Achieving aesthetic and functional result is a major challenge in oral rehabilitation, as the constant search for a harmonic smile raises patients level of demand and expectation. Restoring anterior teeth with aesthetics and predictability is one of the greatest challenges in contemporary dentistry and to know how to select the most suitable ceramic system and to have knowledge, controlling the variables at the time of adhesive cementation such as treatment of dental substrate surfaces and ceramic restoration will define the longevity and success of the restorative treatment. Therefore, the present article describes a rehabilitation with ceramic restorations on upper anterior teeth, seeking smile harmonization. It was emphasized clinical applicability in face of aesthetic challenge for different substrates, how to achieve predictable results with ceramic laminates associated with the lithium disilicate crown, and a crown with zirconia coping and laminate on it to mask the implant. This material presents excellent reproduction of dental structure optical properties in addition to high fracture resistance. A very pleasant aesthetic result was achieved, and also aesthetic harmonization of the rehabilitated teeth, restoring not only the aesthetic and saturation of the substrate, but promoting patient well-being associated with masticatory function and longevity for this clinical situation.

ABSTRACT Immediate esthetic rehabilitation is a common practice in contemporary restorative dentistry. Sometimes without a precise diagnosis and clinical planning, two or three clinical sessions have been sufficient to start and end an extensive all-ceramic oral rehabilitation. It is essential, however, for these treatments to be based on the biological, mechanical, and occlusal principles that have been established for decades, and not only on aesthetic considerations. In this case report, a 26 year-old bruxist patient with functional and aesthetic needs, was rehabilitated with ceramic restorations in the six maxillary anterior teeth, through an interdisciplinary approach. The diagnostic wax-up was essential to predict the outcome; guide the periodontal surgery; serve as a reference for dental preparations, and restore the dynamics of jaw movements. After the adhesive cementation, a protective occlusal splint for use during sleep was introduced. The authors concluded that careful diagnosis and planning was critical to outcome of this case, and that the quality and longevity of restorative treatment depended on the respect for biomechanical, occlusal, and aesthetic concepts.

<p>All-ceramic restoration of anterior teeth is a challenging clinical situation for which a variety of all-ceramic systems are available. The aim of this study is to present a case report of the construction of two single-unit metal-free fixed prostheses in anterior teeth. A 30-year-old male patient came to the Faculty of Dentistry, Federal University of Amazonas, concerned about his teeth. Elements 11 and 12 were endodontically retreated and fiberglass intracanal retainer posts were inserted. Dental crowns were prepared using the silhouette technique. The crowns were cemented with resin cement. The patient was very satisfied with his new smile and the aesthetic rehabilitation of the anterior teeth provided by the treatment.</p><p><strong>Keywords</strong>: Aesthetics; Oral rehabilitation; Dental porcelain.</p>

Oral rehabilitation with free-end removable partial denture (RPD) is one of the major challenges in prosthodontic dentistry. The absence of a distal abutment produces undesirable RPD movement during masticatory function due to the high resilience of the residual ridge, which limits the denture efficiency and may damage the abutment teeth and bone over time. The advent of dental implants made possible to substitute the missing teeth with fixed implant-supported dentures as the first choice treatment to overcome such inconveniences. However, this indication may not be suitable for all patients due to financial, anatomical or systemic health conditions. Nevertheless it is possible to improve free extension RPD by using implants in the posterior edentulous ridge to achieve biological, biomechanical, physiological and social benefits. This article aims to present a case report on oral rehabilitation in which a RPD was made combined with an implant for posterior support in a sizeable edentulous ridge. The patient appreciated the retention improvement and the aesthetic result. The combination of a RPD with a posterior implant is an alternative treatment for cases in which implant-supported fixed prosthesis is not indicated, reducing the displacement and minimizing the limitations of the free-end devices.

Background: Metal frame denture design is common used for partially dentate patient, one of the problem was a metal appearance from wrought wire or a metal casting clasp as retainer, which is not really aesthetic. Telescopic overdenture with its double crown will be the best solution for those cases. Purpose: The purpose of this case report was to reported telescopic overdenture design and fabrication where the aesthetic was the main consideration. Case: Sixty years old female patient, a businesswoman, came to Dental Hospital of Universitas Airlangga, complained about her upper partially dentate and 6 years lower fixed partial denture, which is not comfortable to wear and her upper anterior teeth doesn’t look quite good. She wanted to make crowns and removable denture. Case management: After the diagnosis were made, the pretreatment plan were remove crown for the upper jaw, remove the bridges on lower jaw, and transitional dentures were made to maintain the VDO. The crowns and veneer were fabricated and inserted while the inner copings were tried up on prepared teeth. After the the dentures were produced, the inner copings were luted to the abutment teeth, and then the dentures were inserted using FGP (Friction GriP) cement for the friction effect. Discussion: Telescopic overdenture is consists of a double crown system known as “the telescopic”, the procedure involves fitting the remaining natural teeth with inner metal crowns, followed by outer crowns as part of an over denture that can be removed by the patient. Conclusion: This technique ensures that telescopic overdentures give natural aesthetic result.

AbstractAmelogenesis imperfecta (AI) is an inherited disease that expresses a disorder in the development of enamel structure. In its mildest form, it promotes tooth color change; and in more severe cases, it presents a loss of enamel structure initiated during the eruption phase. Different AI manifestations can coexist in the same patient or in the same tooth, both in the primary and permanent dentures. In addition, several subtypes are described, characterized according to the variety of phenotype and genotype. Successful treatment requires early diagnosis and therapeutic solutions involving different dental specialties. Although some professionals prefer to postpone permanent rehabilitation until the development of complete permanent dentures, the aesthetic and functional impact of this disease in childhood and adolescence requires that restorative treatment be started as soon as possible. The proposed therapies demonstrate numerous challenges such as extreme dentinal sensitivity, difficulties installing and maintaining the orthodontic appliance and the need for restorative and prosthetic intervention in malformed teeth. This work aims to demonstrate the interaction between Orthodontics, Restorative Dentistry and Prosthesis in the treatment of a patient with AI, reporting the success of treatment involving aesthetics, function and well-being and the long-term benefit of this interdisciplinary approach for patients with this disease. ResumoA amelogênese imperfeita (AI) é uma doença hereditária que expressa uma desordem no desenvolvimento da estrutura do esmalte. Na sua forma mais branda, promove alteração na cor dos dentes; e em casos mais severos, apresenta perda de estrutura do esmalte iniciada durante a fase de irrupção. Diferentes manifestações da AI podem coexistir no mesmo paciente ou no mesmo dente, tanto na dentadura decídua quanto na permanente. Além disso, são descritos diversos subtipos, caracterizados de acordo com a variedade do fenótipo e genótipo. O sucesso do tratamento requer diagnóstico precoce e soluções terapêuticas que envolvam diversas especialidades odontológicas. Embora alguns profissionais prefiram adiar a reabilitação definitiva até o desenvolvimento da dentadura permanente completa, o impacto estético e funcional desta doença na infância e adolescência exige que o tratamento restaurador seja iniciado o mais cedo possível. As terapias propostas demonstram inúmeros desafios como a sensibilidade dentinária extrema, as dificuldades para instalação e manutenção do aparelho ortodôntico e a necessidade de intervenção restauradora e protética em dentes com má formação. O presente trabalho tem como finalidade demonstrar a interação entre a Ortodontia, a Dentística Restauradora e a Prótese no tratamento de um paciente com AI, relatando o sucesso do tratamento envolvendo estética, função, bem estar e o benefício em longo prazo desta abordagem interdisciplinar para os portadores desta doença. ABSTRACT The aim this study is to present a clinical case of conservative aesthetic restorative treatment of teeth 11 and 21 in a patient diagnosed with molar incisor hypomineralization. An 18-year-old female patient came to the Outpatient Restorative Dentistry Clinic at the Federal University of Espírito Santo, complaining mainly of not feeling comfortable with a color change in her anterior teeth. Molar incisor hypomineralization was diagnosed in molars and incisors and the negative psychosocial implications for the aesthetics involved were noticed. Due to factors such as age, financial viability, technical facility and low degree of severity, we opted for maximum preservation of the healthy tooth structure, through minimal surgical intervention to remove the irregular enamel only and subsequently restore the teeth involved (11 and 21) with resin composite. For the patient, this conservative procedure restored function and aesthetics, and thereby improved her emotional and social behavior. Conservative restorations can provide real solutions, especially in cases of molar incisor hypomineralization in anterior teeth where the patient's psychological state should always be considered. An appropriate assessment of the dental substrate and a careful restorative technique are necessary for success in terms of aesthetics and function and can quickly promote the patient's well-being.

Introduction: The aesthetic rehabilitation of endodontically treated teeth depends on the degree of destruction of the crown, the tooth in question, the bone support, the type of prosthesis and the types of forces to which these teeth will be submitted. The indirect composite resins have improved physical and mechanical properties thanks to the incorporation of inorganic components and multifunctional monomers with a greater number of bonding points, making them capable of solving the problems in which ceramic restorations would be indicated. Objective: To present a clinical case report of an aesthetic rehabilitation of anterior teeth with indirect composite resin as a viable and less expensive treatment alternative. Case Report: Upon clinical examination, extensive and deficient restorations were observed in the upper anterior teeth. The proposed treatment plan aesthetic rehabilitation of dental elements with ceromeres. Conclusions: This treatment represented a viable and less expensive option that can be used both in situations of great dental destruction and in small dental absences, in substitution for ceramic restorations.

ABSTRACT An ideal aesthetic standard consists of facial and smile harmony. Discolored teeth generally cause an imbalance in facial aesthetics, leading to low self-esteem and disturbance in social relationships. Causes of teeth discoloration are classified as intrinsic or extrinsic. Bleaching is considered a safe and effective treatment for discolored nonvital teeth and can be associated with restorative treatment to properly correct the dental color and maximize the aesthetic results. This paper aims to present two case reports of single discolored nonvital anterior teeth and the protocol used to reestablish the dental harmony of these patients by bleaching and direct composite resin restoration. To exert minimally invasive dentistry is very important to preserve tooth structure and reduce the risk of fracture. Internal bleaching, although may not be sufficient to reestablish the aesthetic in some cases, it is important to minimize dental preparation to perform an indirect or direct restoration. Dentists must take into account the patients expectations, dominate the restorative technique and know the dental materials to properly be able to restore function and aesthetics and consequently improve life quality and social relationships.

The replacement of single anterior teeth by means of endosseous implants implies the achievement of success in restoring both aesthetic and function. However, the presence of wide endoperiodontal lesions can lead to horizontal hard and soft tissues defects after tooth extraction, making it impossible to correctly place an implant in the compromised alveolar socket. Vertical augmentation procedures have been proposed to solve these clinical situations, but the amount of new regenerated bone is still not predictable. Furthermore, bone augmentation can be complicated by the presence of adjacent teeth, especially if they bring with them periodontal defects. Therefore, it is used to restore periodontal health of adjacent teeth before making any augmentation procedures and to wait a certain healing period before placing an implant in vertically augmented sites, otherwise risking to obtain a nonsatisfactory aesthetic result. All of these procedures, however, lead to an expansion of treatment time which should affect patient compliance. For this reason, this case report suggests a surgical technique to perform vertical bone augmentation at a single gap left by a central upper incisor while placing an implant and simultaneously to regenerate the periodontal attachment of an adjacent lateral incisor, without compromising the aesthetic result.

ABSTRACT Bulk Fill Composite resins are restorative materials that present low tension and polymerization shrinkage, allowing them to be inserted in a single increment of up to 4 mm of thickness, in a restorative cavity. This paper aims to report a clinical case of restorations done on posterior teeth with Bulk Fill restorative system, using selective acid-etching with self-etch adhesive system. Patient, male, 24 years old, presented a carious lesion on the occlusal surface of dental element 37 and an occlusal unsatisfactory amalgam restoration, on element 36. A restorative treatment with Universal Bond 3M / ESPE and bulk fill composite resin (3M / ESPE) was done. According to the case described, it was possible to observe that the materials and techniques used restored the form and function of the teeth involved, preserving pulp vitality with a satisfactory aesthetic result. The bulk fill resins employed offer practicality and decrease clinical time, with satisfactory clinical applicability in the aesthetic and functional rehabilitation of posterior teeth.
