Ileocolic arterial aneurysm associated with segmental arterial mediolysis: A case report

Objectives Segmental arterial mediolysis is a rare disease that most commonly affects the superior mesenteric artery among abdominal arteries. However, aneurysms involving the ileocolic arterial branch of the superior mesenteric artery are extremely rare. Here, we describe the treatment of a patient with an ileocolic arterial aneurysm suspected to have occurred secondary to segmental arterial mediolysis. Methods We confirmed the diagnosis of ileocolic arterial aneurysm, which showed the characteristic “string-of-beads” appearance of the distal main trunk of the superior mesenteric artery on angiography. We performed endovascular coil embolization for the aneurysm, and for both the inflow and outflow vessels. After confirming that the aneurysm was no longer visible, the treatment was completed. Results There were no clinical findings suspicious of ischemic enteritis or intestinal necrosis after embolization. We confirmed that the ileocolic arterial aneurysm was not observed on computed tomography angiography one month after treatment. Conclusions While development of an ileocolic arterial aneurysm associated with segmental arterial mediolysis is very rare, it is at a high risk of sudden rupture. Therefore, coil embolization is a useful intervention in such patients and can be implemented based on the size and morphology of the aneurysm.

The rapid development of transplantation, endovascular and minimally invasive surgery necessitates a detailed study of the structural features of the vessels of the abdominal cavity. The purpose of the study is the characteristics of the morphometric parameters of the abdominal aorta and its unpaired branches in men and women at different ages and depending on the body type. The analysis of 266 computer tomograms of the abdominal part of aorta and its unpaired branches was carried out in adult men women divided into three age groups (first and second periods of mature age and elderly age), as well as into three groups according to the Pinier index (asthenic, normosthenic and hypersthenic body types). It was established that individual morphometric parameters of the abdominal part of aorta and its unpaired branches in men and women significantly change with age. It is proved that the diameter of the aorta at various levels significantly increases with age from the first mature to the elderly age by an average of 5 mm. However, in men, the length of the abdominal aorta, celiac trunk and main trunk of the superior mesenteric artery does not change with age. In women, the length of the celiac trunk increases with age at 5.9 mm, the length of the main trunk of the superior mesenteric artery - at 17 mm. The angle of discharge of the celiac trunk in men changes unevenly with age - in the second period of mature age, there is a statistically significant decrease by an average to 12.3°, then in old age an increase to 15°. Moreover, in women, the values of this indicator vary more evenly. The angles of discharge of other vessels did not show a clear age dependence, since this parameter is largely due to the constitutional features. The aorto-mesenteric distance changes with age only in women. In elderly age, it is on average 4.4 mm larger than in the first period of mature age. It was also found that there are significant differences between constitutional types identified using the Pinier index in the overwhelming majority of the morphometric parameters studied. The obtained information has a significant clinical importance, since it will allow objectifying the diagnostic criteria of various vascular syndromes and minimizing the risk of endovascular interventions.

Pancreatic cancer often invades the duodenum and causes obstruction, but rarely causes massive duodenal bleeding. A 68-year-old male was admitted to our hospital because of vomiting. Enhanced abdominal CT showed a hypovascular tumor with air bubbles in the uncinate process of the pancreas. The tumor invaded the duodenum and metastasized to the liver and peritoneum. The main trunk of the superior mesenteric artery (SMA) was circumferentially involved. After admission, he had hematemesis and melena. Emergency gastroduodenoscopy revealed pulsating vessels in the third portion of the duodenum and he eventually experienced hemorrhagic shock. Severe bleeding occurred from his mouth and anus like a catastrophic flood. It was difficult to sustain blood pressure even with massive blood transfusion with pumping. After insertion of an intra-aortic balloon occlusion catheter, the massive bleeding was eventually stopped. Although we attempted interventional radiography, aortography revealed direct communication between the main SMA trunk and the duodenal lumen. The tumor was considered anatomically and oncologically unresectable. Thus, we did not perform further intervention. The patient died 2 h after angiography. Herein, we report the case of pancreatic head cancer causing lethal bleeding associated with tumor-involved SMA. Duodenal bleeding associated with pancreatic cancer invasion should be considered as an oncogenic emergency.

Endovascular microcoil embolization of the superior mesenteric artery (SMA) main trunk is not currently employed as a treatment for penetrating injury to the SMA in hemodynamically instable patients. Immediate laparotomy is the initial gold standard therapy. We report a case of successful SMA microcoil embolization with preservation of mesenteric perfusion by an unusual endovascular technique.
