THE WANDERING INTRA UTERINE DEVICE: A RARE CASE REPORT

Intra Uterine Device (IUD) is one of the widely used reversible methods of female contraception. There are many complications reported with IUD insertion, among which device migration and delayed perforation of viscera like small bowel is one of the rare and serious complication. Here we report a case of 29 year old female who presented with sudden onset of abdominal pain associated with vomiting. Her Computed Tomography (CT) scan of abdomen revealed IUD lying within peritoneal cavity causing terminal ileal perforation. She was treated surgically and she recovered well. Extra uterine visceral organ injury many months or years following IUD insertion is rare and should be considered as a differential in any female with IUD presenting with abdominal symptoms.

Worldwide tubal sterilization is commonly used procedure for family planning method. Tubal ligation by minilaparotomy under local anaesthesia is most commonly used method of female sterilization in India. The death rate after tubal sterilizations is 72/100000 for all procedures and mainly due to general anaesthesia or vascular injuries. Iatrogenic injury to bowel can occur in minilaparotomy tubal ligations if there are dense adhesions of intestines or history of previous surgery. In the present case of interval post tubal ligation by minilaparotomy, fatal ileal perforation due to typhoid fever was detected on autopsy which was confirmed after complete histological and lab investigations. Atypical complications of typhoid fever were also noted. In developing countries, typhoid fever is the leading cause of non-traumatic free perforation of intestine and its incidence ranges from 0.9% to 39%, with a high mortality rate. This is rare case of an alleged medical negligence after surgery; in which deciding factor was cause of intestinal perforation i.e. iatrogenic or natural and it posed a difficult challenge.

Mucocoele of appendix and partial non-rotation of gut are rare pathological entities, and when they are present together they pose diagnostic and therapeutic difficulties due to atypical clinical presentation. This dilemma of varied clinical presentation can lead to delayed diagnosis and complications, thereby adversely affecting patient management and outcome. We report a 33 year old lady, presenting with intermittent abdominal pain in the periumbillical since 3 months diagnosed to have an appendiceal mucocoele with partial non rotation of gut on imaging. The patient underwent laparoscopic appendectomy and was discharged on post operative day 1 following an uneventful course with no recurrence on 6 months of follow up. Histopathology of the specimen confirmed the diagnosis of mucocoele of appendix with no evidence of malignancy. To our knowledge, this is the first reported case of mucocoele of appendix with partial non rotation of gut managed with minimally invasive laparoscopic approach.

The ocular involvement of the cysticercosis is a rare entity involving eyelids, extraocular muscles, orbit, conjunctiva, anterior chamber, uvea, retina, vitreous and optic nerve. All the extraocular muscles are involved in myocysticercosis. Ultrasonography and Computed tomography are the imaging modalities for evaluation of ocular cysticercosis. The patients can be treated with systemic steroids and albendazole; however surgical excision is the treatment of choice. We report a rare case of extraocular myocysticercosis in nine years old boy diagnosed on ultrasonography and computed tomography. Primary pulmonary meningiomas are rare and mostly benign tumors. They usually appear as a solid peripheral pulmonary nodule on chest radiography and computed tomography and are frequently diagnosed incidentally. Herein, we report a 55-year-old female case of primary pulmonary meningioma mimicking pulmonary metastasis.

Hydatid cyst is a parasitic disease caused by Echinococcus granulosus or Echinococcus multilocularis. Humans are accidentally infected with the parasite. The cyst is usually found in the liver and lungs and rarely occurs in other body parts. The present article describes a rare case of pelvic hydatid cyst in a young man who presented with nausea, vomiting, and right abdominal pain. Two large cystic masses were discovered during a CT scan in the patient’s pelvic region, resulting in right urinary tract hydroureteronephrosis. Additionally, the antibody index was used to confirm the presence of a primary hydatid cyst.

Gall bladder carcinoma is an exceedingly rare and fatal cancer with a high mortality rate.  Detecting gall bladder carcinoma in early stages can be difficult, despite improvements in ultrasound and computed tomography (CT) imaging. Most diagnoses of gallbladder carcinoma are made at advanced stages, with majority being found incidentally during surgery for cholelithiasis.  The presented case demonstrates suspicion of Gallbladder carcinoma pre-operatively.

Infarcted regenerative nodules in a cirrhotic liver is a rare condition that may be confused with hepatocellular carcinoma on imaging. We report here a case of a 58-year-old man with live cirrhosis who presented with abdominal pain and distension and sudden onset of haematemesis. Computed tomography (CT) showed diffuse multinodular infarcted regenerative nodules and gastric bleeding. Physicians should include infarcted regenerative nodules in any differential diagnosis of multiple hepatic lesions in liver cirrhosis, particularly in patients with gastrointestinal varices.
