Background: Analysis of synovial uid has been recommended as a routine procedure to assist in the diagnosis of arthritis. Arthritis can be either a monoarticular or polyarticular lesion leading to morbidity, affecting all ages . Aim of the study: To study synovial uid analysis in the diagnosis of joint diseases in a teaching hospital. Materials and methods: Prospective study was done on synovial uid samples over a period of two years at the Department of Pathology. ACS medical college ,Chennai for duration of 6 months ie, from February 2021 to August 2021. Results: Majority of the cases were osteoarthritis constituting 33.3 % .Rheumatoid arthritis constituted 20%..Chronic nonspecic synovitis were noted in 30% cases .01 case of Traumatic arthritis and 02 cases of Tubercular arthritis were noted. Conclusion: Synovial uid analysis will give us an idea about the differential diagnosis of joint diseases. Synovial uid aspiration should be done for the analysis and also used as a treatment procedure of synovial inammation.

ObjectiveTo investigate the potential of ultrasound (US) in the differential diagnosis between rheumatoid arthritis (RA) and psoriatic arthritis (PsA) at metacarpophalangeal (MCP) joints level.Methods18 RA patients and 20 PsA patients with clinical involvement of MCP joints were included. All US examinations were performed by two rheumatologists investigating: presence of joint cavity widening (JCW), synovial fluid and/or synovial hypertrophy, peritenon extensor tendon inflammation (PTI) and intra-articular or peri-tendinous power Doppler (PD) signal.ResultsA total of 83 MCP joints in 18 RA patients were assessed. In all of these the authors found different degrees of JCW. 15 of 83 (18%) MCP joints showed synovial fluid, whereas 68 of 83 (82%) MCP joints showed synovial hypertrophy. In 72 of 83 (86.7%) MCP joints intra-articular PD was detected. No PTI pattern was found in these patients.In PsA patients, a total of 82 MCP joints in 20 patients were assessed. 54 of 82 (65.8%) MCP joints showed PTI pattern (p = 0.001). In 50 of these 54 (92.5%) MCP joints extra-articular PD signal was detected (p = 0.001). 28 of 82 (34.1%) MCP joints showed different degrees of JCW. 6 of 28 (21.4%) MCP joints presented synovial fluid, whereas 22 of 28 (78.5%) MCP joints showed synovial hypertrophy. In 8 of 82 (9.7%) MCP joints the JCW and PTI patterns were found contemporaneously.ConclusionsPreliminary results demonstrate that PTI pattern is a higher characteristic of PsA, which suggests a potential role of US in the differential diagnosis between RA and PsA at MCP joints level.

Aim: This study was done to compare the effectiveness of drotaverine hydrochloride and valethamate bromide in shortening duration of labour. Methods: It is a hospital based prospective study, carried out in the department of Obstetrics & Gynaecology at Kathmandu Medical College Teaching Hospital (KMCTH), Sinamangal from 1st June to 31st August 2005. 99 patients, who were in active labour, were taken into the study. Out of which 50 were in drotaverine group (Group 1) who had received 40 mg of drotaverine hydrochloride intramuscularly at 3 to 5 cm dilatation of the cervix. Rest of 49 patients were in valethamate bromide group (Group 2). They received 3 doses of 8 mg valethamate bromide and 20 mg hyoscin butyl bromide intravenously half an hour interval. Result: The injection-delivery interval was studied in both Groups. The duration of injection-delivery interval was found to be almost equal in primigravidae of both Groups (p=0.72). But in multigravidae interval was significantly shorter in drotaverine Group (p=0.03). Conclusion: Both drotaverine and valethamate bromide are equally effective in shortening duration labour in primigravidae, where as drotaverine is more effective in multigravidae in accelerating labour.
