Despite the introduction of a number of effective disease modifying therapies (DMT) for multiple sclerosis (MS) over the past 25 years, a need remains for an effective method of treating breakthrough relapses, which impact disease progression. ACTH and corticosteroids have fulfilled this role for the past 50 years. However, based upon our current understanding of the cadence of an exacerbation, and its importance in the progression of MS, an alternative is proposed. This utilizes a very brief course of an ultra-high dose of intravenous methylprednisolone, adapted from the megadose protocol of methylprednisolone in acute spinal cord injury. Despite the known side effect profile of corticosteroids, and the ultra-high dose employed, the brevity in administration yields a well-tolerated, rapid resolution of the relapse

There are few more devastating injuries that afflict man than those associated with spinal cord injury (SCI). The economic, psychological, and social impact are encompassing and enormous to the individual and society. During the last several years, there has been a renewed interest in the study of SCI, with the hope of finding a cure. It is appropriate, then, to examine the efforts accomplished throughout medical history. With this retrospective view, potential avenues for future treatment become more apparent and clear. As in the past, the integration of basic science and clinical innovation will create the path toward progress for treatment of this disease.

Objective To review trends in bladder emptying methods over a 20-year period in patients with spinal cord injury (SCI) by severity according to the American Spinal Injury Association impairment scale (AIS).Methods Medical records of patients with SCI from 1994 to 1998 (group 1) and from 2012 to 2016 (group 2) were retrospectively reviewed. We classified bladder emptying methods according to the International Spinal Cord dataset. We grouped patients with normal voiding, bladder reflex triggering, and bladder expression as those using voiding without catheter.Results A total of 667 patients were included in the analysis. The proportion of patients using voiding without catheter and intermittent catheterization decreased from 67.0% to 30.0% and increased from 26.8% to 54.8%, respectively. In patients with AIS-A and AIS-B, the proportion of patients with intermittent catheterization increased from 32.8% to 73.3%. In patients with AIS-D, the proportion of patients using voiding without catheter and intermittent catheterization decreased from 88.5% to 68.9% and increased from 11.5% to 26.8%, respectively. In group 2, among 111 patients with AIS-D using voiding without catheter at admission, 8 (7.2%) switched to intermittent catheterization at discharge due to decreased bladder volume, increased post-voiding residual urine, or incontinence.Conclusion Over the past 20 years, trends in bladder emptying methods in patients with SCI changed from voiding without catheter to intermittent catheterization in Korea. This was especially prominent in patients with AIS-A, AIS-B, and AIS-C. Even in patients with AIS-D, the use of intermittent catheterization at hospital discharge increased.

ABSTRACT:Although medical advancements have significantly increased the survival of spinal cord injury patients, restoration of function has not yet been achieved. Neural transplantation has been studied over the past decade in animal models as a repair strategy for spinal cord injury. Although spinal cord neural transplantation has yet to reach the point of clinical application and much work remains to be done, reconstructive strategies offer the greatest hope for the treatment of spinal cord injury in the future. This article presents the scientific basis of neural transplantation as a repair strategy and reviews the current status of neural transplantation in spinal cord injury.

ABSTRACT Objective: To identify, describe, differentiate and qualify the instruments used to evaluate depression and also identify the percentage and average rates of depression in people with spinal cord injury. Method: This is a systematic review of the literature implementing a qualitative approach. SciELO, LILACS, PubMed and PsycINFO databases were consulted. Original articles were classified according to the variables (Spinal Cord Injury, Depression and Assessment instruments). The studies are presented according to the instruments in recent publications in the past 11 years. Results: Five evaluation instruments were found. PHQ-9, a screening instrument for diagnosing depression was used in most studies, with all being performed in the United States and having a higher impact factor and number of participants. Studies with PHQ-9 presented a total score of people who met the criteria for depression without grading by severity with an average of 17.6% ± 7.3%, while BDI presented 51.6% ± 15.1% and the HADS showed 37.5% ± 31% of people with some degree of depression. Conclusion: This study was important in identifying which instruments can be used to assess depression, the description of each, some of their differences, which ones are used in the best studies and o the depression rates in different parts of the world. It was also important to show how the PHQ-9 is an important instrument, but is limited to being used in the USA.
