Latar belakang: Penggunaan endoskop memberikan spesifisitas yang tinggi dalam membedakan mukosa yang tidak sehat dengan yang sehat sehingga preservasi mukosa sehat dapat dioptimalkan danfisiologi mukosiliar dapat dipertahankan. Maksilektomi medial endoskopik adalah prosedur penanganantumor jinak untuk mengurangi morbiditas yang ditimbulkan oleh prosedur operasi terbuka. Tujuan:Untuk mengurangi morbiditas dan tingkat kekambuhan pada  penanganan tumor jinak sinonasal. Kasus: Dilaporkan 2 kasus tumor jinak sinonasal, yaitu inverted papilloma dan schwannoma yang ditangani secara maksilektomi medial endoskopik dengan mengoptimalkan preservasi mukosa danpatensi duktus nasolakrimal. Teknik marsupialisasi endoskopik dilakukan untuk menjamin patensi duktusnasolakrimal dengan bantuan sonde Bowman. Penatalaksanaan: Maksilektomi medial endoskopik danmarsupialisasi duktus nasolakrimal. Kesimpulan: Maksilektomi medial endoskopik dapat menjadi pilihanpada penatalaksaan tumor jinak sinonasal karena memberikan keuntungan pada pasien yaitu tidak adanyasikatriks pada wajah dan preservasi mukosa sehat lebih optimal. Endoscopic medial maxillectomy is a procedure for treating benignsinonasal tumor, aimed to reduce the morbidity caused by open surgical procedure. Purpose: To reducethe morbidity and the recurrence rates in the treatment of benign sinonasal tumors. Case report: Twocases of benign sinonasal tumor which were inverted papilloma and schwannoma, treated by  endoscopicmedial maxillectomy with optimal mucosal preservation and nasolacrimal duct patency. Marsupializationendoscopic technique was performed to ensure the patency of nasolacrimal duct supporting by Bowmansonde. Management: Endoscopic medial maxillectomy with marsupialization of nasolacrimal duct.Conclusion: Endoscopic medial maxillectomy could be an optional treatment for benign sinonasal tumorbecause the procedure has less facial cicatrix and optimal preservation of healthy mucosa, which areadvantages for the patient. Introduction.Inverted papilloma (IP) is an uncommon, benign yet aggressive neoplasm characterised by high recurrence rates and tendency towards malignant transformation. The majority of IP cases originate in the ethmoid region, lateral wall of the nasal fossa, and maxillary sinus. The authors report a case of an IP originating primarily from the nasolacrimal duct (NLD).Case.A 69-year-old Caucasian gentleman presented with a lump in his right medial canthal region, epiphora, and discharge bilaterally. Radiological investigation revealed a well-defined, heterogeneous mass within the proximal NLD eroding the bony canal, protruding into the middle meatus and into the right orbit. The tumour was excised en bloc utilizing a combined external and endoscopic approach based on its location. Histology revealed hyperplastic ribbons of basement membrane-enclosed epithelium growing endophytically into the underlying stroma with no evidence of invasive malignancy. The patient made an uneventful recovery with unchanged visual acuity and normal extraocular movements.Conclusion.The case demonstrates variability within the sinonasal tract that IP can develop and the individuality of each case necessitating tailored operative techniques for complete excision whilst minimising recurrence rates. We also present a combined endoscopic approach for the en bloc resection of a NLD IP with no clinical recurrence at 15-month follow-up.

For treatment of a sinonasal inverted papilloma (IP), it is essential to have a definite diagnosis, to identify its origin by computed tomography (CT) and magnetic resonance imaging (MRI), and to select the appropriate surgical approach based on the staging system proposed by Krouse. Recently, a new surgical approach named endoscopic modified medial maxillectomy (EMMM) was proposed. This approach can preserve the inferior turbinate and nasolacrimal duct. We successfully treated sinonasal IP with EMMM in a 71-year-old female patient. In this patient, the sinonasal IP originated from the entire circumference of the maxillary sinus. EMMM is not a difficult procedure and provides good visibility of the operative field. Lacrimation and empty nose syndrome do not occur postoperatively as the nasolacrimal duct and inferior turbinate are preserved. EMMM is considered to be a very favorable approach for treatment of sinonasal IP.

This series was undertaken to investigate the incidence of associated polyps and the recurrence rates of inverted papillomas with, or without, malignancies, as well as the treatment outcomes of the recurred cases.A retrospective study evaluating the pathology, associated polyps, types of operation, recurrence rate after original operation, and surgical outcomes of recurred cases was conducted upon 96 patients diagnosed with inverted papilloma.The overall malignancy rate was 11.5 per cent with synchronous tumours accounting for 9.4 per cent and metachronous for 2.1 per cent. Inflammatory polyps were found in association with inverted papilloma in 21.9 per cent. In cases without malignancies, the recurrence rate after the original conservative procedure was 33.9 per cent, compared to 14.3 per cent after a medial maxillectomy. In cases with malignancies, the recurrence rate after the original surgical procedure was 22.2 per cent.Our study indicates that more aggressive and bolder surgical resection of inverted papilloma should be undertaken as a primary treatment method, or as a treatment for recurrent cases to reduce the recurrence rate of inverted papilloma with, or without, malignancy.

BACKGROUND: To assess the efficacy of a new endonasal medial maxillectomy technique (EMM) for the treatment of inverted papilloma (IP). METHODOLOGY: A prospective series of 55 consecutive patients diagnosed with IP between March 2002 and April 2009 were entered into this study. The new surgical technique was applied to tumors arising from the anterior part of the maxillary sinus. After conventional EMM, the entire nasolacrimal duct was separated from the bony component of the nasolacrimal canal and preserved. Schirmer`s test and a visual analog scale (VAS) score were used to assess the lacrimal duct function after surgery. RESULTS: Ten of the 55 patients underwent the new surgical procedure. All patients were categorized with stage T3 or T4 tumors. No patients suffered tumor recurrence. There was no difference in lacrimal duct function between the diseased side and healthy side of the nasolacrimal duct. The mean VAS score was 2.8/100. CONCLUSIONS: This new surgical technique preserves the whole length of the nasolacrimal unit. It also offers several advantages including good visualization, nasolacrimal function after surgery and fewer adverse effects such as facial numbness and epiphora.

Abstract Inverted papilloma is a rare benign sinonasal tumor with a still unclear etiology. In the last decades, HPV implication in inverted papilloma etiopathogeny has been studied and demonstrated in some scientific research papers, but with no causality proven. Imagistic investigations and the Krouse classification establish the surgical approach. Endoscopic surgery represents the golden standard treatment for most cases of inverted papilloma, while external or combined approaches present limited indications. The authors present a case and therapeutic plan of an inverted papilloma located in the ethmoid sinus, with origin in the lamina papyracea and high-risk HPV implication.

Foreign body in the urinary bladder is not very common. It may occur by self insertion or migration from neighboring organs. It represents a urologic challenge which requires prompt management and should be treated as emergencies. Most of the foreign bodies in the urinary bladder can be successfully removed endoscopically. Sometimes open surgical procedure may require in removing the foreign body. Removal of foreign body from urinary bladder may be quite challenging requiring imaginations & high level surgical skills. Here we reported a case of 29 years old mentally retarded man with an intravesical foreign body. We successfully removed the foreign body by endoscopic procedure under general anesthesia. In post operative follow up there was no voiding problem & patient was advised for psychiatric consultation. JCMCTA 2012 ; 23 (2): 47-49
