Manual therapy and visual color-impulse therapy in the rehabilitation of patients with piriformis syndrome

Introduction. Piriformis syndrome (PS) is a condition accompanied by tension of the piriformis muscle and followed then by compression of the sciatic nerve passing through the piriformis muscle. According to statistics, PS occurs in 6–35 % of patients with lower back pain. Practitioners still face difficulties in treating patients with PS, and it necessitates the searching of new therapy methods and assessment of their compatibility.The goal of research — to study the clinical efficacy of the combined use of manual therapy and visual colorimpulse therapy in patients with piriformis syndrome.Materials and methods. A prospective, controlled, randomized study was conducted in 2019 at the Department of Sports Medicine and Medical Rehabilitation of I. M. Sechenov First Moscow Medical State University. In accordance with the inclusion criteria, 40 patients participated in the study. All participants, depending on the used treatment methodology, were divided by the method of simple randomization using envelopes into two equal groups. In the main group (group I), an integrated approach to treatment was tested: manual therapy in combination with visual color-impulse therapy (CIT); and in the other group (group II) only manual therapy was used.Results. The combined use of manual therapy and CIT in patients with piriformis syndrome leads to a significantly more pronounced decrease in the pain degree and normalization of impaired muscle tone. Also, an integrated approach helps to eliminate existing angiospastic disorders of the lower extremities.Conclusion. The study shows a clear positive dynamics in the integrated use of manual therapy methods in combination with visual color-impulse therapy in the treatment of patients with piriformis syndrome. It is planned to continue the study and assess the possibilities of using the combined technique in different groups of patients (athletes, pregnant women) with this syndrome.

In recent years, manual therapy has been increasingly used. This was facilitated by the scientific justification of the method, wellestablished training of specialists, good results of manual diagnostics and therapy of patients. Manual medicine is a system of diagnostic and therapeutic manual techniques aimed at identifying and treating disorders of the musculoskeletal system, manifested in the form of functional joint blockages, hypermobility and regional postural muscle imbalance. In addition to nosological diagnosis, manual examination data are of great importance for successful treatment. Manual diagnostics is a method aimed at detecting disorders in the spine, muscles and joints, as well as ligaments of the human body. The main task of diagnostics is to establish a differential diagnosis between diseases of the spine and joints and other nosological forms. Timely manual diagnostics with the use of manual therapy technologies helps to increase the effectiveness of complex treatment and helps to avoid complications and disability at an early age.

The urgency of the problem is due to the significant increase in recent years in the number of young and middle-aged people who have passed the war, the need to develop adequate measures of social assistance and protection. An important task in the organization of the rehabilitation process is to determine the structure of the needs of servicemen with disabilities in various types of medical and social care and the development of individual rehabilitation programs (IRP) based on them. The purpose of the study: to determine the structure of the needs of servicemen with disabilities in medical and social rehabilitation depending on the severity of disability. Statistical data from 25 administrative territories of Ukraine for 2018 are analyzed, the needs of participants of military service with disabilities in medical and social rehabilitation measures, their structure are calculated. The data of the information base of the centers and the bureau of medical and social examination of the regions were used. Processing of the primary material was performed using the universal statistical package "Excel". In 2018, medical and social expert commissions (MSEC) of Ukraine for the first time and re-certified and recognized persons with disabilities 7843 combatants. Of these, disability of group I (IA and IB) was established in 2.6 %, II – in 29.2 %, III – in 68.2 % of cases. All victims for MSEC were formed IRP. The dependence of the needs of combatants in medical and social rehabilitation measures on the severity of disability has been established. For persons with disabilities of groups II and III, priority is given to medical and professional rehabilitation, group I – social rehabilitation and technical means of rehabilitation with medical support. Of the medical rehabilitation services for persons with II and III groups of disability, sanatorium treatment is significant, and group I – rehabilitation therapy. Among vocational rehabilitation services, employment in production conditions is important – for persons with group III disabilities, in specially created conditions – for persons with group II disabilities, at home – for persons with group I disabilities. Vocational training was offered to a small number of people with disabilities of all groups. Among social rehabilitation services and technical means of rehabilitation, the services of social workers of territorial social service centers were significant for representatives of all disability groups, simpler means of transportation for persons of group III disability and more complex means for persons of group I. Determining the characteristics of the needs of servicemen with disabilities depending on its severity allows MSEC specialists to better form the IRP, develop targeted rehabilitation programs at the regional level, assess shortcomings in the organization of the rehabilitation process and build a rehabilitation system for combatants.

Background/Aim. Some studies suggest the benefit of applying different probiotic strains in combination with antibiotics in the eradication of Helicobacter pylori (H. pylori) infection. The aim of this study was to evaluate the effect of co-administration of multiple probiotic strains with triple H. pylori eradication therapy. Methods. This prospective study included 167 patients with dyspeptic symptoms and chronic gastritis who were diagnosed with H. pylori infection and randomized into two groups. The group I of 77 patients underwent triple eradication therapy, for 7 days, with lansoprazole, 2 ? 30 mg half an hour before the meal, amoxicillin 2 ? 1.000 mg per 12 hours and clarithromycin 2 ? 500 mg per 12 hours. After the 7th day of the therapy, lansoprazole continued at a dose of 30 mg for half an hour before breakfast for 4 weeks. The group II of 90 patients received the same treatment as the patients of the group I, with the addition of the probiotic cultures in the form of a capsule comprising Lactobacillus Rosell-52, Lactobacillus Rosell-11, Bifidobacterium Rosell-1755 and Saccharomyces boulardii, since the beginning of eradication for 4 weeks. Eradication of H. pylori infection control was performed 8 weeks after the therapy by rapid urease test and histopathologic evaluation of endoscopic biopsies or by stool antigen test for H. pylori. Results. Eradication of H. pylori infection was achieved in 93.3% of the patients who received probiotics with eradication therapy and in 81.8% of patients who were only on eradication therapy without probiotics. The difference in eradication success was statistically significant, (p < 0.05). The incidence of adverse effects of eradication therapy was higher in the group of patients who were not on probiotic (28.6%) than in the group that received probiotic (17.7%), but the difference was not statistically significant. Conclusion. Multiple probiotic strains addition to triple eradication therapy of H. pylori achieves a significantly better eradication success, with fewer side effects of antibiotics.

Not every case of neurologically based pelvic/genital numbness/incontinence is due to cauda equina syndrome. Pelvic pain, incontinence, and sexual dysfunction can result from treatable peripheral nerve injury or entrapment affecting the pudendal nerves or impar ganglion. Learning the signs, physical exam findings, tests, and surgical options greatly expands a neurosurgeon’s range. The pudendal nerve and nerve to the obturator internus muscle arise after S2, S3, and S4 spinal nerves traverse the piriformis muscle. They exit the sciatic notch with the sciatic nerve but then re-enter the pelvis, where the pudendal nerve then gives off bladder, rectal, and genital branches.

Abstract The barley chafer grub, Coptognathus curtipennis, is an important field pest of barley, wheat and linseed seedlings at Tikur Inchini district of theWest Shewa zone of Ethiopia in Ormoia regional state. Two entomopathogenic nematodes (EPN; Heterorhabditis bacteriophora and Steinernema yirgalemense, strains ANEPN21, ANEPN22, respectively) and three entomopathogenic fungal isolates of Metarhizium anisopliae (MM, EE and PPRC66) were compared separately at different rates of concentration for their effect against C. curtipennis in laboratory bioassays. Heterorhabditis bacteriophora and M. anisopliae isolate MM were found to be the most pathogenic to C. curtipennis at 1000 infective juveniles (IJ)/grub and 108 conidia (g soil)–1 concentration, causing 88.7% and 88.6% mortality, respectively, after 3-4 weeks exposure. These two most effective entomopathogens were further tested against the same insect in simultaneous and sequential combination with each other. Insect larvae were exposed to optimal concentration of H. bacteriophora and M. anisopliae isolate MM, which caused 50% larval mortality, i.e., 380 IJ/grub and 1.7 × 107 conidia (g soil)–1, respectively, as observed in the first experiments. The combined application increased larval mortality in an additive and synergistic manner. To achieve a strong synergistic effect, larva had to be exposed to M. anisopliae isolate MM 3 weeks before the addition of nematodes. The result of the whole experiment suggests that the damage caused by barley chafer grub can be controlled or managed by applying EPN and/or fungi. Furthermore, the combined use of M. anisopliae with nematodes may offer an integrated approach to increase the efficacy of EPN for C. curtipennis control and perhaps other insect pests.

ABSTRACT Introduction The increasing incidence of cesarean sections in India has caused a rise in the incidence of postpartum hemorrhage (PPH). There has been expanding interest in the role of misoprostol and tranexamic acid (TXA) in preventing and managing PPH during lower (uterine) segment cesarean section (LSCS). However, the lack of a published study comparing the efficacies of these drugs prompted us to conduct this study. Aims and objectives To compare the efficacies of sublingual misoprostol (600 μg) and intravenous TXA injection (500 mg) in reducing blood loss during LSCS by assessing intraoperative blood loss, perioperative hemoglobin (Hb) fall, and need for additional uterotonic agents. Materials and methods A total of 163 pregnant patients undergoing emergency/elective LSCS during the study period from 2013 to 2014 were randomly assigned to two groups — group I (82) received sublingual misoprostol 600 μg and group II (81) intravenous TXA 500 mg at cord clamping. Visual estimation of blood loss was done and 48 hours postoperative Hb and packed cell volume were measured to compare with preoperative values. Need for added uterotonics, blood transfusion, and adverse effects of drugs was assessed. The two groups were again subgrouped based on presence or absence of risk factors for PPH. Results The TXA significantly reduced blood loss compared with misoprostol (416 vs 505 mL) in patients without high-risk factors for PPH. Misoprostol caused significantly higher minor side effects while TXA reduced operation time. Conclusion The TXA can be routinely used after cord clamping along with oxytocin in patients undergoing elective/emergency LSCS to reduce perioperative blood loss, especially in those without risk factors for PPH.
