Abstract Background The concentrations of cytokines in the aqueous humor from neovascular age-related macular degeneration (nAMD) and polypoidal choroidal vasculopathy (PCV) may vary. The study was conducted to compare various cytokine levels in the aqueous humor of eyes with PCV, nAMD and control. Methods The present case control study included 49 treatment-naïve eyes from 49 patients (PCV 24, nAMD 11, and cataract 14 eyes). Totally 34 angiogenic and inflammatory cytokines in the aqueous humor were measured by Luminex bead-based multiplex array. Results After adjusting for gender and age by multivariate logistic analysis, concentrations of IL-31, LIF, SDF1-α, VEGF-A, VEGF-D were significantly higher in eyes with nAMD or PCV compared with control eyes (all P<0.05, times in nAMD: 59.5, 6.0, 7.0, 4.5, 5.6, respectively, times in PCV: 51.9, 5.21, 6.6, 4.0, 5.1, respectively ), and concentrations of HGF, IP-10, MCP-1, IL-13 were significantly lower in eyes with nAMD or PCV than in control eyes (all P<0.05, times in nAMD: 2.6, 2.0, 4.5, 4.7, respectively, times in PCV: 1.9, 3.0, 3.0, 2.8, respectively), but none of the 34 cytokines, including VEGF and IL-8, showed significantly different between eyes with nAMD and PCV. Conclusions Various cytokines involved in inflammation and angiogenesis including elevated IL-31, LIF, SDF1-α, VEGF-A, VEGF-D might be involved in the pathogenesis of nAMD or PCV. None of the 34 cytokines may help to differentiate nAMD and PCV.

The levels of IL-8, IL-18, IL-21, IL-31, LIF, SDF1-α, FGF-basic, VEGF-A, and VEGF-D in the aqueous humor were significantly higher in the nAMD group and PCV group than the control (nAMD vs control, p=0.004, 0.002, 0.005, <0.0001, <0.0001, <0.0001, 0.023, <0.0001, <0.0001, respectively, PCV vs control, p=0.031, <0.0001, <0.0001, <0.0001, <0.0001, <0.0001, 0.026, <0.0001, <0.0001, respectively). In contrast, the levels of BDNF, HGF, IP-10, MCP-1, and IL-13 in the aqueous humor were significantly lower in the nAMD group and PCV group than control (nAMD vs control, p=<0.0001, <0.0001, 0.003, <0.0001, <0.0001, respectively, PCV vs control, p=<0.0001, <0.0001, <0.0001, <0.0001, <0.0001, respectively). There were significant correlations among nAMD group, PCV group and control group in aqueous humor levels of HGF, IP-10, MCP-1, IL-13, IL-31, LIF, SDF1-α, VEGF-A, VEGF-D by multivariate logistic analysis.Conclusions Various cytokines involved in inflammation and angiogenesis including HGF, IP-10, MCP-1, IL-13, IL-31, LIF, SDF1-α, VEGF-A and VEGF-D may contribute to the pathogenesis of nAMD and PCV. Measurement of IL-8 and IL-12p70 in the aqueous humor may help to differentiate nAMD and PCV.

BACKGROUND: This study aimed to investigate the trend of selecting ranibizumab and aflibercept for the initial treatment of neovascular age-related macular degeneration (AMD) and polypoidal choroidal vasculopathy (PCV). METHODS: This was a retrospective study that included 460 patients who were diagnosed with treatment-naïve neovascular AMD and PCV and were initially treated with either ranibizumab or aflibercept. The patients were divided into two groups: the ranibizumab group (n = 96) and the aflibercept group (n = 324). The patients’ characteristics and the proportion of the subtypes of macular neovascularization (MNV) were compared between the two groups. RESULTS: Patients in the ranibizumab group were significantly older (mean 74.3 ± 8.4 years) than those in the aflibercept group (mean 70.4 ± 8.8 years; p < 0.001). In the ranibizumab group, the proportions of type 1 or 2 MNV, type 3 MNV, and PCV were 50.0%, 27.1%, and 22.9%, respectively. In the aflibercept group, the proportions were 35.2%, 6.8%, and 58.0%, respectively. There was a significant difference in the proportion of MNV subtypes between the ranibizumab and aflibercept groups (p < 0.001). Ranibizumab was used in 54.2% of patients with type 3 MNVs. However, in patients with PCV, aflibercept was used in 89.5% of patients. CONCLUSIONS: Ranibizumab was preferred as an initial treatment agent in older patients and those with type 3 MNV, whereas aflibercept was highly preferred in patients with PCV. The different characteristics and efficacy of the two agents may have partially contributed to this trend.

Abstract Purpose: To compare the choriocapillary flow density (CFD) among the fellow eyes of polypoidal choroidal vasculopathy (PCV), neovascular age-related macular degeneration (nAMD) and healthy controls with spectral-domain optical coherence angiography tomography (SD-OCTA). Methods: This is a cross-sectional study, including the fellow eyes of 38 patients with unilateral PCV, 36 patients with unilateral nAMD, and 36 eyes from 36 healthy volunteers. PCV group was further classified into polypoidal CNV (P-CNV) and typical PCV (T-PCV) for subgroup analysis. Age, subfoveal choroidal thickness (SFCT), Age-Related Eye Disease Study (AREDS) classification and fellow eye diagnosis were acquired. All subjects underwent 6.0 mm scan pattern of SD-OCTA. Circles with radius of 1.00, 1.50 and 3.00 mm were manually selected in choriocapillaris (CC) slab, and CFD was calculated as the percentage of flow area to the whole selected area as CFD-1.00, 1.50, 3.00 respectively. Univariate and multivariate analysis were performed to study correlation between aforementioned factors with CFD, Results: Mean CFD-1.00, 1.50, 3.00 of nAMD group were 61.51, 63.18 and 66.20, significantly lower than PCV group (65.90, 66.89, 67.94, P<0.001 , <0.001, and 0.010, respectively) and control (66.28, 66.96, 68.42, P<0.001 , <0.001, and 0.001, respectively), while no difference were detected between PCV group and control or between PCV subtypes. AREDS classification and fellow eye diagnosis were correlated with CFD in univariate analysis, but only fellow eye diagnosis remained significantly correlated after multiple linear regression. Conclusions: CFD of nAMD fellow eyes were significantly lower than that of PCV and control eyes, while no difference was detected between PCV and control group, indicating different role of CC loss in early pathogenesis of nAMD and PCV.

Abstract Background To compare the choriocapillary flow density (CFD) among the contralateral eyes of polypoidal choroidal vasculopathy (PCV), neovascular age-related macular degeneration (nAMD) and healthy controls with spectral-domain optical coherence angiography tomography (SD-OCTA).Methods This is a cross-sectional study. 36 eyes of 36 PCV patients, 35 eyes of nAMD patients and 34 control eyes were included. All sujects underwent the HD Angio Retina 6.0 mm scan pattern of OCTA (RTVue XR Avanti AngioVue; Optovue, Inc., Fremont, CA, USA). Circles with radius of 1.00, 1.50 and 3.00 mm were manually selected in choriocapillaries (CC) slab, and CFD was calculated as the percentage of flow area to the whole selected area as CFD-1.00, 1.50, 3.00 respectively. Multivariate linear regression analysis was performed to evaluate the differences of CFD among the three groups after adjusting for age, gender and the existence of drusen.Results Mean CFD-1.00, 1.50, 3.00 of nAMD group were 61.09, 62.65 and 65.67, significantly lower than PCV group (65.80, 66.75, 67.86, P= 0.000, 0.000, 0.022) and control (66.17, 66.45, 68.08, P= 0.000, 0.001, 0.025), while no difference were detected between PCV group and control ( P= 0.507, 0.798, 0.575).Conclusions CFD of nAMD fellow eyes were significantly lower than that of PCV and control eyes, while no difference was detected between PCV and control group, indicating different role of CC loss in early pathogenesis of nAMD and PCV.
