Lobomikosis merupakan penyakit yang menyerang bagian kulit yang disebabkan oleh Lacazia loboi (yeast-like organism) dari ordo Onygenales. Tulisan ini melaporkan seekor lumba-lumba hidung botol Indo Pasifik Tursiop aduncus jantan berumur 14 tahun yang mengalami kelainan pada kulit berupa nodul-nodul ulseratif menyerupai ringworm pada sirip ekor. Gejala yang ditimbulkan dengan cepat menyebar dan gejala pada permukaan kulit mengalami penggabungan nodul yang menyerupai bunga dan disertai adanya perdarahan. Pemeriksaan laboratorium ditemukan bentukan sel khamir dan bentuk unisel. Pengobatan dilakukan dengan menggunakan antifungal ketoconazole 200 mg (mycoral®) dan ketoconazole cream 2% selama 30 hari. Pengobatan lain yang digunakan adalah antihistamin Homochlorcyclizine HCl 20 mg (Homoclomin®) dan antibiotika Quinolone 500 mg (Ciproxin®) untuk mengobati infeksi sekunder akibat bakteri. Lumba-lumba hidung botol Indo Pasifik dinyatakan sembuh dengan menghilangnya nodul ulseratif dari sirip ekor setelah 30 hari pengobatan

SUMMARY The aim of this study was to evaluate the effects of the protein-calorie malnutrition in BALB/c isogenic mice infected with Lacazia loboi, employing nutritional and histopathological parameters. Four groups were composed: G1: inoculated with restricted diet, G2: not inoculated with restricted diet, G3: inoculated with regular diet, G4: not inoculated with regular diet. Once malnutrition had been imposed, the animals were inoculated intradermally in the footpad and after four months, were sacrificed for the excision of the footpad, liver and spleen. The infection did not exert great influence on the body weight of the mice. The weight of the liver and spleen showed reduction in the undernourished groups when compared to the nourished groups. The macroscopic lesions, viability index and total number of fungi found in the footpads of the infected mice were increased in G3 when compared to G1. Regarding the histopathological analysis of the footpad, a global cellularity increase in the composition of the granuloma was observed in G3 when compared to G1, with large numbers of macrophages and multinucleated giant cells, discrete numbers of lymphocytes were present in G3 and an increase was observed in G1. The results suggest that there is considerable interaction between Jorge Lobo's disease and nutrition.

In a previous study, the authors inoculated Swiss mice with Lacazia loboi (L. loboi) and succeeded in maintaining a granulomatous infiltrate and viable fungal cells up to one year and six months after inoculation. Considering the experimental work on paracoccidioidomycosis, 0.03 ml of a fungal suspension obtained from a biopsy of a Jorge Lobo's Disease patient were inoculated into both hind foot pads of 32 six week-old BALB/c mice of both sexes. The animals were sacrificed 1, 4, 7 and 10 months post inoculation. The suspension contained 1.3 x 10(6) fungi/ml and presented 38% viability. Seven months after inoculation, most of the animals presented profuse infiltrates consisting of isolated histiocytes, foreign body and Langhans' giant cells and a large number of fungi, most of them viable. Emergence of macroscopic lesions was observed during the 8th month. Based on fungal count, viability index before and after inoculation, presence of macroscopic lesions and histopathological findings similar to the findings in humans, the authors believe that BALB/c mice may be a good experimental model to study Jorge Lobo's Disease, mainly regarding therapeutic evaluation.

Lacaziose é infecção crônica causada pelo fungo Lacazia loboi, originalmente descrita por Jorge Lobo (1931) em paciente oriundo da Amazônia brasileira. Manifesta-se, sobretudo, por lesões cutâneas queloidiformes. Relata-se o caso de paciente com placa eritêmato-violácea, cuja hipótese diagnóstica inicial foi de hanseníase dimorfa tuberculóide, doença também endêmica na região amazônica. Ressalta-se a importância da inclusão de formas paucibacilares de hanseníase no diagnóstico diferencial da lacaziose.

<p>La lacaziosis es una rara micosis cutánea y subcutánea, de evolución crónica, ocasionada por el hongo profundo Lacazia loboi. Ha sido descrita principalmente en regiones tropicales y subtropicales.Clínicamente se caracteriza por lesiones cutáneas de aspecto queloideano y progresión lenta. La enfermedad se presenta más frecuentemente en adultos varones que desempeñan actividades laborales en el campo. Se presenta el caso de un paciente masculino de 68 años, agricultor, con una dermatosis crónica localizada en el miembro superior derecho, caracterizada por una neoformación compuesta por múltiples lesiones nodulares de aspecto queloideano con una lesión satélite proximal. El diagnóstico histopatológico fue compatible con una lacaziosis</p>

Caso de lobomicose em paciente procedente da região amazônica brasileira. Essa micose subcutânea, causada pela levedura Lacazia loboi, acomete, frequentemente, homens adultos e foi também diagnosticada em golfinhos. O tratamento depende da apresentação clínica: letrocoagulação, exérese cirúrgica e crioterapia são opções terapêuticas para as lesões localizadas, como a do paciente relatado, enquanto itraconazol e clofazimina, isolados ou em associação, podem ser empregados para lesões disseminadas. Até o presente, não há tratamento adequado para os casos com lesões disseminadas.
