SummaryIndividuals who have suffered fractures caused by osteoporosis – also known as fragility fractures – are the most readily identifiable group at high risk of suffering future fractures. Globally, the majority of these individuals do not receive the secondary preventive care that they need. The Fracture Liaison Service model (FLS) has been developed to ensure that fragility fracture patients are reliably identified, investigated for future fracture and falls risk, and initiated on treatment in accordance with national clinical guidelines. FLS have been successfully established in Asia, Europe, Latin America, North America and Oceania, and their widespread implementation is endorsed by leading national and international osteoporosis organisations. Multi-sector coalitions have expedited inclusion of FLS into national policy and reimbursement mechanisms. The largest national coalition, the National Bone Health Alliance (NBHA) in the United States, provides an exemplar of achieving participation and consensus across sectors. Initiatives developed by NBHA could serve to inform activities of new and emerging coalitions in other countries.

This chapter looks at how the War on Women is manifested in multiple vulnerabilities for women, namely economic, bodily, and reproductive health. The aim of this chapter is to demonstrate that the legal restrictions placed on women’s bodies and lives serves to police and limit women’s citizenship and contribution to America’s polis. A need to control women is an anxious response to women’s continued independence, which is seen as a threat to patriarchal structures. As such, the chapter turns to the language used by the National Coalition for Men, the Red Pill forum on Reddit, and Breitbart to describe women’s positions in US and international society, demonstrating the sexual objectification and violently paternalistic attitude toward women that helped promote the Trump campaign.

IntroductionAdvocacy is vital for advancing tobacco control and there has been considerable investment in this area. While much is known about tobacco industry interference (TII), there is little research on advocates’ efforts in countering TII and what they need to succeed. We sought to examine this and focused on low- and middle-income countries (LMICs) where adoption and implementation of the Framework Convention on Tobacco Control (FCTC) tend to remain slower and weaker.MethodWe interviewed 22 advocates from eight LMICs with recent progress in a tobacco control policy. We explored participants’ experiences in countering TII, including the activities they undertake, challenges they encounter and how their efforts could be enhanced. We used Qualitative Description to analyse transcripts and validated findings through participant feedback.ResultsWe identified four main areas of countering activities: (1) generating and compiling data and evidence, (2) accessing policymakers and restricting industry access, (3) working with media and (4) engaging in a national coalition. Each area was linked to challenges, including (1) lack of data, (2) no/weak implementation of FCTC Article 5.3, (3) industry ties with media professionals and (4) advocates’ limited capacity. To address these challenges, participants suggested initiatives, including access to country-specific data, building advocates’ skills in compiling and using such data in research and monitoring, and in coalition development; others aiming at training journalists to question and investigate TII; and finally, diverse interventions intended to advance a whole-of-government approach to tobacco control. Structural changes to tobacco control funding and coordination were suggested to facilitate the proposed measures.ConclusionThis research highlights that following years of investment in tobacco control in LMICs, there is growing confidence in addressing TII. We identify straightforward initiatives that could strengthen such efforts. This research also underscores that more structural changes to enhance tobacco control capacity building should be considered.

The concept of critical realignment has shaped much of the thinking of political scientists and historians about the processes and patterns of change in American politics. Research on re-alignment has, however, tended to focus on successful cases and to concentrate on the electoral breakpoints rather than the process of regime formation, with the result that little systematic thinking has been devoted to the question of why some electoral upheavals lead to party realignment while other large vote shifts do not. This article begins from the proposition that the election does not so much constitute the realignment as offer the opportunity and the momentum for the new party to build a lasting national coalition. Whether the party capitalizes on this potential depends on processes and events that follow the critical election, during what could be called the ‘consolidation phase’ of the realignment. The question is ultimately one about public opinion, but the concept of consolidation needs to take in the interaction between the public and political elites, since mass opinion is formed in the context of elite initiatives and interpretations. The model of consolidation depicts two interrelated processes. The first involves strategic competition among elites, including elected officials and organized societal interests, who frame the conflict, by prioritizing issues and cleavages, and by relating policy proposals to group identities and widely-shared values. The second focuses on the public. Their standing loyalties disrupted by the crisis and the incumbents' inability to deal with it successfully, citizens engage in a process of experiential search as they seek to re-establish the stable political orientation given by attachment to a political party. The article draws on qualitative and quantitative information from the New Deal to illustrate the model of consolidation.
