The bilobed flap has been extensively used in the reconstruction of distal nasal defects. It is a local flap that recruits skin from areas where there is relative skin mobility to close defects in areas where the skin has less plasticity. Besides its usefulness in the reconstruction of small to moderate cutaneous nasal defects it can also be used to reconstruct large defects located in other anatomical areas. We present a clinical case in which the bilobed skin flap was employed to reconstruct a large surgical defect of the neck resulting from excision of a basal cell carcinoma, with excellent functional and cosmetic results.

SUMMARY Basosquamous carcinoma (BSC), as described in 1910, is a distinctive variety of skin cancer and its etiology and pathological characteristics have generated much controversy over the years. Currently, BSC is considered a basal cell carcinoma (BCC) subtype with aggressive behavior and greater tendency for recurrence and metastases. We present a clinical case recently reported in our unit.

Basal cell carcinoma (BCC) is the most common type of nonmelanoma skin cancer (NMSC). It grows slowly and very rarely metastasizes but can cause substantial morbidity due to its tendency to relapse and locally invasive nature, especially when located on the face. Excision surgery is still the gold standard treatment for primary BCC and is usually followed by reconstruction procedure. Skin flap techniques vary widely, one of which is flap advancement technique. The main benefit of flap advancement technique is the ability to hide the excision line, thus resulting in an aesthetically sound outcome. We report a case of 72-year-old female with hyperpigmented plaque brownish lump on the left lateral cheek. A diagnosis of igmented basal cell carcinoma had been confirmed through histopathological examination. The patient was treated with wide excision surgery and the defect was closed by multiple advancement flaps. Follow-up after three months showed excellent cosmetic and functional outcome.

<p class="Abstract">Reconstruction of soft tissue at the nose following excision of basal cell carcinoma is always challenging, because of both functional and aesthetic importance of nose. The local flap is always preferable to skin graft as this produces “like with like” replacement, pliable cover and vascularized tissue over the skeletal framework. In this paper, we discussed six cases of nasal reconstruction with bilobed flap, forehead flap, and nasolabial flap. All flaps survived and the patients had satisfactory outcome.</p>

Defects resulting from surgical procedure may be challenging to reconstruct depending on the size and location. In case of large defects, primary closure is often not possible and thus requires skin flap or graft. In such cases, skin flap is advantageous as it produces a similar color and texture with the surrounding tissue. Skin flap techniques such as advancement flap, rotation flap dan transposition flap are useful in closing small defects. In moderate to large-sized defects, especially in areas with high tension, a combination of more than one type of flap might be necessary. This report describes two cases of large defects on the temple and cheek following basal cell carcinoma excisional surgery which were successfully closed using a combination of rotation and advancement flaps. Both patients showed excellent functional and cosmetic outcome.

Skin cancer as the most common cancer diagnosis tend to be increasing. This condition is a particularly significant issue in developed countries. This study aimed to describe the clinical features, histopathological features, complications, and early surgical treatment outcomes of skin cancer in CanTho Oncology Hospital from 2014 to 2015. This descriptive prospective study involved all patients with non-melanoma skin cancer that were examined and treated at Can Tho Oncology Hospital from July 2014 to March 2015. There were 78 cases selected. Skin cancer was found to be more common among older patients. The prevalence of basal cell carcinoma was found higher than squamous cell carcinoma with percentage worth 76.9% and 23.1% respectively. Worth 73.1% of all the patients in the study underwent surgery with wide resection and reconstruction. In this study, most patients were the elderly. The basal cell carcinoma was the most common. The main treatment was surgery with wide resection and reconstruction. Penelitian deskriptif prospektif ini melibatkan seluruh pasien dengan kanker kulit nonmelanoma yang diuji dan diobati di Rumah Sakit Onkologi Can Tho dari Juli 2014 sampai Maret 2015. Terdapat 78 kasus terpilih. Kanker kulit ditemukan lebih umum pada pasien yang lebih tua. Prevalensi karsinoma sel basal ditemukan lebih tinggi dibandingkan karsinoma sel skuamosa dengan persentase masing-masing 76,9% dan 23,1%. Sebesar 73,1% dari seluruh pasien dalam penelitian ini menjalani bedah dengan rekonstruksi dan reseksi yang lebar. Dalam penelitian ini, sebagian besar pasien adalah lanjut usia. Karsinoma sel basal adalah yang paling umum. Pengobatan utama adalah bedah dengan rekonstruksi dan reseksi yang lebar. Komplikasi jarang terjadi 1,3% dengan nekrosis lipatan kulit.

Abstract A basal cell ameloblastoma is an uncommon benign neoplasm and has a similar histological appearance to basal cell carcinoma of the skin. This clinical case report will discuss an ameloblastoma originating in the maxilla and of the baseloid variant. The objective of report is to highlight the possibility of misdiagnosis from this challenging case. A 34-year-old male was referred to the maxillofacial department with recurrent nasal regurgitation following dental extraction from the left maxilla 1 year previous. Clinically there was an oro-antral fistula visible and initial CT showed cystic lesion in the left maxilla likely to represent an odontogenic cyst. The patient underwent extraction and enucleation, where the diagnosis of the amelobalstoma was established with need of resection. Partial maxillectomy and primary closure with buccal fadpad was performed in the second stage and histological results showed a 6mm completely excised basal cell carcinoma within the oral mucosa and no bony invasion. Due to this extremely unusual results, secondary histological review was requested, and conclusion was a fully excised basal cell ameloblastoma. Following the procedure and subsequent reviews, the patient has no evidence of oro-antral fistula or recurrence of this rare type of neoplasm. A basal cell ameloblastoma variant is a rare lesion and due to its’ similar histological appearance to a basal cell carcinoma, can lead to a misdiagnosis. The aim of the report is to show the unusual presentation and findings along the way to correct diagnosis in this case.
