ABSTRACT Immediate esthetic rehabilitation is a common practice in contemporary restorative dentistry. Sometimes without a precise diagnosis and clinical planning, two or three clinical sessions have been sufficient to start and end an extensive all-ceramic oral rehabilitation. It is essential, however, for these treatments to be based on the biological, mechanical, and occlusal principles that have been established for decades, and not only on aesthetic considerations. In this case report, a 26 year-old bruxist patient with functional and aesthetic needs, was rehabilitated with ceramic restorations in the six maxillary anterior teeth, through an interdisciplinary approach. The diagnostic wax-up was essential to predict the outcome; guide the periodontal surgery; serve as a reference for dental preparations, and restore the dynamics of jaw movements. After the adhesive cementation, a protective occlusal splint for use during sleep was introduced. The authors concluded that careful diagnosis and planning was critical to outcome of this case, and that the quality and longevity of restorative treatment depended on the respect for biomechanical, occlusal, and aesthetic concepts.

Anterior spacing is a common esthetic problem of patient during dental consultation. The most common etiology include tooth size and arch length discrepancy. Maxillary lateral incisors vary in form more than any other tooth in the mouth except the third molars. Microdontia is a condition where the teeth are smaller than the normal size. Microdontia of maxillary lateral incisor is called as “peg lateral”, that exhibit converging mesial and distal surfaces of crown forming a cone like shape. A carefully documented diagnosis and treatment plan are essential if the clinician is to apply the most effective approach to address the patient’s needs. A patient sometimes requires a multidisciplinary approach to correct the esthetics and to improve the occlusion. This case report describes the management of an adult female patient with a proclined upper anterior teeth, upper anterior spacing, deep bite and peg shaped upper right lateral incisor tooth through orthodontic and restorative treatment approach.

Amelogenesis imperfecta (AI) has been defined as a group of hereditary enamel defects. It can be characterized by enamel hypoplasia, hypomaturation, or hypocalcification of the teeth. AI may be associated with some other dental and skeletal developmental defects. Restoration for patients with this condition should be oriented toward the functional and esthetic rehabilitation. This clinical report describes the oral rehabilitation of a young patient diagnosed with the hypoplastic type of AI in posterior teeth and hypomatured type of AI in anterior teeth.

ABSTRACT Smile is an integral part of dentistry. Patients come with the prime aim of achieving esthetics. The dentist may preserve or alter the relationships of the visible components of the oral region to give the desired esthetic result. But, it is our responsibility to attain this result through a scientifically based and thoroughly planned treatment. The appearance of gingival tissue surrounding the teeth plays an important role in the esthetics of anterior region. Abnormities in symmetry and contour can significantly affect the harmonious appearance of natural or prosthetic dentition. There are a variety of cases of teeth wear, wherein interdisciplinary intervention is essential. Also the factor causing severe wear of natural teeth must be identified and eliminated, or reduced before attempting restorative treatment to improve long-term prognosis of restorative treatment. This article deals with a case of anterior teeth wear, stressing upon diagnosis, treatment planning and progression of treatment with regards to perio-prostho esthetics. ABSTRACT Aim The present case report describes the importance of interdisciplinary approach and gives an understanding on management of an adolescent with unilateral cleft lip and palate. Background Failure of fusion between medial nasal process and maxillary process or between the palatal process leads to the formation of clefts. Clefts are result of genetic or environmental factors or a combination of both. Common dental problems associated with clefts includes anterior and posterior crossbites, hypodontia, malformation and abnormal eruption pattern. Case report A girl, aged 15 years reported with a chief complaint of unesthetic appearance of her maxillary anterior teeth. She had unilateral cleft lip and palate and had received cheiloplasty and palatoplasty when she was in young age and rhinoplasty when she was 14 years of age. At pretreatment evaluation, she had concave profile with maxillary arch constriction and oroantral fistula and mesially tipped maxillary left canine. Conclusion This patient's treatment was unconventional, but it was successful in significantly improving her masticatory function and smile, along with favorable dental and facial results. Generalized esthetics and function were significantly improved in this patient without orthognathic surgery, and treatment results were stable 3 years after the appliance removal. Clinical considerations, sequencing of treatment phases as shown in this case report can be utilized while treating an adolescent with cleft lip and palate. Clinical significance If the skeletal discrepancy is mild and esthetic concerns are minimal, dental compensation by orthodontic treatment alone might be recommended. The cephalometric analysis and prediction tracings provide further information for deciding whether a patient can be treated by orthodontics alone, or by orthodontics and an orthognathic surgical procedure. A change in axial inclination of the teeth can camouflage the skeletal relationship adequately. However, one should be cautious in a growing patient, because he or she might outgrow the dental correction so that ultimately skeletal surgery would be indicated. Achieving aesthetic and functional result is a major challenge in oral rehabilitation, as the constant search for a harmonic smile raises patients level of demand and expectation. Restoring anterior teeth with aesthetics and predictability is one of the greatest challenges in contemporary dentistry and to know how to select the most suitable ceramic system and to have knowledge, controlling the variables at the time of adhesive cementation such as treatment of dental substrate surfaces and ceramic restoration will define the longevity and success of the restorative treatment. Therefore, the present article describes a rehabilitation with ceramic restorations on upper anterior teeth, seeking smile harmonization. It was emphasized clinical applicability in face of aesthetic challenge for different substrates, how to achieve predictable results with ceramic laminates associated with the lithium disilicate crown, and a crown with zirconia coping and laminate on it to mask the implant. This material presents excellent reproduction of dental structure optical properties in addition to high fracture resistance. A very pleasant aesthetic result was achieved, and also aesthetic harmonization of the rehabilitated teeth, restoring not only the aesthetic and saturation of the substrate, but promoting patient well-being associated with masticatory function and longevity for this clinical situation.

Klippel Feil Syndrome comprises of three characteristic deformities of short neck, a low dorsal hair line and restricted neck mobility. This is a case report of Klippel Feil Syndrome and its rare association with cleft of hard and soft palate, coarctation of aorta, dextrocardia and situs inversus. An interdisciplinary approach towards the management included cardiac surgery, cleft repair and complete oral rehabilitation of the patient. Presently the patient is undergoing speech therapy and is under regular follow up.

Abstract Aim The aim of this report is to describe the restorative treatment of an 18-year-old patient diagnosed with autossomal recessive hypocalcified-hypoplastic amelogenesis imperfect (AI). Background Esthetic and functional rehabilitations in AI cases are challenging and should consider individual aspects, such as age, socioeconomic status, AI type, and intraoral condition. Report AI was diagnosed in an 18-year-old patient. Summary Considering the short length of crowns and roots, patient life expectancy, minimal invasiveness, amount of treatment time required, and lower costs, oral rehabilitation with overdentures provided an adequate functional and esthetic rehabilitation of the patient. Clinical Significance This report demonstrated overdentures to be a viable, relatively inexpensive, and noninvasive treatment choice of a patient with AI with concerns about treatment longevity, invasiveness, cost, esthetics, and long-term maintenance. The oral rehabilitation of edentulous patients with immediate loading has become a safe procedure with high predictability. The success is related to immediate fabrication of a passive fit framework to attach the implants. Based on these considerations, this case report shows an alternative technique for mandibular rehabilitation using implants immediately loaded, where the framework was fabricated using cylinders with internal reinforcement and precast pieces, electrowelding, and conventional welding providing esthetics and function to the patient in a short period of time.

<p class="abstract">Oral rehabilitation of bilateral mandibular edentulous posterior segments becomes even more difficult if patient had severe deep bite in natural dentition. This case report demonstrates the intelligent way of managing severe deep-bite by adapting multi-disciplinary approach. Involved steps are: Mandibular subapical osteotomy of mandibular anterior segment to lower down the fragment; secondly, the maxillary anteriors were done with crown lengthening followed by decreased incisal display and thirdly by restoring vertical dimension of occlusion by replacement of missing posterior teeth. The addition of surgical intervention (anterior mandibular osteotomy) in the present case has found to be effective and quick adjunctive in achieving functional improvement in occlusion by reducing anterior teeth overlap (deep bite), by decreasing incisal guidance steepness and esthetic display of anterior teeth. Bilateral edentulous situation was managed by precision attachment retained cast partial denture. A satisfactory functional and esthetic result was obtained.</p>

The functional and aesthetic restoration of crown-root fractured anterior teeth with biological width involvement represents a challenge to the clinician, because it requires a multidisciplinary approach. The trans-operative fragment reattachment technique is a feasible and advantageous alternative since it makes it possible to reestablish the original aesthetics and function of the fractured tooth in a single appointment. This work presents the fundamentals of this technique, highlighting its advantages, indications and limitations. The applications of the surgical and restorative procedures are demonstrated by way of a case report.
