<p class="abstract">Tubercular otitis media is a rare presentation of tuberculosis. High index of suspicion is required for diagnosis. We present a rare case of tubercular otitis media presenting as post auricular swelling with no evidence of pulmonary or any other foci of tuberculosis. Diagnosis was made on the basis of acid fast bacilli seen in fine needle aspirate. Anti-tubercular drugs were initiated and patient responded well to treatment.</p><p class="abstract"> </p>

Syphilitic balanitis of Follmann is a rare presentation of primary syphilis. It can occur before or after the appearance of primary chancre and some time without any associated primary chancre. We are reporting a case of syphilitic balanitis of Follmann, in a 23 years old heterosexual male, who presented with balanitis associated with single indurated lesion over coronal sulcus and few superficial ulcers over prepuce.

The Face

Mycosis fungoides is represented as the most common epidermotropic cutaneous T-cell lymphoma, which is mainly characterized by the proliferation of atypical cells within the epidermis. We report a rare presentation of mycosis fungoides in a 60-year-old male presenting with chronic psoriasiform plaque involving the face. Punch biopsy of the lesion from the forehead was taken for routine histological examination and immunohistochemical stains. Results of biopsy and immunohistochemical findings were consistent with mycosis fungoides and diagnosed as psoriasiform presentation of mycosis fungoides involving the face.

Association of hypercalcemia with SLE is very rare. Since initial report in 1991 only hand full cases have been described mainly in adult patients sporadically. Three pattern observed in this rare presentation. Commonly it is associated with serositis and lymphadenopathy or lymphedema known as hypercalcemia-lymphadenopathy SLE {HL-SLE) or hypercalcemia- lymphedema syndrome. Second group do not show this particular association. Concomitant primary hyperparathyroidism is responsible for third variety. This report describes a case of hypercalcemia associated with SLE from Bangladesh. <p class="abstract">Presentation of two papulosquamous disorders in a same individual is rare condition till date. Independently, psoriasis and Lichen planus (LP) are common inflammatory skin conditions affecting around 2-3% and 1% of HIV (Human immune deficiency) positive population respectively. As reviewed in the literature, psoriasis may be independently associated with other autoimmune conditions like vitiligo, alopecia areata, lichen planus, and discoid lupus erythematosus. In this article, we presented a case report of a HIV seropositive patient who suffered from psoriasis and lichen planus. The coexistence of psoriasis and lichen planus in one individual is rare and underreported in literature. Psoriasis or lichen planus may be the presenting feature of HIV infection and tends to be more severe, to have atypical presentations. Psoriasis and lichen planus can be coexistent or successionally appear one after other in one individual though rare presentation. High index of suspicion is always required while dealing with papulosquamous lesions in PLHIV.</p><p> </p>

Splenic Hydatid Cyst is a very rare presentation with less than 5% of total incidences of Cystic Echinococcosis. It is usually due to spread of cysts from other regions leading to secondaries. Primary isolated splenic hydatid cyst without involvement of any other organs is even rarer. We report an unusual case of a female who presented as Abnormal Uterine Bleeding (AUB) with an incidental finding of Isolated Splenic Hydatid Cyst with absence of involvement of any other organ. Patient underwent laparotomy and total splenectomy was done. This case report targets to report a rare case of incidental finding of isolated unilocular hydatid cyst of spleen and describes its management.

Myiasis is an infestation of live vertebrates (humans and/or animals) by larvae of dipterous fly. Although, it usually infects domestic and wild animals but humans may be rarely affected if they are reared in unhygienic condition. The index case is a seven-day-old male neonate born by normal vaginal delivery, who presented to the Emergency Department with complaint of passage of worms from umbilicus which do not extend to deeper tissues. On examination, periumbilical erythema was also visible along with white glistening worms. After application of turpentine oil to the umbilical stump, the worms were mechanically removed with the help of forceps. Umbilical myiasis is a very rare presentation indicating poor hygiene and a preventable condition.
