Objective: To assess the effect of injecting magnesium sulfate on Doppler parameters of fetal umbilical and middle cerebral arteries (MCA) in women with severe preeclampsia. Materials and Methods: A total of 21 patients with severe preeclampsia admitted to Imam Reza Hospital, Kermanshah (Iran), were evaluated. Before and after administration of magnesium sulfate, Doppler ultrasound scan was carried out to measure fetal middle cerebral artery and umbilical artery blood flow. Paired t-test was used for statistical analysis. Results: After injection of magnesium sulfate, the mean resistivity index (RI)-umbilical, and pulsatility index (PI)-cerebral showed a statistically significant reduction (P < 0.001). The cerebroumbilical C/U ratio increased after the intervention (P < 0.001). The PI-umbilical (P = 0.1) and pre- and post-RI-cerebral (P = 0.96) did not have statistically significant changes. Conclusions: Infusion of magnesium sulfate significantly decreases the flow in the fetus RI-umbilical and PI-MCA, and it increases C/U ratio indices in color Doppler ultrasound.

Fetal hemodynamic changes can occur immediately following invasive chorionic villus sampling (CVS). We decided to study the possible effect of CVS on fetal heart rate (FHR), and uteroplacental resistance index (RI) changes using color Doppler ultrasound. Thirty-five pregnant patients with a gestational age of more than 12 weeks were included. Trans-abdominal CVS was done to assess the possibility of thalassemia. Before and after the CVS, color Doppler ultrasound was done to measure FHR and uteroplacental RI. Mean (SD) values for FHR before and after the CVS were 175.22 (±9) and 173.62 (±9.94) beats per minute, respectively; P=0.18. Mean (SD) uteroplacental RI before the CVS was 0.79 (0.07) which significantly increased to 0.82 (0.08); P=0.03. We observed a significant increase in resistance of blood flow in placental circulation after CVS. However, no significant change was observed regarding FHR after CVS.

The neural network algorithm of deep learning was applied to optimize and improve color Doppler ultrasound images, which was used for the research on elderly patients with chronic heart failure (CHF) complicated with sarcopenia, so as to analyze the effect of the deep-learning-based color Doppler ultrasound image on the diagnosis of CHF. 259 patients were selected randomly in this study, who were admitted to hospital from October 2017 to March 2020 and were diagnosed with sarcopenia. Then, all of them underwent cardiac ultrasound examination and were divided into two groups according to whether deep learning technology was used for image processing or not. A group of routine unprocessed images was set as the control group, and the images processed by deep learning were set as the experimental group. The results of color Doppler images before and after processing were analyzed and compared; that is, the processed images of the experimental group were clearer and had higher resolution than the unprocessed images of the control group, with the peak signal-to-noise ratio (PSNR) = 20 and structural similarity index measure (SSIM) = 0.09; the similarity between the final diagnosis results and the examination results of the experimental group (93.5%) was higher than that of the control group (87.0%), and the comparison was statistically significant ( P < 0.05 ); among all the patients diagnosed with sarcopenia, 88.9% were also eventually diagnosed with CHF and only a small part of them were diagnosed with other diseases, with statistical significance ( P < 0.05 ). In conclusion, deep learning technology had certain application value in processing color Doppler ultrasound images. Although there was no obvious difference between the color Doppler ultrasound images before and after processing, they could all make a better diagnosis. Moreover, the research results showed the correlation between CHF and sarcopenia.

Abstract Background Preeclampsia is pregnancy-specific syndrome of reduced organ perfusion secondary to vasospasm and endothelial activation. Aim of the Work to assess the effect of injecting magnesium sulfate on Doppler parameters of uterine artery, umbilical and fetal middle cerebral arteries (MCA) in women with severe preeclampsia. Patients and Methods This study included eighty pregnant with severe preeclampsia or eclampsia who were given magnesium sulphate therapy in Ain Shams University Maternity hospital. The Study group included 80 pregnant patients Doppler flow velocity parameters (resistance index [RI], pulsatility index [PI] and systolic/diastolic [S/D] ratio) were evaluated in the uterine, fetal umbilical and middle cerebral arteries before and 20 minutes after intravenous administration of 6 grams of magnesium sulfate (Loading dose). Results Magnesium sulfate had significant hemodynamic effects with significant reduction of systolic and diastolic blood pressure. Regarding the Doppler studies, it was found that after administration of magnesium sulfate there was significant reduction in the Doppler velocimetry parameters (Resistance index[RI], Pulsatility index[PI] and systolic/diastolic[S/D] ratio) in the uterine, umbilical and fetal middle cerebral arteries denoting decreased vascular resistance with increased blood flow in these vessels. Conclusion Intravenous administration of magnesium sulfate in pregnant women with severe preeclampsia and eclampsia resulted in a decrease in uterine artery, fetal umbilical and middle cerebral arteries Doppler indices with reduced resistance to blood flow in these vessels.

<b><i>Objective: </i></b>Scrotal ultrasound is not a routine investigation in the clinical approach to male infertility analysis. This study aims to identify the role of testicular Doppler ultrasound in male infertility assessment and its relation to semen parameters in non-azoospermic men. <b><i>Methods: </i></b>Cross-sectional descriptive analysis of 558 men from infertile couples were examined at the Hue Center for Reproductive Endocrinology and Infertility, Hue University Hospital from June 2016 to May 2018. Some cohort characteristics, semen analysis and testicular Doppler ultrasound were analyzed. Men with acute systemic diseases, acute urinary tract infection, hepatic dysfunction, malignant diseases, retrograde ejaculation, cryptorchidism or azoospermia were excluded. <b><i>Results: </i></b>The mean volumes of the right and left testicles were 8.87 and 8.77 ml, respectively. The total volume of the 2 sides was 17.63 ± 4.34 ml (95% confidence interval 17.27-18.00 ml). The mean right resistive index (RI) was 0.61 ± 0.23, and the mean left RI was 0.59 ± 0.01. The rate of normal semen quality was 23.2% in group with varicocele and 30.6% in group with non-varicocele. The ultrasound results from the normal semen group were much different from those of the abnormal semen group regarding testicular volume: mean right testis volume: 9.67 ± 1.88 vs. 8.75 ± 2.34 ml, p = 0.0096; mean left testis volume: 9.54 ± 1.78 vs. 8.51 ± 2.44 ml, p = 0.0047; mean total volume of 2 sides: 19.21 ± 3.60 vs. 17.26 ± 4.59 ml, p = 0.005 (varicocele group); mean right testis volume: 9.21 ± 2.21 vs. 8.63 ± 2.21 ml, p = 0.029 (non-varicocele group). The other indexes of color Doppler ultrasound (peak systolic velocity, end diastolic velocity, RI) were not found to correlate with semen quality. <b><i>Conclusions: </i></b>Testicular volume which has a close relation to the semen parameters could be used as a clinical prediction factor for the quality of semen.

✓ Bilateral measurements of regional cortical blood flow (CBF) and the diameter of superficial cortical arteries were made before and after puncture of the right middle cerebral arteries (MCA) of 11 cats with bilateral craniectomies. The CBF was decreased in the right cerebral hemispheres before puncture, probably because of manipulation and exposure of the MCA. Decreases of CBF occurred after MCA puncture in the contralateral cerebral hemispheres of five of seven animals without subarachnoid blood over the convexities of the hemispheres. The mean CBF value for the contralateral hemispheres was significantly lower after puncture than before. There was no consistent relationship between CBF and the calibers of surface cortical arteries. Thus, basal subarachnoid bleeding can cause decreases of CBF of the cerebral hemispheres, probably because of basal arterial spasm associated, at times, with vasomotor paralysis and failure of autoregulatory responses. If combined with increases of intracranial pressure, such decreases of CBF may be adequate to produce cerebral ischemia.
