Uterine rupture of a non-communicating rudimentary horn pregnancy with resultant successful outcome of an extremely premature baby born at 24 weeks of gestation

Abstract Mullerian anomalies occur in about 0.5% of reproductive-aged women; many remain asymptomatic only being discovered during surgery or on imaging done for unrelated reasons. These result from the abnormal formation, fusion or resorption of the mullerian duct during development. A unicornuate uterus with a rudimentary horn is one of the rarer subtypes, with such anomalies making up about 5% of all mullerian anomalies. Pregnancy in a non-communicating uterine horn (NCUH) is extremely rare, with an incidence between 1:100,000 and 1:140,000. There is a high risk of uterine rupture if pregnancy occurs in a non-communicating uterine horn. We present here a case of a non-communicating rudimentary horn pregnancy, with a successful outcome.

Uterine rupture in first and second trimester in a non-scarred uterus is usually associated with Mullerian anomalies like rudimentary horn pregnancy. Such rudimentary horn pregnancy is difficult to diagnose antenatally but it carries grave consequences for the mother and fetus. Here is a case of undiagnosed ruptured rudimentary horn pregnancy in unicornuate uterus in Gravida 2 Para 0+1 lady at 18 week gestation who was admitted in Emergency with Shock and diagnosis was confirmed at laparotomy.

Unicornuate uterus with rudimentary horn is a rare type of anomaly. The incidence is approximately 1/100,000. Mullerian anomalies are associated with many obstetrical and gynecological complications. Non-communicating and functional rudimentary horn can cause severe pain in abdomen due to accumulation of the blood causing its distension. Authors report a case of 25 years old patient with unicornuate uterus with non-communicating rudimentary horn containing functional endometrium presenting with chief complaints of severe pain in lower abdomen associated with multiple episodes of vomiting.

Müllerian anomalies are uncommon but when present they can increase the risk of obstetrical complications. Anomalies such as bicornuate and unicornuate uterus can also increase the surgical risks of pregnancy termination. The diagnostic approach and surgical management must be individualized for each patient so that the termination procedure is safe and preserves fertility. We present a case of a patient with a 17-week pregnancy with fetal demise in a noncommunicating right uterine horn. Laparotomy and hysterotomy were required for evacuation of the fetus. The use of appropriate imaging studies to diagnose suspected uterine anomalies and a flexible and individualized operative strategy are essential for reducing complications associated with the termination of abnormal or unintended pregnancies in women with Müllerian anomalies.

The prevalence of congenital uterine anomalies has been reported as 6.7% in the general population. The unicornuate uterus accounts for 2.4-13% of all Mullerian anomalies and has a prevalence of 1:1000. A unicornuate uterus may be present alone or with a rudimentary horn and 75-90% of rudimentary horns are non-communicating. A pregnancy in the rudimentary horn may lead to uterine rupture, haemoperitoneum with a high risk of maternal mortality. The association of a unicornuate uterus with a non-communicating uterine horn to the VACTERL association of defects (vertebral anomalies, anorectal malformations, cardiovascular anomalies, tracheoesophageal fistula, esophageal atresia, renal anomalies and limb defects) has been described only twice in the literature. We report a patient who was born with an extra thumb and had a trachea-bronchial remnant with oesophageal stenosis that were both operated on earlier, and they were associated with a non-communicating rudimentary uterine horn. To the best of our knowledge, this is the third case reported in the literature of a VACTERL association to a unicornuate uterus with a non-communicating functional rudimentary horn. Given the high risk for the patient if pregnant an early diagnosis is paramount. This case highlights the importance of considering Mullerian defects in the young patient born with components of the VACTERL association.

A 45-year-old woman admitted to our hospital complaining of perimenopausal uterine bleeding not responding to medical treatment. Ultrasound evaluation revealed unicornuate uterus with adenomyosis and it was so difficult to see the distant small left rudimentary horn on ultrasound. The patient underwent laparotomy with total hysterectomy for both horns and was sent to pathologist that indicated adenomyosis and non-communicating non-cavitated left rudimentary horn.

Mullerian duct anomalies are rare. Unicornuate uterus with a non-communicating rudimentary horn is a rare type of mullerian duct anomaly which occurs due to defective fusion of malformed duct with contralateral duct. The incidence is approximately 1:100000. Patient usually remain asymptomatic due to the absence of functional endometrium in most of the cases. If the rudimentary uterine horn has an endometrium lined uterine cavity and doesn’t communicate externally then the signs and symptoms of obstructed menstruation appears, as soon as menarche begins. It will be associated with severe dysmennorhoea and hematometra. Other complications may be abdominal lump, chronic pelvic pain, infertility, endometriosis, adenomyosis and ectopic pregnancy in rudimentary horn. Authors are presenting a case of refractory dysmenorrhea with lump abdomen in a patient with unicornuate uterus with functional non communicating horn. In a patient with refractory dysmenorrhea mullerian duct anomaly should be kept as differential diagnosis.

AbstractApproximately 1 in every 76,000 pregnancies develops within a unicornuate uterus with a rudimentary horn. Müllerian uterus anomalies are often asymptomatic, thus, the diagnosis is a challenge, and it is usually made during the gestation or due to its complications, such as uterine rupture, pregnancy-induced hypertension, antepartum, postpartum bleeding and intrauterine growth restriction (IUGR). In order to avoid unnecessary cesarean sections and the risks they involve, the physicians should consider the several approaches and for how long it is feasible to perform labor induction in suspected cases of pregnancy in a unicornuate uterus with a rudimentary horn, despite the rarity of the anomaly. This report describes a case of a unicornuate uterus in which a pregnancy developed in the non-communicating rudimentary horn and the consequences of the delayed diagnosis.

Pregnancies within rudimentary horns of unicornuate uteri are extremely rare. These pregnancies account for approximately 1 in 76 000 to 1 in 140 000 worldwide, with uterine rupture being the most concerning complication. A case is presented of a 24-year-old female, gravida 2, para 0, who underwent multiple sonograms pertaining to two gestations over a 3-year period. Both pregnancies occurred within the left rudimentary horn of a unicornuate uterus. The left rudimentary horn was confirmed, on magnetic resonance imaging, to have no communication with the right horn or endocervical canal. The first pregnancy resulted in a missed abortion, and the second pregnancy, 3 years later, was a twin gestation, which was carefully monitored. At 20 weeks, the uterus began to rupture, and an emergency C-section was attempted. Neither the fetus nor the rudimentary horn survived, which was excised. This case demonstrates a rare uterine finding, a pregnancy occurred twice within the horn that lacked communication with the cervix.

Velamentous and furcate cord insertion with concomitant placenta accreta is a very rare and life-threatening event of pregnancy for both the mother and the fetus. Obstetricians should be cautious about umbilical cord insertion and placental adherence abnormalities in pregnancies conceived by assisted reproductive technologies (ART) particularly in women with Müllerian anomalies.
