Diseases of the musculoskeletal system are accompanied by nociceptive, neuropathic pain which is, in addition to kinesitherapy, physical therapy and acupuncture, widely treated using the local injection therapy. This treatment method is commonly used by physicians of different specialties for about 100 years and includes intra-articular, periarticular, perineural, intramuscular administration of different medicines, homeopathic remedies or medical devices. Hyaluronic acid salts, nonsteroid anti-inflammatory drugs, glucocorticosteroids, local anesthetics, vitamin B12, botulinum toxin A medications are often used for the treatment of degenerative conditions, posttraumatic joint disease, delayed complications of aseptic, bacterial and viral inflammatory, rheumatic processes within and out of the joints, such as: pathology of periarticular soft tissues as a consequence of local physical overload after acute injury, chronic microtrauma, sport trauma, affected facet joints and periarticular space at patients with dorsopathy, local muscle hypertonus (myofascial pain syndrome), neuropathy. The unique medication Hyalrepair-02/10 chondroreparant® based on the solid-state modificated hyaluronic acid salts and biogenic metabolites (non-drug bioorganic depot complex consisting of hyaluronic acid, amino acids and ascorbyl phosphates) is widely used at spine diseases and diseases of the musculoskeletal system, and it has showed good safety at local injection therapy. Heterogeneity of target tissues at patients with the musculoskeletal disorders, pharmacological substances as a part of injection drugs, and dosages of medicines and medical devices as well as individual patient reactions to the local therapy depending on severity of the disease, compensatory abilities and other reasons determines the necessity of carrying out a clinical comparison between different types of local injections with various groups of preparations.

As a rule, periodontal diseases are accompanied by marginal recession of the gums, loss of attachment and exposure of the roots of the teeth. Traditional drugs for injection treatment are being replaced by modern drugs based on high molecular weight hyaluronic acid, which are successfully and highly efficiently used for local therapy of inflammatory and destructive periodontal diseases. In order to restore the volume of soft tissues in the interdental spaces, eliminate recessions and bleeding of the gingival margin, a preparation of high molecular weight hyaluronic acid “Revident +” was used, which was injected into the base of the gingival papilla at a distance of 2—3 mm from its top with the bevel of the needle upwards. To assess gingival bleeding, the PBI (papilla bleeding index) index was used, which is a sensitive indicator of the severity of gingival inflammation. The magnitude of gingival recession before and after treatment was assessed according to its severity: mild — up to 3 mm, moderate — 3—5 mm, and severe — 6 and more mm. Examination of periodontal patients before and 1.5 months after the application of high molecular weight hyaluronic acid in the composition of the Revident + preparation showed that the degree of gingival bleeding according to PBI decreased significantly after the third visit from 4 to 2 and from 3 to 1, respectively. The degree of gingival recession in the area of the anterior group of teeth decreased by an average of 2.1 mm, in the area of premolars — by 2.5 mm, and in the area of molars — by 2.8 mm.

The experiments were performed on 280 male Wistar rats weighing 300-340 g. The design of the study included: modeling of compression injury of the soft tissues of the thigh, local injection of the preparation of hyaluronic acid "Gialift 3.5" into the area of damage 3 hours after the cessation of compression, taking into account the results after 7, 14 and 28 days. The introduction of hyaluronic acid into the area of compression injury of soft tissues reduces the severity of myoglobinemia, increases the immunohistochemical density of VEGF-positive cells, improves the perfusion characteristics of microcirculation, oxygen saturation, and specific oxygen consumption by tissues. The aim of the study is to find the best approaches to the treatment of infantile hemangiomas (IH). Materials and methods of research: the experience of treatment of pediatric patients with IH of various localization is presented, which was carried out according to the protocol elaborated by the authors. Systemic therapy with propranolol, local therapy, laser therapy were used in the complex of treatment. The age of patients at the beginning of therapy ranged from 1 month up to 4,5 years. Results: in all cases, patients with propranolol treatment showed a quick and stable effect. Even at the time of the selection of the dose of the drug, IH patients began to involute, became paler, less tense. The thickness of the soft tissues in the IH area decreased by 65% compared to the baseline, the number of functioning vessels decreased by 86%. The maximum effect was achieved within the first 6 months from the start of therapy. Clinical case report – a 2,5-year-old child with extensive hemangioma of the right buttock. There was a complex treatment with propranolol, local therapy, laser therapy. The general course of treatment with propranolol was 2 years 8 months. The total number of laser interventions was 6 with the overall effect in the form of maximum regression of residual clinical manifestations of IH. A positive clinical and ultrasound picture was noted, indicating a pronounced involution of IH, and a gradual withdrawal of propranolol was started. Conclusion: the effectiveness of a comprehensive interdisciplinary approach to the treatment of extensive complicated IH was demonstrated with the participation of specialists of various disciplines: pediatricians, cardiologists, surgeons, specialists in functional and ultrasound diagnostics, specialists in laser therapy, endocrinologists in a severe premature baby with a large number of somatic problems. The clinical treatment algorithm developed and implemented for patients with IH makes it possible to avoid more aggressive methods of treatment of this pathology and to improve the quality of life of this category of patients. On the basis on the results obtained, indications for laser therapy with a selective pulsed laser were formulated.
