Monitoring of patients with microdialysis following pancreaticoduodenectomy—the MINIMUM study: study protocol for a randomized controlled trial

Background Postoperative pancreatic fistula after pancreatoduodenectomy is a much-feared complication associated with substantial mortality and morbidity. The current standard for diagnosing postoperative pancreatic fistula, besides routine clinical examination, include radiological examinations, analysis of pancreatic drain amylase activity, and routine blood samples. Another promising method is by intraperitoneal microdialysis to monitor intraperitoneal metabolites measured at the pancreaticojejunostomy, thereby detecting what occurs locally, before chemical events can be reflected as measurable changes in systemic blood levels. Methods The MINIMUM study is a prospective, randomized, controlled, single center enrolling 200 patients scheduled for open pancreatoduodenectomy comparing the microdialysis method to the “standard of care.” Half of the included patients will be randomized to receive an intraperitoneal microdialysis catheter implanted at the end of surgery and will be monitored by microdialysis as an additional monitoring tool. The other half of the patients will not receive a microdialysis catheter and will be monitored according to the current standard of care. The primary objective is to evaluate if the microdialysis method can reduce the total length of stay at the hospital. Secondary endpoints are the frequency of complications, length of stay at the hospital at our institution, catheter malfunction, number of infections and bleeding episodes caused by the microdialysis catheter, patient-reported quality of life and pain, and cost per patient undergoing pancreatoduodenectomy. The patients will be randomized in a 1:1 ratio. Discussion Intraabdominal microdialysis could potentially reduce morbidity and mortality after pancreatoduodenectomy. Furthermore, there is a great potential for shortening the in-hospital length of stay and reducing the financial aspect considerably. This study may potentially open the possibility for using microdialysis as standard monitoring in patients undergoing pancreatoduodenectomy. The hypothesis is that the microdialysis method compared to “standard care” will reduce the total length of hospital stay. Trial registration Clinicaltrials.gov (NCT03631173). The Effect of Microdialysis Catheter Insertion on Glutamate and Serotonin Levels in Masseter Muscle in Patients with Myofascial Temporomandibular Disorders and Healthy Controls

Myofascial temporomandibular disorders (TMD) are the most common cause of chronic pain in the orofacial region. Microdialysis has been used to study metabolic changes in the human masseter muscle. The insertion of the microdialysis probe causes acute tissue trauma that could affect the metabolic milieu and thereby influence the results when comparing healthy subjects to those with TMD. This study aimed to investigate the levels of serotonin and glutamate during the acute tissue trauma period in healthy subjects and in patients with TMD. Microdialysis was carried out in 15 patients with TMD and 15 controls, and samples were collected every 20 min during a period of 140 min. No significant alterations of serotonin or glutamate were observed over the 2 h period for the healthy subjects. For the TMD group, a significant decrease in serotonin was observed over time (p < 0.001), followed by a significant increase between 120 and 140 min (p < 0.001). For glutamate, a significant reduction was observed at 40 min compared to baseline. The results showed that there was a spontaneous increase of serotonin 2 h after the insertion of the catheter in patients with TMD. In conclusion, the results showed that there are differences in the masseter muscle levels of serotonin and glutamate during acute nociception in patients with myofascial TMD compared to healthy subjects.

Subcutaneous microdialysis in the awake rat.

Background. Microdialysis is a valued method to study the pharmacokinetics of drug distribution from an injection site or to a tumor site. The objective of this study was to assess the feasibility of prolonged use of subcutaneously implanted microdialysis catheters to assess local retention and systemic uptake of carboplatin in rats post anesthetic recovery. Methods. Twelve rats were used: one control rat and 11 treatment rats. The control rat was used to outline the surgical implantation technique of the microdialysis catheter. Treatment rats were administered 5mg carboplatin in poloxamer 407 and divided in two treatment cohorts. Microdialysis catheters were used in both cohorts to collect extracellular fluid. Placement technique was modified between the two cohorts. Treatment cohort 1 (n=6) had the microdialysis catheter implanted in the wound bed, sutured in place and tunneled subcutaneously along the dorsum to exit dorsally in the cervical area. The ends were secured to a Velcro strip sutured to the dorsal cervical skin to avoid trauma by the rat. Treatment cohort 2 (n=5) had the catheter looped around the right hind limb, with the functional part sutured in place in the wound bed, and tunneled through a subcutaneously implanted sterile intravenous line exiting dorsally in the cervical area. The ends were maintained within the IV line and held in place with wound clips. Results. All 11 microdialysis catheters in the treatment cohorts were functional immediately after implantation, but only one out of 11 microdialysis catheters in the treatment cohorts was functional at 24hrs, and none at 48hrs. Discussion. The results of this study suggest that microdialysis catheters cannot be maintained in unanesthetized rats without further modification of the implantation protocol.

Μελέτη της τοπικής εγκεφαλικής αιματικής ροής με τη μέθοδο της Thermal Doppler Flowmetry και συσχέτιση των τιμών της με το κυτταρικό μικροπεριβάλλον (μέσω των τιμών της εγκεφαλικής μικροδιάλυσης) σε υπαραχνοειδή αιμορραγία

Εισαγωγή. Η παρακολούθηση της εγκεφαλικής μικροδιάλυσης επιτρέπει την αξιολόγηση της τοπικής εγκεφαλικής μεταβολικής φυσιολογίας και παρέχει βιοδείκτες σε κρίσιμες καταστάσεις, όπως η υπαραχνοειδής αιμορραγία (ΥΑΑ).Σκοπός Ο σκοπός της παρούσας μελέτης ήταν να διερευνηθεί η συσχέτιση μεταξύ της περιοχικής αιματικής εγκεφαλικής ροής και των παραμέτρων της μικροδιάλυσης (οι συγκεντρώσεις γλυκόζης, γαλακτικού, γλυκερόλης και πυροσταφυλικού και η μεταβολική αναλογία γαλακτικού/πυροσταφυλικού) σε ασθενείς με υπαραχνοειδή αιμορραγία.Μεθοδολογία Eίκοσι ένα ασθενείς με υπαραχνοειδή αιμορραγία συμπεριλήφθησαν στην παρούσα προοπτική μελέτη.Εισήχθη ένα ελάχιστα επεμβατικό, πολυπαραμετρικό σύστημα νευροπαρακολούθησης κατά τις πρώτες 8 ώρες μετά την εισαγωγή στη ΜΕΘ.Έγινε κρανιοανάτρηση στο μετωπιαίο οστό, στο σημείο Kocher υπό άσηπτες συνθήκες όπου τοποθετήθηκε ένα τρικαναλικό bolt kit που χρησιμοποιήθηκε για την εισαγωγή: (α) ενός καθετήρα μέτρησης ενδοκράνιας πίεσης (ICP) (Codman microsensor kit, Codman, Johnson & Johnson, Raynham. ΜΑ, USA), (β) ενός καθετήρα μέτρησης οξυγόνου του εγκεφαλικού ιστού (PbrO2 catheter, Licox, Integra Neurosciences. Plainsboro. NJ. USA) και (γ) ενός καθετήρα εγκεφαλικής μικροδιάλυσης (CMA 70 Brain Microdialysis Catheter, 10 mm membrane length, 20 kDa cut off, CMA. Stockholm, Sweden).Έγινε δεύτερη κρανιοανάτρηση, 1-1.5 cm πρόσθια από την προηγούμενη (προς αποφυγή επιμόλυνσης των δύο πεδίων), για την εισαγωγή καθετήρα μέτρησης της εγκεφαλικής αιματικής ροής (CBF) (Hemedex, Bowman. Boston, MA, USA). Καταγράφηκαν οι μετρήσεις της εγκεφαλικής αιματικής ροής (βασιζόμενη στη μεθοδολογία της θερμικής διάχυσης), ο θερμικός συντελεστής Κ και η μικροδιάλυση των εγκεφαλικών βιοχημικών δεικτών. Όλοι οι ασθενείς υποβλήθηκαν σε θεραπεία με πρωτόκολλο κατευθυνόμενο από την πίεση εγκεφαλικής αιμάτωσης (CPP).Η διάρκεια της παρακολούθησης του εγκεφάλου και καταγραφής των τιμών ήταν 10 ημέρες.Αποτελέσματα•Η εγκεφαλική θερμοκρασία βρέθηκε να είναι υψηλότερη στους επιζώντες (36,83 oC έναντι 36,48 oC στους μη επιζώντες).•Η CBF παρατηρήθηκε υψηλότερη στους επιζώντες (33,66 έναντι 29,34 ml/ 100gr/min στους μη επιζώντες).•Οι τιμές του γαλακτικού οξέος βρέθηκαν να είναι υψηλότερες στους επιζώντες (8,52 mmol/L έναντι 5,89 mmol/L για τους επιζώντες και μη επιζώντες αντίστοιχα).•Η γλυκόζη, γλυκερόλη και πυροσταφυλικό βρέθηκαν σε υψηλότερες τιμές στους μη επιζώντες σε σύγκριση με τους επιζώντες (1,18 mmol/L αντί 0,99 mmol/L, 122,92 μmol/L έναντι 92,29 μmol/L και 124,47 μmol/L αντί 112,06 μmol/L, αντίστοιχα).•Οι μη επιζώντες είχαν αυξημένο το λόγο L/Ρ και το συντελεστή Κ σε σύγκριση με τους επιζώντες (L/P: 50,01 ± 24,79 αντί 39,36 ± 9,57, K: 3,34 ± 0,69 αντί 3,02 ± 0,35, αντίστοιχα). Οι διαφορές δεν έφθασαν το επίπεδο στατιστικής σημαντικότητας (p>0,10).•Όσον αφορά τις συσχετίσεις των παραμέτρων της μικροδιάλυσης με τη CBF, την Κ, και την Τ, αλλά και μεταξύ τους, οι συσχετίσεις διαφοροποιούνται ανάλογα με το επίπεδο της CBF.Στο σύνολο του δείγματος: •Ο συντελεστής Κ συσχετίστηκε θετικά με τη CBF, την Τ, τη γλυκόζη και αρνητικά με το πυρουβικό (GEE) (p<0,05).•Η αναλογία L/P συσχετίστηκε θετικά με τη γλυκερόλη και αρνητικά με τη CBF (p<0,001).ΣυμπεράσματαΟι βιοχημικοί δείκτες κυτταρικού στρες και θανάτου (γλυκερόλη, L/P) σχετίζονται θετικά μεταξύ τους. Οι τιμές Κ δεν θα πρέπει να θεωρούνται ως σταθερές όπως πιστευόταν παλαιότερα, ιδιαίτερα σε ασθενείς σε κρίσιμη κατάσταση. Ο συντελεστής Κ μπορεί να μεταβάλλεται καθώς εξελίσσεται η εγκεφαλική βλάβη και αλλάζουν οι ιδιότητες των ιστών. Οι ιατροί οφείλουν να είναι γνώστες της πολύπλοκης σχέσης μεταξύ του συντελεστή Κ και των μετρήσεων της CBF για την ακριβή ερμηνεία όλων των παραμέτρων νευροπαρακολούθησης, και κατά συνέπεια, της διαχείρισης των ασθενών
