in the territory was given initially by mapping of care networks available in the territory, as well as the questioning of care actions in primary care, in order to broaden the debate on the role of living networks in health care. This discussion enabled the development of health actions based on integrality of care, social cohesion, production of life in primary care. These results point to the need to think about shared and embedded actions in the territory, with a view that health also involves the existential territories for which individuals circulate and produce relations. Occupational therapy has significantly contributed to the consolidation of territorial care.Keywords: Comprehensive health care; Primary health care; Social network; Mental health, Occupational therapy. El estudio propone un debate sobre el cuidado en salud mental en la atención básica, basado en el entendimiento de que la producción de la atención se lleva a cabo en las redes, no sólo institucional, sino redes vivas y con poder creativo. El enfoque metodológico fue a través de la medio de la cartografía, por lo que fue posible rastrear el territorio más allá de los servicios de salud. Como medio de producción de datos se utilizaron diarios de campo producidos en la vigencia de doce meses del proyecto de extensión: Salud Mental en la atención básica -  desafio y necesidad, desarrollado en una unidad de salud de la familia integrada un condado en el estado de Paraiba. La experiencia y las operaciones en el territorio fue dado inicialmente por el mapeo de las redes de atención disponibles en el territorio, así como el cuestionamiento de las acciones de atención en la atención primaria, con el fin de ampliar el debate sobre el papel de las redes vivas en el cuidado de la salud. Esta discusión permitió el desarrollo de las acciones de salud basados en la atención integral, la cohesión social, la producción de vida. Estos resultados apuntan a la necesidad de pensar acciones compartidas en el territorio, teniendo en cuenta que la salud involucra también los territorios existenciales por los cuales los sujetos circulan y producen relaciones. Although falling outside its traditional domain of concern, medication management merits consideration by occupational therapists owing to both the beneficial and the detrimental effects of pharmacology on occupation. However, a lack of clarity about the profession's role in promoting this activity has important consequences from legal and professional perspectives. In a health care environment, where a client-centred approach to medication adherence is currently advocated, there is scope to use the profession's models to explore this topic from an occupational viewpoint. This could enhance effective collaboration between prescribers and patients and enable the use of occupational therapy to augment, support or replace pharmacological approaches.
