Introduction: Concerning the poor clinical outcomes in cases of acute myocardial infarction (AMI) with bifurcation lesions undergoing primary percutaneous coronary intervention (PCI), we described a successful application of modified flower petal technique and called it the REVERSE modified flower petal technique in a patient with AMI. Case Presentation: A 56-years-old man who suffered from chest pain was admitted to the emergency ward. After clinical examination, the primary diagnosis was acute anterolateral MI. Therefore, PCI on left anterior dimension (LAD) and diagonal arteries using modified flower petal stenting was performed in the side branch, which we called it REVERSE modified flower petal. Conclusions: Modified flower petal stenting was performed by the REVERSE mechanism as described in the report of the new intervention (2.5 × 18). The follow-up after 24 months by the exercise stress test using the Bruce protocol supported the success of the intervention.

Abstract. Ventricular Septal Rupture (VSR) is a serious complication of acute myocardial infarction. This incidence continues to decline with the thrombolytic and PCI era, but the mortality rate is still high. VSR occurs most frequently after anterior AMI. VSR can occur in the apical, mid or basal of the septum. The most common location is in the apical portion of the septum. The timing of VSR closure surgery depends on the state of the patient's hemodynamic and VSR diameter. Various studies have suggested that VSR closure concomitant with Coronary Artery Bypass Grafting (CABG) provide a better outcome.

Emergency is a condition in which someone needs immediate treatment or help because if he does not get first aid quickly it will threaten his soul or cause permanent disability. Patients of emergency are brought to Emergency Installation. Emergency Installation is the spearhead of the hospital where all incoming patients will be handled on this unit. The scope of emergency services is to conduct a primary survey, such as airway, breathing, circulation, disability and secondary survey. Goals:The purpose of this study is to find out the handling of Emergency patients at the Emergency Installation of Santa Elisabeth Hospital Medan in 2017 Methods:The research design used was descriptive. The population used was emergency patients, with sampling technique of pulposive sampling of 297 people. The criteria used were inclusion with the criteria of an acute myocardial infarction patient, head trauma, airway obstruction, pneumothorax, burns and shock Results:Technique used by researcher was documentation study with the results of the study showed male gender of 176 people (59.3%), age> 65 years were 98 people (33%) Handling of the case of the highest emergency patients was acute myocardial infarction 98 people (33%). Conclusion: The conclusion shows that male patients tend to suffer from heart disease. Age> 65 years is age that susceptible to disease because of the lack body system work such as heart
