Introduction The problem of treating chronic unstable pelvic injuries is characterized by the complexity of delayed one-step or staged reduction, difficulties in choosing the technology and treatment options, complexity of selecting criteria for evaluating planning along with the effectiveness of treatment. The use of spinal systems to perform reduction manipulations and fixation of the pelvis is described in few clinical observations. Therefore, the assessment of the effectiveness of the sequential use of various fixation systems in one patient was regarded by the authors as a rare opportunity and determined the purpose of the study. Purpose Demonstration of the possibilities of various osteosynthesis methods and their combination in reconstructive surgery of the pelvis in a case of its severe chronic injury. Materials and methods We present a clinical case and a brief analysis of the literature. This is a case of a patient with posttraumatic pelvic deformity and imbalance syndrome as a leading component of pelvic ring deformity. The effectiveness of reconstruction options, including those with the use of spinal systems, was analyzed. The treatment was assessed with radiological study methods (X-ray and CT) with balance evaluation on a digital platform and functional scores for the quality of life. Results Correction of frontal deformity of the pelvis was achieved with compensation for a relative shortening of the left lower extremity, restoration of the center of rotation of the hip joints, and relief of pain in the lumbar region. The functional state according to the Majeed score system and clear radiological signs of stabilization of the pelvic ring while maintaining the position of the acetabulum in the frontal plane prove the effectiveness of hybrid osteosynthesis with the use of spinal fixation systems. Conclusion The leading syndromic complexes are instability with clinical manifestations of non-union and pain along with an imbalance syndrome, manifested by a gross deformation of the pelvic ring. Syndromic evaluation determines the tactics of surgical treatment while a detailed planning of the sequence of intervention, choice of the level of osteotomy, combination of osteosynthesis options using spinal fixation systems provide the solution of reconstructive pelvic surgery tasks in one session.

Clinical observation of the patient with parasagittal meningioma at the level of the middle third of the superior sagittal sinus and falx is presented. When such tumors are removed, the main task is to prevent damage to the parasagittal veins, the injury of which can cause a persistent neurological defi cit in the postoperative period. A feature of this case is the complex use of advanced minimally invasive methods of angiographic diagnosis when planning surgical treatment, which signifi cantly increases the chances of a successful outcome of the operation.

Aim. Demonstration of potentials of preoperative planning and implementation of surgical resection in patients with adrenal cysts. A clinical observation of a successful surgical treatment of a rare pathology cyst of the right adrenal is presented. The choice of surgical treatment tactics is determined by the size of tumor and clinical presentation of the disease. The surgical treatment was accomplished laparoscopically which permitted to reduce the time of recovery and rehabilitation of the patient. In this clinical observation, the benefit of using 3D-modeling of the surgical area was shown for visualization of topographic and anatomic peculiarities and facilitation of the intraoperative navigation with the help of Avtoplan program developed by Samara State Medical University. Conclusion. Preoperative 3D-modeling permits to prepare to surgical intervention taking into account individual anatomic peculiarities of a patient, and to determine the optimal volume of the operation.

SummaryThree middle-aged (6-8 years), intact male Dachshunds were admitted to the University of Tennessee Veterinary Teaching Hospital (UTVTH) with acute onset of back pain and pelvic limb paralysis.Physical examination of dog #1 revealed hyperaesthesia of the thoracolumbar spine. Superficial pain sensation was absent in the pelvic limbs, while deep pain sensation was intact. The cutaneous trunci (panniculus) reflex was absent caudal to the thoraco-lumbar region. Reflexes to the pelvic limbs were exaggerated.A myelogram showed dorsal deviation of the ventral contrast column and attenuation of the ventral and dorsal contrast columns at the T9-T10 disc space. A hemilaminectomy was performed from T9 to T10, and mineralized disc material was retrieved from the vertebral canal at T9-T10. The T9- T10 to L3-L4 disc spaces were fenestrated. Two weeks after surgery, superficial and deep pain sensation of the rearlimbs were present, but paralysis persisted. The animal was euthanatised at the owners’ request. A postmortem examination was not performed.Dog #2 displayed absent superficial pain sensation, and intact deep pain sensation of the pelvic limbs. Hyperaesthesia of the thoracolumbar region and hyperreflexia of the rear limbs were noted. Examination of the cutaneous trunci reflex was not performed. A myelogram revealed ventral extradural compression of the spinal cord at T9- T10. A T9-T10 hemilaminectomy revealed a large amount of extruded disc material, which was removed. The Tll- T12 to L4-L5 intervertebral discs were fenestrated. Forty-eight hours postoperatively, the patient regained voluntary motor function, and recovery was uneventful.Neurologic examination of dog #3 revealed absence of deep pain sensation in the pelvic limbs; duration of which was not known. Spinal radiographs and myelography revealed ventral extradural spinal cord compression at T9-T10 (Fig.). A right-sided hemilaminectomy at T9-T10 revealed a large amount of calcified disc material, ventral to the spinal cord. The T11-T12 through Ll- L2 intervertebral discs were fenestrated. Sixty days postoperatively, paralysis persisted and the dog was euthanatised. No postmortem examination was performed.Herniation of the T9-T10 intervertebral disc was diagnosed in three Dachshunds with acute paraplegia. The clinical diagnosis and surgical management of T9-T10 disc herniation are similar to that in the more common sites of disc herniation.

This work aims at discussing through personal observations and literature review clinical semiology and surgical treatment.

The paraurethral cysts are benign cystic formations, the clinical symptoms of which vary depending on the size of the cyst. The presented clinical observation describes the clinical picture, stages of surgical treatment, and results of histological examination of a large paraurethral cyst in a 36-year-old woman. The review of modern methods of diagnostics and treatment of paraurethral formations in women is made.

The article is presented the description of rare clinical observation of the patient with pharyngeal lipofibroma complicated by compression syndrome, accompanied by impaired swallowing and breathing. The patient was successfully operated on with cervical access.
