The article presents data of economic efficiency of simultaneous operations in women with combined surgical and extra-gynecological pathology, allowing rational use of material and technical base of medical institutions, and conducted simultaneous operations leading to significant financial savings, both in hospital and at the outpatient . The objective: to determine the clinical and economic effectiveness of simultaneous operations in women with combined extra-gynecological and surgical pathology operated in the department of operative gynecology control IPOG (Institute of Pediatrics, Obstetrics and Gynecology) NAMS of Ukraine for 2015 and modified using special formulas. Patients and methods. The study laid out a retrospective analysis of the immediate results of surgical treatment of 40 patients department of operative gynecology control IPOG NAMS Ukraine. The calculation was performed on the following parameters: value of preoperative examination, providing perioperative medical support, operational support, anesthesia and postoperative laboratory monitoring. Results. Analysis of the research shows that simultaneous operation in two to three times shortens the patient’s stay in hospital and the duration of temporary disability, three times increases the cost-effectiveness of treatment and improve the quality of life of patients. To increase the profitability of the surgical departments recommended minimum break-even calculate the intensity of operations, which is the base for operational planning department. Conclusion. Conducting simultaneous operations in women with combined extra-gynecological and surgical pathology can effectively reduce the cost of surgical treatment of patients with no significant prolongation of the transaction period and the postoperative period. Simultaneous operations may be performed in specialized hospitals with modern equipment available and a multidisciplinary team of surgeons in identifying women with two or more diseases requiring surgical correction. The growing spread of combined pathology, especially among the elderly and geriatric population, requires updating of approaches to diagnosis and treatment. In particular, surgical practice has encountered high incidences of combined diseases, requiring operative treatment, in patients with variously localized hernias. In such cases the standard common approach is to treat in stages gradually eliminating certain diseases. But so far, there has been evidence of effective concurrent treatment of combined surgical pathology by conducting simultaneous operations. This review presents the studies of the direct results of simultaneous operations during variously localized hernia plastic repair in combination with other abdominal surgical pathology.

The article presents data on the influence of medical and socio - psychological factors on changes in the psychological status of patients who underwent simultaneous surgical interventions. The objective: to determine the features of the psychological status and quality of life in the postoperative period in women who underwent simultaneous surgical interventions. Patients and methods. To solve the tasks, 110 patients were surgically assisted in the department of operative gynecology of the State Institution «IPAH of the National Academy of Medical Sciences of Ukraine». They were divided into two groups. The first group consisted of patients with isolated gynecological pathology (n=83), the second group consisted of simultaneous pathology (n=27). Patients with isolated surgical pathology (n=26) were included in the study, surgical care was provided in the surgical department of the City Clinical Hospital No. 10 in Kiev. They made up the third group. Results. According to the data received, the majority of patients underwent psychological and psychological influences on their psychological state, but the frequency and level of influence were different. Women (group II) operated on the simultaneous pathology were much more sensitive to the effect of socio - psychological factors, in relation to patients of groups I and III with isolated gynecological and surgical pathology, respectively, in whom the negative influence of medical and socio - psychological factors was revealed. Conclusion. The results of the research prove that the violations of the psychological status (in particular, anxious and depressive conditions) in the patients of the study groups who underwent operative interventions in a large percentage of cases depend not only on medical, but mainly on the socio-psychological factors of influence, which in the future will Is necessarily taken into account when developing measures for psychological correction in this patient population. Simultaneous operations in case of laparoscopic cholecystectomy constitute 0.5-16% of the total surgery. The aim of the study was to analyze the results of laparoscopic simultaneous cholecystectomies combined with surgical pathology. Among the 216 patients, 58 (26.85%) cases of simultaneous operations were performed with combined access and 158 (73.15%) cases of simultaneous operations performed in open way. The study is indicative of reasonability of simultaneous operations in case of combined surgical pathology. Severe specific complications are prevented, a positive medical and social effect and efficiency is presents as compared to performing two or three separate surgeries.
