The impact of collateral information on ability estimation in an adaptive test battery

Abstract Introduction The transition from early childhood to teen years (5-12) is a critical time of development, which can be made particularly challenging by a burn injury. Assessing post-burn recovery during these years is important for improving pediatric survivors’ development and health outcomes. Few validated burn-specific measures exist for this age group. The purpose of this study was to generate item pools that will be used to create a future computerized adaptive test (CAT) assessing post-burn recovery in school-aged children. Methods Item pool development was guided by the previously developed School-Aged Life Impact Burn Recovery Evaluation (SA-LIBRE5-12) Conceptual Framework. The item pool development process involved a systematic literature review, extraction of candidate items from existing legacy measures, iterative item review during expert consensus meetings, and parent cognitive interviews. Results The iterative item review with experts consisted of six rounds. A total of 10 parent cognitive interviews were conducted. The three broad themes of concern were items that needed 1) clarification, needed context or were vague, 2) age dependence and relevance, and 3) word choice. The cognitive interviews indicated that survey instructions, recall period, item stem, and response choices were interpretable by respondents. Final item pool based on parental feedback consist of 57, 81, and 60 items in Physical, Psychological, and Family and Social Functioning respectively. Conclusion Developed item pools (n=198) in three domains are consistent with the existing conceptual framework. The next step involves field-testing the item pool and calibration using item response theory to develop and validate the SA-LIBRE5-12 CAT Profile.

Dopamine activity can modulate physical performance in the heat, but less is known about its effects on cognition during thermal stress. Twelves males completed a randomized, double-blinded protocol consisting of oral ingestion of 20 mg of methylphenidate (MPH) or placebo (lactose pill) during passive heating using a water-perfused suit (water temperature ~49°C). To identify the impact of peripheral versus central thermal strain, a cognitive test battery was completed at four different thermal states: baseline (BASE; 37.2±0.6˚C core, 32.9±0.7˚C skin), neutral core-hot skin (NC-HS; 37.2±0.3˚C, 37.4±0.3˚C), hyperthermic core-hot skin (HC-HS; 38.7±0.4˚C, 38.7±0.2˚C), and hyperthermic core-cooled skin (HC-CS; 38.5±0.4˚C, 35.1±0.8˚C). The cognitive test battery consisted of the 2-back task (i.e. working memory), set-shifting (i.e. executive function), Groton Maze Learning Task (i.e. executive function) and detection task (i.e. psychomotor processing). MPH led to significantly higher heart rates (~5-15 b·min-1) at BASE, NC-HS, and HC-HS (all p<0.05). There were no significant differences in the number of errors made on each task (all p<0.05). Participants were significantly faster (p<0.05) on the set-shifting task in the HC-HS timepoint, irrespective of drug condition (p>0.05). In summary, we demonstrated that 20 mg of MPH did not significantly alter cognitive function during either normothermia or moderate hyperthermia. Novelty: ● 20 mg of MPH did not significantly alter cognitive function during passive heat stress ● MPH led to significant higher heart rates (~5-15 bmin-1) in thermoneutral and during passive heat stress ● Future studies are needed to determine the mechanisms of why MPH improves physical but not cognitive performance during heat stress

Abstract Evidence suggests that women outperform men in core aspects of odor perception, and sex hormones may play a significant role in moderating this effect. The gender-affirming treatment (GAT) of transgender persons constitutes a powerful natural experiment to study the psychological and behavioral effects of high dosages of cross-sex hormone applications. Therefore, our aim was to investigate the effects of GAT on odor perception in a sample of 131 participants including female and male controls, as well as transmen and transwomen over their first 4 months of gender transition. The Sniffin’ Sticks test battery was used to measure odor detection, discrimination, and identification at baseline, as well as 1 and 4 months after the start of GAT. Plasma levels of estradiol, testosterone, and sex hormone-binding globulin were analyzed for each assessment point. Results revealed no significant change of olfactory performance in the two transgender groups compared with female and male controls. There was no significant difference between groups at baseline or any other time point. Neither biological sex, nor gender identity had an influence on odor perception. Moreover, there was no significant correlation between sex hormones and odor perception and between GAT-induced changes in sex hormones and changes in odor perception. Our results indicate that the effects of sex hormones on olfactory performance are subtle, if present at all. However, our results do not preclude hormonal effects on odors not included in the Sniffin’ Sticks test battery, such as body odors or odors associated with sex.
