The last years of Pamela’s life were marked by further illness but also by a remarkable dedication to work. She was hospitalized several times for respiratory illnesses, but in 1974 she published a book of autobiographical essays, Important to Me, which covered such topics as memories of her father, her relationship with Dylan Thomas, her visits to the USSR, and her friendship with other writers such as Edith Sitwell. After months of undiagnosed pain, Snow died in 1980 of a perforated ulcer and Pamela died almost one year later of congestive heart failure and respiratory illness exacerbated by having smoked since the age of fourteen. Yet characteristically she worked courageously until the very end on a novel published posthumously: A Bonfire, which similarly to her first novel deals explicitly with sexual desire. Her ashes were scattered at Stratford-upon-Avon, a place she visited every year on Shakespeare’s birthday.

Here are represented our results according to making and clinical using of different TiNi devices for treatment of digestive system diseases. Constructions for perfoming of compression anastomoses, cliping of tubular structures during laparoscopic operations and perforated ulcer plastic. 130 criminal and civil cases had been maintained against physicians in Tomsk region within 1996—2002. There were pitfalls in diagnoses and wrong medical aid in 59 (45,4%) cases. Across diagnostic pitfalls, such as incomplete examination of patients, incomplete registration of laboratory results etc., there were methodological errors. It is known that diagnosis process proceeds in hypothesis form at all stages. While obtaining the history a physician is guided by suppositions arising during inquiring a patient. These suppositions are later verified in the process of an objective examination of the patient. Therefore the most frequent reasons of medical errors are the final diagnosis on the basis of the history and physicians’ inability to use the diagnostic suppositions while diagnosing at all stages. The majority of pitfalls has been made in diagnoses of appendicitis, stomach and duodenum perforated ulcer.

Perforated ulcers of the excluded stomach or duodenum are exceedingly rare in patients who have undergone Roux-en-Y gastric bypass surgery. The diagnosis of perforated ulcer after Roux-en-Y gastric bypass remains challenging as there is often absence of free air or contrast extravasation from the biliopancreatic limb. We present a patient with signs and symptoms of acute cholecystitis. Laparoscopic cholecystectomy was complicated by postoperative bile leak. EDGE procedure was performed to access the remnant stomach and endoscopic evaluation revealed a perforated ulcer in the posterior duodenal bulb. Although unusual, in patients with bariatric surgery and upper abdominal pain, differential diagnosis including perforated ulcer of the biliopancreatic limb must be considered and early surgical exploration is essential.
