The efficiency of Epworth sleepness scale and Berlin questionnaire for detecting Obstructive sleep apnea syndrome(OSA) in 100 Tunisian patients with non valvular atrial fibrillation(AF)

Obstructive sleep apnea syndrome (OSAS) is an important risk factor for atrial fibrillation (AF). GJA1 gene encoding connexin43, a major protein in cardiac gap junctions, plays a crucial role in the synchronized contraction of the heart and in cardiac arrhythmia. However, little is known regarding the role of GJA1 expression in the incidence of AF in patients with OSAS. All prospectively enrolled OSAS patients underwent polysomnography, electrocardiography, a 24-hour Holter test, and echocardiography. Moderate-to-severe OSAS was defined as an apnea-hypopnea index (AHI) ≥ 15. Exosomes were purified from the plasma of all OSAS patients and incubated in HL-1 cells to investigate the effect of exosomes from patients with and without AF on GJA1 expression. A total of 129 patients were recruited for this study; 26 were excluded due to an AHI < 15. Of the 103 enrolled patients, 21 had AF, and 82 did not. Multivariate analysis showed diabetes mellitus, lower sleep efficiency, lower left ventricular ejection fraction, and larger left atrial (LA) size were independent predictors of AF occurrence in OSAS patients. The area under the receiver operating characteristic curve for LA with a size ≥ 38.5 mm for predicting AF occurrence in OSAS patients was 0.795 (95% confidence interval [0.666, 0.925]); p < 0.001). GJA1 expression in HL-1 cells incubated with exosomes from OSAS patients with AF was lower than that with exosomes from patients without AF after controlling for age and sex and was negatively correlated with the AHI and oxygen desaturation index (ODI), especially during the non-rapid eye movement period (NREM) of OSAS patients with AF (all p < 0.05). LA size was an independent predictor of AF occurrence in OSAS patients. The AHI and ODI in the NREM period of OSAS patients with AF were negatively correlated with GJA1 expression in HL-1 cells, which offers a hint that GJA1 may play a crucial role in the development of AF in patients with OSAS.

Abstract Objective: To determine the relationship between frequency of traffic accidents and presence and severity of the disease in bus drivers who are at risk of having obstructive sleep apnea syndrome (OSAS). Method: In the present study, polysomnography (PSG) was applied on 162 city bus drivers directed to the sleep laboratory from a total of 1450 drivers after being determined as risky with regard to OSAS symptoms according to the questionnaire results. Their demographic characteristics, health status and accidents were compiled. Statistical analyses were made for those diagnosed with OSAS according to the PSG result and those with a traffic accident after which comparisons were made. Results: Obstructive sleep apnea syndrome was detected in 127 out of the 162 drivers determined to be risky with regard to OSAS based on the Berlin questionnaire result. While 35% of the drivers were normal according to the polysomnography (PSG) results in the study, 39 (24.1%) were determined as light OSAS, 35 (21.6%) as moderate OSAS and 53 (32.7%) as severe OSAS. While 105 (64.8%) of the cases had no accident, 37 (22.8%) were almost involved in an accident due to sleepiness and 20 (12.3%) were actually involved in an accident. A statistically significant relationship was determined between accident rates and OSAS severity (p:0.009; p<0.05). Conclusion: It was concluded that presence and increased severity of OSAS is an important risk factor for being almost or directly involved in an accident among city bus drivers, even though they were not driving for long distances.

Anthropometric Predictors of High Risk of Obstructive Sleep Apnea Syndrome in a Rural PopulationObjective. To evaluate which anthropometric parameter better predicts the high risk of obstructive sleep apnea syndrome (OSA) in a rural population. Material and Method. 254 subjects were enrolled. We measured weight, height, waist circumference (WC) and neck circumference (NC) and calculated body mass index (BMI), waist-to-height ratio (WHtR) and neck circumference/height ratio (NC/Height). The risk of OSA was assessed by using Berlin Questionnaire. Results. Subjects with high risk of OSA had a significant higher BMI, WC, WHtR, NC, and NC/Height. A higher percentage of those with large WC (≥80cm and ≥94cm for women and men, respectively) (p<0.001), WHtR ≥0.5 (p<0.001), NC ≥40cm (p=0.004), NC/Height ratio ≥0.23 (p=0.002) had a high risk of OSA. Using ROC curves of anthropometric parameters studied we found that WHtR was the best predictor for high risk of OSA, with AUC of 0.760, 95% CI: 0.699 to 0.815. Conclusions. WHtR was the best predictor for high risk of OSA as assessed by the Berlin Questionnaire.
