Differentiation Between Phenotypes of Polycystic Ovarian Syndrome With Sonography

Objectives: To differentiate between the classic (A/B) from the nonclassic (C/D) phenotypes of the polycystic ovarian syndrome (PCOS) with sonography. Materials & Methods: Retrospective analysis was performed upon 72 female patients with PCOS. All patients underwent transvaginal sonography. The patients were classified into classic and nonclassic phenotypes of PCOS. Image analysis was performed for the ovarian volume, follicle counts, follicle size, and endometrial plate thickness. Results: There were significant differences in the ovarian volume ( P = .001), follicle counts ( P = .001), follicle size ( P = .001), and endometrial plate thickness ( P = .001) between classic and nonclassic phenotypes of PCOS. The threshold value for ovarian volume, follicle count, follicle size, and endometrial plate thickness used to differentiate classic from nonclassic phenotypes were 12.5, 10.5, 5.25, and 5.75, respectively, with an area under the curve of 0.79, 0.82, 0.83, 0.77 and an accuracy of 75%, 73.6%, 79.2%, and 68.1%, respectively. The serum testosterone level and the body mass index were significantly higher in patients with classic than nonclassic phenotypes of PCOS ( P = .001, .04), respectively. Conclusion: Sonography findings can differentiate classic from nonclassic phenotypes of PCOS.

Background: Ovarian androgen production can be promoted by insulin resistance which leads to reproductive abnormalities in Polycystic Ovarian Syndrome (PCOS). A wide variety of female tissues can synthesize and secrete Prostate Specific Antigen (PSA). Androgens may take part a significant role in PSA secretion in PCOS. As insulin resistance stimulates androgen production, the baseline value of PSA may decline by insulin sensitising agents in PCOS. Present study is an attempt to measure the function of PSA as a marker of androgen excess in PCOS and to assess the role of insulin sensitising agent metformin in altering PSA level in PCOS.Methods: The study was undertaken to assess the insulin resistance, testosterone and PSA level in 45 women diagnosed as PCOS and 45 healthy controls. Alteration of insulin resistance, serum testosterone and PSA levels by metformin was also analysed.Results: A significant increase in testosterone, PSA level and insulin resistance was observed in PCOS cases when compared with control (p<0.001). When metformin was given for 4 months, improvement in insulin resistance and testosterone level was found in cases, but PSA values observed no change. Correlation was not found linking insulin resistance with PSA level prior to and after therapy.Conclusions: Serum PSA level could be detected in high significant concentration in PCOS women. Various researches explain that insulin resistance and BMI may perhaps control serum PSA level, but our result demonstrate no effect of insulin sensitising agent on serum PSA value.

Background: Ovarian steroidogenesis requires gonadotropin stimulation, Luteinizing Hormone (LH) is a key factor in the hyperandrogenaemia of the polycystic ovary syndrome. Progesterone is the primary regulator of Gonadotropin-Releasing Hormone (GnRH) pulse frequency; however, in the polycystic ovary syndrome, the GnRH pulse generator is relatively resistant to the negative feedback effects of progesterone.  Study aims to evaluate the association of Anti-mullerian hormone with serum androgen and gonadotropin level in adolescents and young women of Polycystic Ovary Syndrome (PCOS).Methods: This was a single centre observational Cross-sectional study carried out in the department of Endocrinology and metabolism, Medical College, Kolkata from March 2017 to January 2019. Total number of study subjects were 207 out of which 138 were cases.Results: The AMH had strong positive correlation with serum testosterone in both case and control groups (r 0.542, p<0.001 and r 0.57, p<0.001) respectively .After the adjustment of age and BMI , the AMH moderately positive  but extremely significant correlation with serum testosterone as compare to control.Conclusions: Hyperandrogenaemia and higher ratio of LH and FSH associated with higher serum AMH level is associated with the higher serum AMH in polycystic ovarian syndrome.

Polycystic ovarian syndrome (PCOS) is a common disorder for female with fertile age. Along with other clinical and biochemical manifestations, thyroid function and prolactin level may be altered in patients with PCOS. This study aimed to evaluate the clinical and biochemical status, as well as alteration of thyroid stimulating hormone (TSH), prolactin (PRL) level in patients with PCOS. Present study comprised of 100 diagnosed PCOS patients according to revised Rotterdem Consensus criteria. All patients were studied for serum testosterone, LH (lutenizing hormone), FSH (follicle stimulating hormone), blood glucose, lipid profile as well as TSH, FT4 (free thyroxin) and prolactin level. Out of 100 PCOS patients 97 had hirsutism, 64 had acanthosis nigricans, where menstrual irregularities were in 94 patients. Diastolic blood pressure (74±1.1 vs. 77±0.9, mmHg; p=0.017), total cholesterol (163±5.3 vs. 193±6.2 mg/dl; p<0.001), low density lipoprotein (LDL, 104±3.7 vs. 124±4.9 mg/dl; p=0.002) and frequency of acanthosis (25% vs. 75%; p<0.001) were significantly higher among the patients having BMI>25 Kg/m2 than those of have ? 25 Kg/m2. Among the fertile women (n=53), 47% had primary and 41.5% had secondary infertility; whereas of the total patients, 21% had altered thyroid function and 6.1% had raised prolactin (PRL, ng/ml) level. Differences of TSH (4.1±3.6 vs. 3.5±6.8, mIU/L; p=0.725) was not significant; whereas level of PRL (13.87±6.9vs. 9.4±5.2 ng/ml; p=0.018) was significantly higher in the group of primary infertility. Hirsutism, menstrual disturbance and acanthosis were very common in PCOS. Both primary and secondary sterility were also commonly observed and PRL was higher in primary infertility. Background: In women with Polycystic Ovarian Syndrome (PCOS), metabolic abnormalities are common, including insulin resistance, obesity, and dyslipidemia, suggesting an increased risk for cardiovascular disease (CVD). Objectives: To evaluate the effect of healthy lifestyle promotion on eating behavior and metabolic biomarkers in women with PCOS. Subjects and Methods: The study was carried out in 102 women (30±7 years) with PCOS. Patients received nutritional counseling based on Mediterranean diet principles and recommendations to practice a regular physical activity, and were followed up during six weeks. Dietary survey and biomarkers analysis were assessed at baseline (T0), after three weeks (T1) and 6 weeks (T2). Results: After six weeks of intervention, there was a significant decrease in total energy intake by (-30%) at T1 and (-32%) at T2 (p˂0.001), compared to baseline. Significant decrease was recorded in carbohydrates, proteins and lipids intake at T1 and T2 (p˂0.001). An increase in monounsaturated fatty acid intake was noted at T1 and T2 (p˂0.001). The body mass index (BMI) diminished at T2 (p˂0.01), and waist circumference at T1 and T2 (p˂0.001). Glucose values decreased at T1 (p˂0.05) and T2 (p˂0.001). Total cholesterol decreased at T1 (p˂0.05) and T2 (p˂0.01). HDL-C increased at T1 (p˂0.001) and T2 (p˂0.001), when CRP values remained unchanged. A decrease in Thiobarbituric acid reactive substances concentrations was observed at T1 and T2 (p˂0.05). Superoxyde Dismutase, Catalase activities and thiols amount increased at T2 (p˂0.001). Conclusion: In the long term, healthy lifestyle promotion could be beneficial to prevent or reverse the clustering of metabolic abnormalities and prevent CVD in women with PCOS. ABSTRACT Background Patients with polycystic ovarian syndrome (PCOS) are believed to have large ovaries due to increased stroma. They also have derangement in luteinizing hormone (LH) and testosterone levels and high insulin resistance. As insulin resistance is thought to be associated with androgen and stromal excess, correlation was expected between insulin resistance and stromal excess. Aim To assess if ovarian and stromal volumes in PCOS patients have any correlation with fasting and postprandial insulin levels. Materials and methods A prospective study of 153 subfertile patients was done over a period 18 months. After detailed history, clinical examination and informed consent of all patients were scanned by two-dimensional (2D) on day 3 of the cycle. Patients were divided into PCOS and non-PCOS groups according to Rotterdam criteria. Patients with hormonal derangements other than PCOS were excluded from the study. Patients were assessed by three-dimensional (3D) ultrasound (US) for ovarian and stromal volumes and fasting and postprandial insulin levels were checked on the same day. Results With Pearson correlation significance level of 0.354 (2 tailed) correlation for ovarian volume to fasting insulin was 0.588, for ovarian volume to postprandial insulin was 0.523, for stromal volume to fasting insulin was 0.601, and for stromal volume to postprandial insulin was 0.523. No correlation could be established in non-PCOS group. Conclusion In PCOS patients, a strong correlation was found between ovarian and stromal volume and fasting and post prandial insulin levels.
