Tumor Testis Methachronous Bilateral Dengan Histopathology Berbeda

Kanker testis adalah keganasan padat paling umum pada pria antara usia 15 hingga 35 tahun dan insidensinya telah meningkat secara signifikan selama dua dekade terakhir. Tumor sel germinal adalah jenis kanker testis yang paling umum terjadi. Namun, tumor testis kurang umum di Asia dibandingkan dengan negara-negara barat, kejadiannya sangat rendah yaitu 0,4 per 100.000 populasi. Mereka selanjutnya diklasifikasikan sebagai seminoma dan non-seminoma. 40% dari semua tumor sel germinal adalah seminoma. Pasien dengan kanker testis unilateral adalah 500 - 1.000 kali lebih mungkin untuk mengembangkan karsinoma testis di testis kontralateral. Tumor sel germinal testis bilateral adalah entitas yang sangat langka, dan insidensinya berkisar dari 1% hingga 5% seperti yang diterbitkan sebelumnya dalam seri besar. Mereka terjadi secara metakron pada 80- 85% kasus dan serempak pada 15-20% kasus. Kami menyajikan kasus tumor testis methachronous bilateral dengan histopatologi yang berbeda pada pria berusia 29 tahun dengan hasil hystopatologycal adalah skrotum seminoma kiri, dan skrotum kanan adalah tumor kantung kuning telur. PURPOSE We report long-term paternity in men with stage I testis tumors who were managed initially by surveillance. PATIENTS AND METHODS One hundred five patients with clinical stage I nonseminomatous germ cell tumors of the testis were entered on a surveillance protocol and followed up for more than 10 years. Actual fertility potential was assessed by pregnancy. RESULTS Of the 105 patients, 41 (39%) have fathered children, which includes 36 of 78 (46%) patients while on active surveillance and five of 27 (19%) patients after treatment for relapse. Of 63 couples who attempted a pregnancy on surveillance or were presumed capable of impregnation (whether they tried or not), 41 (65%) were successful. CONCLUSION These results show that the majority of men with stage I testis tumor who are on surveillance after orchiectomy, have a suitable partner, and attempt impregnation achieve a successful pregnancy. Pregnancy rates appear to be less than reported in men who have a nerve-sparing retroperitoneal lymph node dissection (RPLND) because more patients on surveillance require treatment for relapse, which reduces their chances for pregnancy.

Primary testicular lymphoma, is a rare testis tumor that accounts for only less than 9% of all testis tumors. In the preoperative period, it is extremely difficult to distinguish this tumor from other testis tumors. Its diagnosis is done by histological analysis. Most commonly encountered histological type is diffuse large Bcell lymphoma. Adjuvant radiotheraphy and/or chemotheraphy is given after orchiectomy. Prognosis is worse than other testis tumors. Non-metastatic tumors indicates good prognosis within one year. Ongoing research in patients with primary testicular lymphoma, are on efficacy of adjuvant theraphies and preventive and cure effect on extranodal extension to central nervous system which is the most common site for recurrency. There are conflicting results because of the small number of patient size. Here we present two cases with primary testicular lymphoma at the ages 71 and 82.
