The management of youth with severe obesity is strongly impacted by social determinants of health and family dynamics. We present case studies of three patients seen in our tertiary care obesity treatment clinic as examples of the challenges faced by these patients and their families, as well as by the medical team. We discuss how these cases illustrate potential barriers to care, the role of child protective services, and we reflect upon lessons learned through the care of these patients. These cases highlight the need for comprehensive care in the management of youth with severe obesity, which can include: visits to multiple medical specialists, and mental and behavioral health providers; school accommodations; linkage to community resources; and, potentially, child protective services involvement. Through the care of these youth, our medical team gained more experience with using anti-obesity medications and meal replacements. The care of these youth also heightened our appreciation for the integral role of mental health services and community-based resources in the management of youth with severe obesity.

Hundreds of thousands of Americans are released from prison every year. Drawing on interviews conducted in the mid-2010s in the context of the Multi-site Family Study on Parenting, Partnering and Incarceration, this article explores how the strains of prisoner re-entry interact with those of poverty and family life, and how these combined strains condition proactive engagement with the legal system among re-entering individuals and their intimate and co-parenting partners. We focus our analysis on problems, tensions and struggles for control in parenting and partnership, including inter-parental violence, as these often led to calls or actions that clearly allowed for coercive intervention by parole authorities, courts, child support enforcement, or child protective services. We identify the precise circumstances and motives that lay behind such requests or allowances, and explain how these related to the cynical regard in which former prisoners and their partners typically held the coercive apparatus of the state. Through bringing our empirical findings into an interplay with scholarship on the role of punishment in the governance of poverty under neoliberalism, we examine how the strains faced by former prisoners' households and the tactics they used to deal with them pertain to broader politico-economic arrangements.

Many children are treated for mental health disorders in primary care settings. The system of care (SOC) provides a framework for collaboration among pediatric mental health providers, but it is unclear if youth treated for mental health disorders in primary care receive such coordination. At the South Boston Community Health Center from September /2012 to August 2013 for 74 individuals ≤18 years, the odds of contact with SOC agencies (mental health, education, child protective services, juvenile justice and developmental disabilities) were compared for mental health treatment in primary versus specialty care. The odds of SOC contact within primary care were lower compared to specialty care (OR = 0.43, 95% CI = 0.29-0.66), specifically for mental health (OR = 0.54, 95% CI = 0.25-1.2), education (OR = 0.12, 95% CI = 0.050-0.28), and child protective services (OR = 0.64, 95% CI = 0.22-1.9). As care coordination may improve health outcomes, increased support and education for care coordination specific to youth treated for mental health disorders in primary care settings may be warranted.

The objective of this article is to frame, understand, and draw implications from existing research on families screened out by child protective services (CPS) after a referral alleging child maltreatment. We review descriptive and outcome data together with emerging intervention research amidst a developing consensus that the current reactive role of CPS should be supplemented by supportive and preventative services that primarily address poverty. State-level data indicate that screened-out families are at high risk of re-referral and similar to higher-risk families reported to CPS. Intervention research is scant and mixed, but there is indication that providing community-based services may reduce future CPS involvement. Considering that screened-out referrals present an opportunity to prevent future maltreatment, CPS should identify and collaboratively engage screened-out families in community-based services. More research on the outcomes of these community responses is needed to identify best practices related to engagement and service provision.

AbstractThe COVID-19 pandemic has underscored the changing role of scientists, clinicians, ethicists, and educators in advocacy as they rapidly translate their findings to inform practice and policy. Critical efforts have been directed towards understanding child well-being, especially with pandemic-related educational disruptions. While school closures were part of early widespread public health measures to curb the spread of COVID-19, they have not been without consequences for all children, and especially for children from disadvantaged backgrounds. In a recent Isr J Health Policy Res perspective, Paltiel and colleagues demonstrate the integral role of academic activism to promote child well-being during the pandemic by highlighting work of the multidisciplinary academic group on children and coronavirus (MACC). In this commentary, we explore parallels to MACC’s work in an international context by describing the efforts of a multidisciplinary team at Johns Hopkins University in Baltimore, Maryland, United States, to aggregate data, conduct analyses, and offer training tools intended to minimize health and educational inequities for children throughout the COVID-19 pandemic. As both MACC and our work collectively demonstrates, multidisciplinary partnerships and public-facing data-driven initiatives are crucial to advocating for children's equitable access to quality health and education. This will likely not be the last pandemic that children experience in their lifetime. As such, efforts should be made to apply the lessons learned during the current pandemic to strengthen multidisciplinary academic-public partnerships which will continue to play a critical role in the future.
