DESMOID TUMOR OF THE ANTERIOR ABDOMINAL WALL IN A YOUNG FEMALE: A RARE CASE REPORT

Desmoid tumours are rare lesions which has a strong tendency to invade locally and to recur. They constitute 3% of all soft tissue tumours and 0.03% of all neoplasms. These tumours are commonly seen in women of fertile age, especially during and after pregnancy. We report a case of thirty year old female patient presenting with a painless mass in the anterior abdominal wall with past history of abdominal surgery 8 years back. Ultrasonography revealed a solid, heterogenous hypoechoic lesion within abdominal wall. On Fine needle aspiration cytology of the mass, spindle cell tumour suggestive of desmoid tumour was diagnosed. Wide local excision of the affected abdominal wall mass was performed and the defect was replaced with a polypropylene mesh. The specimen was sent for histopathological examination where cytological diagnosis was conrmed as desmoid tumour of anterior abdominal wall.

In this paper, we report a case of bilobed schwannoma, presented in the roof of orbit arising from supraorbital nerve. A 62-year male presented with a nontender mass in superior part of orbit and eccentric proptosis. Visual acuity and rest of ocular examination were normal. CT scan and MRI orbit revealed an extraconal homogenous bilobed mass, of size 3.5 to 2.5 cms in roof of orbit. Fine needle aspiration cytology was done, which was suggestive of schwannoma a peripheral nerve tumor. Successful surgical excision of intact bilobed schwannoma was done with careful separation and preservation of supraorbital nerve from which it was originated. Postoperative period was uneventful though rare, less than 1%, schwannoma can present as painless mass in the orbit and proptosis. Treatment of choice is surgical excision of intact tumor to prevent recurrence and preservation of peripheral nerve from which it arises.

Abstract:Ganglioneuroma is a benign tumor that originates from primordial neural crest cells which migrate from the mantle layer of the developing spinal cord to the sympathetic ganglia, adrenal medulla, and other sites. The most affected anatomical sites are the posterior mediastinum, retroperitoneum, adrenal gland, head and neck. It occurs most commonly in children over 10 years of age and consists of ganglion and Schwann cells. We report a case of 9 – year- old male child who presented with mass per abdomen in the right hypochondrium. Fine needle aspiration cytology showed mature ganglion cells and cluster of Schwann cells suggestive of Ganglioneuroma. Cytology diagnosis was confirmed by histopathological examination of tru-cut biopsy and subsequently excised specimen. On regular follow he is doing fine without any symptoms.

Extra skeletal soft tissue chondroma (ESSC) is very rare slowly growing benign tumours usually occur in hands and feet. Lesion must be differentiated from other benign and malignant lesions in these locations by clinical, radiological, cytological and histopathological examination for proper management of such benign entity. A case is presenting here in a thirty five years old lady suffering from similar type of lesion over right thumb diagnosed preoperatively by fine needle aspiration cytology because of rarity. Cysticercosis is a systemic parasitic disease caused by the larval form of cestodeT. solium. It has a worldwide distribution and is potentially harmful with variable clinical manifestations. The patient most commonly presents with subcutaneous and muscle involvement in the form of nodular lesions. The other most commonly involved sites include eye, brain, bladder wall, and heart. Cysticercosis can be diagnosed on serology, and radiologically but confirmatory diagnosis is based on histopathological examination of the involved tissue biopsy specimen. Fine needle aspiration cytology is a useful low-cost outpatient procedure tool for preoperative diagnosis of cysticercosis and is absolutely essential for diagnosis of the parasitic lesions in a peripheral hospital, one like ours.

<p class="abstract"><strong>Background:</strong> Diseases of the thyroid continue to be a common clinical problem having a prevalence rate of 4 to 7% in the general population.</p><p class="abstract"><strong>Methods:</strong> This retrospective study was conducted in the Department of ENT, Head and Neck Surgery, SMGS Hospital, Jammu from January 2018 to January 2020. A thorough history taking, meticulous clinical examination, routine hematological and biochemical investigations were done in all the patients. General examination of the patient was done and looked for thyroid functional abnormality followed by local examination was carried out to locate and identify the site of the swelling, shape, size and consistency of the thyroid swelling and clinical diagnosis was made. All thyroid swelling cases were subjected to fine needle aspiration cytology (FNAC). The type of thyroidectomy is based on the size and type of thyroid whether neoplastic or non-neoplastic. All the specimens were sent for histopathological examination.  </p><p class="abstract"><strong>Results:</strong> Out of 60 patients in the study, 56 were females and only 4 were males. The most common age group was 31 to 40 years seen in 24 (40%) patients. The most common presenting symptom was swelling in the neck seen in 58 (96.67%) patients. The most common diagnosis on FNAC was colloid goiter seen in 40 (66.67%) patients. The most common was benign nodular colloid goiter seen in 35 (58.33%) patients.</p><p class="abstract"><strong>Conclusions:</strong> Fine needle aspiration is a very useful and indispensible test in the diagnosis of thyroid lesions.</p>
