Aim: Starting from a case presentation, this review aims to present literature data on inflammatory fibroid polyps (IFPs) of the small intestine. Methods: Case report and systematic review. A comprehensive systematic review of English literature using PubMed was conducted, based on Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines. The used key words were: “inflammatory fibroid polyp” or “Vanek”, including only cases with IFPs localized of the small intestine, published from 1976 to 2019. Results: We present a case of a 38-year old patient with intestinal IFP presenting with acute abdomen due to intussusception diagnosed with ultrasound (US) based on a target sign and visible solid tumor in the small intestine leading to prompt surgical treatment. A diagnosis of IFP was made based on the pathohistological findings. Moreover, a systematic review of small intestine IFPs was conducted which is, to our knowledge, the first comprehensive systematic literature review on this topic. The analysis included 53 case reports or case series concerning 77 cases of small bowel IFPs. The patients were aged from 4 to 75 years (average 45.2), with a female predominance (59.7%). The most common localization was the ileum in 77.9% cases, followed by the jejunum (13%) and the duodenum (6.5%). The most common clinical presentation was abdominal pain due to intussusception (63.6%). Regarding diagnostic methods, computed tomography (CT) was frequently used as primary diagnostic method (26%) followed by exploratory laparotomy (16.9%), endoscopy (7.8%) and US (6.5%). Combination of US and CT contributed to the diagnosis in 9.1% of cases. The majority of cases were treated surgically (92.21%), while only a minority benefited of minimally invasive techniques such as endoscopy. Conclusions: Small bowel IFPs, ones of the least common benign tumors, are characterized by variable clinical signs and symptoms and can potentially lead to serious consequences for the patient.

AbstractEvery five years or so a case of adult small bowel intussusception secondary to pathologies such as inflammatory fibroid polyp (IFP) appears in English literature. Likewise rare cases of adult colonic intussusception due to a tumour have been reported including, more recently, their successful management by laparoscopic approach. We describe two such cases, one each of small bowel and large bowel intussusception, due to IFP and caecal tumour respectively and discuss their management. We also suggest role of combined laparoscopy/endoscopy in selected cases of colonic resections.

Resumen Introducción: la intususcepción es el deslizamiento de un segmento de tracto gastrointestinal hacia otro distalmente adyacente, pudiendo alterar el transito gastrointestinal y su vascularización. Es más frecuente en niños y raro en adultos (5-16%), representando en estos últimos 1-5% de los casos de obstrucción intestinal. Una rara causa de esta es el pólipo fibroide inflamatorio (PFI). Presentamos un caso y revisamos la literatura. Material y métodos: se trata de un caso de intususcepción íleo-ileal causada por un PFI en una mujer de 26 años y realizamos una búsqueda en la literatura (actualizada el 9 de febrero de 2018). Como criterios de inclusión se seleccionaron aquellos pacientes con cuadros de intususcepción de intestino delgado por PFI. Se excluyeron los casos en otras localizaciones. Resultados: la mediana de edad fue de 45 años con un rango intercuartilítico (RI) de 35-58 años, siendo mas frecuente en mujeres (55%). La localización mas frecuente de los PFI el íleon. La duración de los síntomas hasta la valoración hospitalaria tuvo una mediana de 10 días (RI 3-45 días), presentando una masa palpable abdominal en 22 casos (22%). La tomografía computarizada fue la prueba más utilizada en el diagnóstico. En el tratamiento quirúrgico, la técnica más realizada fue la resección y anastomosis de intestino delgado por laparotomía. La mediana del tamaño del PFI fue de 3,5 cm (RI 3-5 cm). Discusión y conclusiones: La intususcepción causada por PFI en intestino delgado ha sido descrita en 99 ocasiones en la literatura revisada, considerándose un hallazgo excepcional.
