Observation on therapeutic effect of magnetic acupuncture treatment of simple obesity complicated with hyperlipidemia

Abstract Background: The underlying neurological mechanism of acupuncture treatment in migraine without aura (MwoA) remains unclear. Therefore, we explored the dynamic alterations of intrinsic brain activity and effective connectivity in patients with MwoA after acupuncture treatment.Methods:The fMRI scans were separately obtained at baseline, after the first and after the 12th acupuncture sessions in 40 patients with MwoA. The acupuncture treatments were finished within 6 weeks as twice a week. 36 matched healthy controls (HCs) were recruited and performed once fMRI scan. The dynamic amplitude of low-frequency fluctuation (dALFF) and dynamic granger causality analysis(GCA) were used to analyze the difference of different time points in patients with MwoA. The correlation analyses were performed in dALFF variability, dynamic effective connectivity (DEC) variability with clinical variables in patients with MwoA.Results:Compared with HCs, Patients with MwoA at baseline showed decreased dALFF variability in regions within rostral ventromedial medulla (RVM), the superior lobe of left cerebellum (Cerebelum_Crus1_L), the right inferior frontal gyrus, triangular part (IFGtriang.R), the right median cingulate and paracingulate gyri (DCG.R), the right precuneus (PCUN.R), and the left inferior parietal, supramarginal and angular gyri (IPL.L), increased dALFF variability only in the left inferior occipital gyrus (IOG.L). After acupuncture treatment, the decreased dALFF variability of the RVM, Cerebelum_Crus1_L , and PCUN.R progressively recovered, the RVM revealed gradually increased DEC variability to the right middle frontal gyrus (MFG.R), the left insula (INS.L), the right precentral gyrus (PreCG.R), and the right supramarginal gyrus (SMG.R). And enhanced DEC variability from the right fusiform gyrus (FFG.R) to RVM. Furthermore, the increased DEC variability were found from Cerebelum_Crus1_L to the left middle occipital gyrus (MOG.L) and the left precentral gyrus (PreCG.L), from PCUN.R to the right thalamus (THA.R). These dALFF variabilities were significantly positive correlated with frequency of migraine attack and negative correlated with disease duration at baseline, dynamic GCA coefficients were significantly positive correlated with Migraine-Specific Quality of Life Questionnaire (MSQ) score, negative correlated with frequency of migraine attack and visual analog scale (VAS) score postacupuncture treatment.Conclusions:Our results provide insight into dynamic alterations from the perspective of dynamic local brain activity and effective connectivity for the understanding mechanisms of cumulative therapeutic effect of acupuncture in patients with MwoA.Trial registration: ChiCTR, ChiCTR1900023105. This paper systematically reviewed the clinical update of traditional Chinese exercises in the treatment of simple obesity in recent years and discussed their specific advantages in this aspect. This review focused on several typical traditional Chinese exercises, namely, Tai Chi, Ba Duan Jin, Yi Jin Jing, Wu Qin Xi, Shaolin Neigong, and Liu Zi Jue, which all showed clinical beneficial effect on the treatment of simple obesity with their own characteristics. To optimize the clinical therapeutic effect of these traditional Chinese exercises, we need to seek the most appropriate exercise or the combo exercise based on the characteristics of different obese population, to improve the efficiency of weight loss, reduce sports injury, and consolidate the therapeutic effect. In the future, we need to further evaluate the efficacy of sitting exercise, lying exercise, and static training in the treatment of simple obesity, subdivide the treatment population, and explore the working mechanism of these traditional Chinese exercises.

A previous study has shown that laser acupuncture is a useful healing method for the treatment of visceral postmenopausal obesity in combination with a low-calorie diet. We observe and evaluate the therapeutic effect of laser acupuncture in subjects of simple obesity with a non-restrictive diet protocol. Subjects included 73 women and 22 men with simple obesity and body mass indices ≥ 27 kg/m2. Daily energy intake recommendations for obese females and males were 1620.0 and 1894.2 kcal in average, respectively. The gallium aluminum arsenide Handylaser Trion was used to apply 0.25 J of energy to each of the following acupuncture points three times per week for four consecutive weeks: Stomach, Hunger, ST25, ST28, ST40, SP15, and CV9. The subjects' body weights and body mass indices were recorded before treatment, and four weeks after treatment, and the percent reduction in each parameter was calculated. Statistically significant reductions in body weight and body mass index were detected after four weeks of treatment. The mean reduction and mean percent reduction in body weight were 3.17 kg and 3.80% (p < 0.0001), respectively. The corresponding values for the body mass index were 1.22 kg/m2 and 3.78% (p < 0.0001), respectively. We concluded that laser acupuncture was found to exert a therapeutic effect on simple obesity by reducing both body weight and body mass index. Moreover, subjects showed good compliance with this comfortable and non-restrictive diet protocol.
