✓ A case of intracranial multiple aneurysms associated with an arteriovenous malformation (AVM) is described. Three aneurysms were found arising from an enlarged anterior cerebral artery feeding an AVM. In spite of the fact that two of these aneurysms received no surgical treatment, they disappeared almost completely several months after excision of the AVM. Seventy-three previously reported cases of cerebral aneurysms associated with AVM's are reviewed, and the effect of hemodynamic stresses on the development of these aneurysms is summarized.

✓ The authors report the cases of 32 patients with aneurysms measuring 2.5 cm or greater in diameter found among 1080 patients with saccular cerebral aneurysms. Of the 32 patients, 24 patients were treated by direct operation, four by common carotid ligation, and the other four by conservative therapy. The appropriateness of surgery and surgical method are discussed.

✓ Nine cases of intracranial aneurysm associated with cerebral arteriovenous malformation (AVM) were found in a total of 140 patients with cerebral AVM (6.4%). Radical operation was performed for both angioma and aneurysm in eight cases and for aneurysm only in one case. The suitability of surgical treatment of both aneurysm and angioma is discussed.

✓ We have operated on 12 of 14 cases of arteriovenous malformation (AVM) in the posterior fossa since 1968, with one death. The lesions were in the cerebellum in 10 cases (three anteromedial, one central, three lateral, and three posteromedial), and in the cerebellopontine angle in two; in two cases the lesions were directly related to the brain stem. The AVM's in the anterior part of the cerebellum were operated on through a transtentorial occipital approach.

✓ Two cases are presented in which clip occlusion of a third distal anterior cerebral artery segment occurred during treatment of anterior communicating artery aneurysms. Case histories, angiograms, operative descriptions, and postmortem findings are presented. The incidence of this anomalous vessel is reviewed. Preoperative and intraoperative vigilance in determining the presence of this anomaly prior to clip placement is emphasized.

Object. The aim of this study was to determine the prevalence of cerebral saccular aneurysms in patients with persistent primitive trigeminal artery (PPTA). The prevalence of cerebral saccular aneurysms in patients with PPTA previously has been reported to be 14 to 32%, but this rate range is unreliable because it is based on collections of published case reports rather than a series of patients chosen in an unbiased manner.Methods. The authors retrospectively evaluated their own series of 34 patients with PPTA to determine the prevalence of cerebral aneurysms in this population. The prevalence of intracranial aneurysms in patients with PPTA was approximately 3% (95% confidence interval 0–9%).Conclusions. The prevalence of intracranial aneurysms in patients with PPTA is no greater than the prevalence of intracranial aneurysms in the general population.

✓ Review of a series of 460 patients with spontaneous intracranial hemorrhage showed that 241 had demonstrable aneurysms and 38 had multiple aneurysms. The importance of demonstrating the entire circulation following ligation of one aneurysm is emphasized. Treatment of the 38 patients with multiple aneurysms is described. Ligation of unruptured aneurysms that are incidentally discovered is advocated in patients who are Grade 1 on the Botterell scale and who are well informed as to the risks.

✓ A patient is reported with a purely pial arteriovenous malformation (AVM) supplied from the posterior parietal artery. The prominent middle meningeal artery contributed to opacification of the angular branches distal to the AVM, but did not contribute to the AVM. After total removal of the AVM, the angular branches became opacified from the middle cerebral artery. Review of the literature suggests that hypertrophied dural arteries which do not contribute to the AVM's but which do opacify the cortical branches distal to the AVM's are rare.
