Blunt aortic injury: Surgical treatment of the ascending and descending aorta

Traumatic aortic injury is potentially fatal. Although uncommon, involvement of the aortic arch and the ascending aorta can occur. This case shows concomitant dissection of the ascending and descending sections of the aorta after blunt chest trauma where the open surgical approach was successfully performed to treat both aortic injuries.

Aortic injury in closed chest trauma takes the second place in the structure of mortality in closed injuries. The type of aortic injury is a crucial factor in determining the optimal timing of the operation. For many years, the treatment strategy was reduced to immediate surgical intervention. The mortality rate in open operations is still high. The widespread introduction of endovascular technologies and aortic endoprosthetics has significantly reduced mortality and reduced the number of severe postoperative complications. With the advent of new endoprostheses, the possibilities of treatment have increased even more and its results have improved for various aortic injuries. A case of successful treatment of a patient with traumatic dissection and pseudoaneurysm of the thoracic aorta by endoprosthetics is presented.

Blunt chest trauma produces a variety of injuries. We present a case report of a hemodynamically stable patient after blunt chest trauma with radiographic images suggestive of the left fifth rib penetration to the heart. The diagnosis, surgical approach, and course of the patient are discussed.

Traumatic aortic injuries can be a lethal event. Almost 88% of patients with traumatic aortic injuries die within the first hour and only 2% survive long enough to develop a chronic aneurysm. Injury to the ascending aorta, whether acute or chronic, are typically managed with surgery, and those in the descending aorta, are managed conservatively or in some cases with stents. We present a rare case of a 53-year old gentleman with intra-aortic migration of a left clavicular prosthesis used for restoration of the left shoulder girdle.
