ABSTRACT Background and objectives The influence of the loss of teeth on the condylar position and on temporomandibular joint (TMJ) dysfunction syndrome remains a controversial issue. This study analyses the condylar position by means of a tomogram in partially dentate subjects which serves as a guide to predict which of the partially dentate statuses is prone to develop TMJ dysfunction syndrome in personnel without symptoms of the same. Methodology Eighty personnel were enrolled in this study consisting of Kennedy's class I, II, III, IV and control to analyze the condylar position by means of a tomogram. In all the five groups TMJ sectional tomogram programmed in a panoramic radiographic machine (PLANMECA PM 2002 CC PROLINE) was taken with the subjects at maximal intercuspal position and rest position. Tomograms were evaluated using linear measurements of the anterior and posterior intra-articular joint spaces on the basis of drawings and tracings. Results The results of the study revealed a predominance of reduced posterior condylar space in Kennedy's class I and II. A disparity was seen between the maximum intercuspal position and rest position, where the posterior joint space was reduced in the rest position. Conclusion Within the limitations of this study, it has been revealed that in Kennedy's classes I and II, for partially dentate personnel, a posterior displacement of the condyles was seen. This predisposition would suggest towards the necessity of restoring the missing dentition in order to maintain the harmony of the stomatognathic system. Background & Objectives:The study was conducted with objective to compare the horizontal condylar guidance (HCG) obtained by protrusive interocclusal records and panoramic radiographic images in completely edentulous patients.Materials & Methods:The horizontal condylar guidance was measured in 25 completely edentulous patients by protrusive interocclusal records using zinc oxide eugenol paste through a face bow transfer (HanauTM Spring bow, Whip Mix Corporation, USA) to a semi-adjustable articulator (HanauTM Wide-Vue Articulator, Whip Mix Corporation, USA).  In the same patients, HCG was traced in the panoramic radiograph. The angles formed by the intersection of two lines: Frankfurt’s horizontal plane and posterior slope of articular eminence was measured using protractor to represent the horizontal condylar guidance angle on each side.Results:The mean difference between the horizontal condylar guidance angles values obtained using protrusive interocclusal record and panoramic radiograph was 2.68 degrees and 3.40 degrees for the right and the left side respectively, with the panoramic radiograph values being higher. This difference between the values was found to be highly significant between the two methods for the right side (t = 2.70, p = 0.012) and left side (t = 3.69, p = 0.001). A significant positive correlation was found between the horizontal condylar guidance obtained from protrusive interocclusal record and panoramic radiograph for the right (r = 0.643, p = 0.001) and left sides (r = 0.622, p = 0.001) separately.Conclusion:The panoramic radiographic tracing can be used to calculate the mean horizontal condylar guidance in the completely edentulous patients and these values can be used to programme semi-adjustable articulators avoiding the cumbersome process of obtaining protrusive interocclusal records.

Background: Complete dentition is not always appropriate for old aged medically compromised and financially weak individuals. It is the dentist to decide what minimum number of teeth should be restored to have the proper oral function. The aim was to find out the opinion of prosthodon­tists of Nepal regarding the suitability of shortened dental arch as treatment modality and how frequently is this treatment choice for an old aged Nepalese patient. Methods: A pretested close ended questionnaire was usedin this study.After taking ethical clear­ance from Nepal Health Research Council, online questionnaire was sent to the participants along with the consent. Analysis was done using SPSS (version 21) statistical software package. Descrip­tive statistics was used. Percentage and frequencies were calculated. Results: Among respondents, 96.2% (n=51) had heard about shortened dental arch therapy but 3.8 % (n=2) had never heard about it. Forty-two (79.2%) do support shortened dental arch. Eleven (20.8%) do not favorshortened dental arch. Their main reason for replacing molars was to improve masticatory function.Instead of shortened dental arch,n=5(45.5%) prefer cast partial denture and 6(54.5%) advice implant supported fixed prosthesis. Among Prosthodontists who recommended shortened dental arch therapy, eight (19.1%) always advised patients not to replace molars. Twen­ty-four (57.1%) prosthodontists who support shortened dental arch therapy mentioned chewing function in shortened dental arch patients as satisfactory. Conclusions: Majority of Prosthodontists agree that shortened dental arch is a suitable treatment option for older people in developing countries like Nepal. But the treatment is not employed in clinical practice as required.

Abstract Introduction Understanding the effects of periodontal therapy is essential in performance and satisfaction of partially dentate patients. Objective To assess the effect of subgingival periodontal treatment in performance and masticatory satisfaction and ability as reported by partially dentate patients with untreated chronic periodontitis. Materials and method Patients were given nonsurgical periodontal treatment and masticatory satisfaction, ability, and performance were evaluated before and after treatment. Performance was assessed based on sieving chewed standardized chewable material, while satisfaction and ability were analyzed using a questionnaire based on OHIP-EDENT. Result Mean numbers of functional tooth units were 10.36 ± 1.43 anterior and 6.73 ± 0.79 posterior teeth. Masticatory performance did not vary significantly (p = 0.075), and mean particle sizes before and after treatment were 4.23 ± 1.84 mm and 3.90 ± 0.75 mm, respectively. Overall satisfaction and ability scores after treatment were higher (p > 0.05) compared with scores prior to the periodontal intervention. Conclusion Periodontal treatment improves masticatory satisfaction and ability with no effect on performance.

Improvements in oral health including increased retention of natural teeth have given rise to a partially dentate older population. Replacement of missing natural teeth is important to improve function, aesthetics and quality of life for this patient group. A variety of options are available to replace missing teeth in partially dentate older adults, including fixed, removable and implant retained prostheses. This article will discuss the provision of removable partial dentures including treatment planning and denture design. When planning removable partial dentures, careful attention must be paid to stabilising the patient prior to delivering any prosthesis. Partial dentures should be designed to minimise the potential for plaque accumulation with carefully designed metal based frameworks. Acrylic resin can also be utilised with attention to detail to minimise the risk of damage to delicate supporting tissues. Removable dentures have the advantage that they can be readily added to in the event of further tooth loss which may be particularly relevant for older adults. Partial dentures which optimise support, retention and stability can function very successfully and significantly improve patients’ oral health related quality of life.
