Pagers and phone conversations have been the stalwarts of hospital communication. With good reason - they are simple, reliable and relatively inexpensive. However, with the increasing complexity of patient care, the need for greater speed and the general inexorable progress of health technology, hospital communication systems appear to be increasingly inefficient, non-secure, and inadequate. Thus, this study is proposed to answer the key research question: How can ICT (information communication technology) solutions ameliorate the current challenges regarding communication inefficiencies within healthcare? To answer this question, the study will design and develop a bespoke ICT solution for a specific context using three strong theories; communication theory, activity theory and agency theory to make a robust body of knowledge for the proposed solution. Further, it will serve to establish proof of concept, usability and feasibility of the proffered solution. The study participants will be selected from medical and nursing staff.

Due to the advancement of hardware technologies and mobile communication systems, the mobile devices are transforming into multimedia devices capable of consuming multimedia data. The mobile multimedia devices having the 3G/4G mobile communication interfaces have created the ubiquitous multimedia applications paradigm. The ubiquitous multimedia advocates that adaptable media contents should be available to users any time and any where. These ubiquitous multimedia applications have promising business potentials. The ubiquitous multimedia applications create an infrastructure for multimedia information management, where contents can be managed with interconnection and collaboration between users. The Personalized Ubiquitous Multimedia (PUM) is a subset of ubiquitous multimedia applications, where users can create, store, share and re-use the personalized heterogeneous media contents using mobile multimedia devices. Hence, PUM is an example of interconnected and collaborative multimedia content management system. This chapter illustrates the evolution of Computer-Phone and the concept of PUM. An integrated architecture is described aiming to deploy the PUM applications. The integrated architecture is composed of Mobile Agent Systems (MAS) and a specialized Mobile Distributed File System. A set of advantages of the integrated architecture is described.

Objetivo: descrever a construção de um instrumento de passagem de plantão utilizando a metodologia SBAR (Situation-Background- Assessment-Recommendation). Método: trata-se de estudo metodológico para construção de um instrumento de passagem de plantão em uma enfermaria de gastroenterologia cirúrgica. Foi realizada uma revisão bibliográfica abrangente sobre a metodologia SBAR. O estudo foi aprovado pelo Comitê de Ética em Pesquisa da instituição cenário da pesquisa. Resultados: os itens constantes do instrumento foram: dados de identificação do paciente que incluíram: data, leito, nome, idade sexo, data de admissão; indicadores: sistema de classificação de paciente, presença de acompanhante; Situação (S): diagnóstico médico e diagnósticos de enfermagem; Breve história (B): alergias, co-morbidades, histórico cirúrgico, isolamento/precauções e barreiras para a comunicação; Avaliação (A): sinais vitais, alimentação, eliminações, curativos, drenos, cateteres e medicamentos; Recomendação (R): interconsultas, intervenções de enfermagem e intercorrências. Conclusão: O instrumento contribui para a padronização da passagem de plantão dos profissionais de enfermagem.Descritores: Gerenciamento da prática profissional, Estudos de validação, Sistema de comunicação no hospital, Equipe de enfermagem.CONSTRUCTION OF A SHIFT CHANGE INSTRUMENTObjective: to describe the construction of a shift change instrument using the SBAR (Situation-Background-Assessment-Recommendation) methodology. Methodolgy: this is a methodological study for the construction of an instrument of a shift change in a surgical gastroenterology ward. A comprehensive bibliographic review was done on the SBAR methodology. The Research Ethics Committee of the scenario research institution approved the study. Results: the items included in the instrument were: patient identification data that included: date, bed, name, age, sex, date of admission; indicators: patient classification system, presence of companion; Situation (S): medical diagnosis and nursing diagnoses; Background (B): allergies, comorbidities, surgical history, isolation / precautions and barriers to communication; Assessment (A): vital signs, nutritional aspects, eliminations, dressings, drains, catheters and medications; Recommendation (R): interconsultations, nursing interventions and intercurrences.
